Baclofen Side Effects: Common, Serious, and Rare

Baclofen is a muscle relaxant that works by calming nerve activity in the brain and spinal cord. Its most common side effects are drowsiness, dizziness, nausea, confusion, and weakness. Most people tolerate it well once their body adjusts, but the drug carries real risks if stopped abruptly or taken in excess, and certain groups need to be especially careful.

How Baclofen Causes Side Effects

Baclofen activates a specific type of receptor in the brain and spinal cord that normally responds to GABA, one of the body’s main calming chemicals. When these receptors are switched on, they reduce the release of excitatory signals between nerve cells and make neurons less likely to fire. That’s what relaxes tight muscles, but it also slows down brain activity more broadly, which is why so many of baclofen’s side effects involve sedation, confusion, and coordination problems.

Common Side Effects

The side effects most people experience tend to appear early, often within the first days or weeks of starting the medication. They include:

  • Drowsiness and fatigue: the most frequently reported effect, sometimes described as unusual tiredness or weakness
  • Dizziness or lightheadedness, especially when standing up quickly from a seated or lying position
  • Nausea
  • Headache
  • Confusion
  • Constipation
  • Increased urination
  • Sweating
  • Trouble sleeping

For many people, these effects ease as the body adapts over a couple of weeks. Taking baclofen with or just after food can help reduce nausea and stomach discomfort.

Less Common Side Effects

A smaller number of people experience diarrhea, loss of appetite, muscle or joint pain, numbness or tingling in the hands or feet, stomach pain, a stuffy nose, or weight gain. These are generally mild but worth tracking, especially if they persist or worsen over time.

Side Effects That Need Medical Attention

Some reactions are uncommon but more concerning. These include mood changes or depression, hallucinations (seeing or hearing things that aren’t there), slurred speech, a pounding heartbeat, chest pain, fainting, ringing in the ears, skin rash or itching, and swelling in the ankles. A false sense of well-being or unusual excitement can also occur, which may be easy to overlook as a “side effect” since it doesn’t feel unpleasant, but it signals that the drug is affecting your brain in ways worth monitoring.

Clumsiness, unsteadiness, or trembling can develop as well. If you notice problems with coordination or muscle control that feel new or different from your underlying condition, that’s worth flagging to your prescriber.

Baclofen and Alcohol

Baclofen and alcohol affect the brain in strikingly similar ways. Both produce dizziness, unsteady gait, confusion, mood changes, drowsiness, and impaired attention and memory. Combining them amplifies all of these effects because they’re both depressing the same neural pathways. The overlap is so strong that the two substances can even produce similar withdrawal syndromes, including hallucinations, delirium, and seizures. Drinking while taking baclofen significantly raises your risk of dangerous sedation and impaired breathing.

Risks for Older Adults

Older adults are more sensitive to baclofen’s effects on the central nervous system. Confusion and dizziness hit harder in this age group, and the lightheadedness that comes with standing up too quickly creates a real fall risk. Falls in older adults can lead to fractures and hospitalizations, so the drowsiness and unsteadiness that might be merely inconvenient for a younger person become genuinely dangerous. Lower starting doses and slower increases are typical for people over 65.

Kidney Function and Toxicity Risk

Baclofen is cleared from the body primarily through the kidneys. When kidney function is reduced, the drug builds up in the bloodstream and can reach toxic levels even at standard doses. Research on patients with severely impaired kidney function (filtration rates below 30 mL/min) found that much lower doses were needed to avoid neurological side effects. Patients on dialysis tolerated only about 2.5 mg, while those with advanced kidney disease who weren’t on dialysis did better at around 7.5 mg. There’s no universally agreed-upon dose adjustment chart, which makes close monitoring essential for anyone with kidney problems.

What Happens in Overdose

Baclofen overdose is a medical emergency. Early signs include nausea, vomiting, confusion, and extreme sleepiness. As toxicity worsens, it progresses to coma, dangerously slow breathing, low heart rate, low blood pressure, loss of muscle tone, dilated pupils, and abnormally low body temperature. Seizures can occur. In severe cases, the coma can be so deep that it mimics brain death and may persist for several days. There’s no readily available blood test that helps guide treatment in real time, so diagnosis depends on knowing that baclofen was taken and recognizing the pattern of symptoms.

Why You Should Never Stop Suddenly

Abruptly stopping baclofen, whether oral or delivered through a pump, can trigger a withdrawal syndrome that ranges from uncomfortable to life-threatening. Symptoms include restlessness, insomnia, confusion, hallucinations, seizures, fever, worsening spasticity, and in some cases psychotic or manic states. Involuntary movements and dysfunction of the body’s automatic systems (heart rate, blood pressure, temperature regulation) have been reported after sudden discontinuation of high doses.

This applies to children as well. Stopping baclofen suddenly in a child can cause increased anxiety, behavioral changes, seizures, high temperature, and severe muscle spasms. If baclofen needs to be discontinued, the dose should be reduced gradually over several weeks.

Intrathecal Pumps vs. Oral Baclofen

Some people receive baclofen through a small pump surgically implanted under the skin that delivers the drug directly into the fluid around the spinal cord. This approach generally produces fewer systemic side effects than oral baclofen because it uses much smaller doses and targets the drug more precisely. However, pump-related complications are unique to this method: if the pump malfunctions or the catheter kinks, you can experience sudden withdrawal. If too much drug is delivered, overdose symptoms like drowsiness, difficulty breathing, low body temperature, seizures, and loss of consciousness can develop quickly.

Side Effects in Children

Children taking baclofen for conditions like cerebral palsy experience a similar side effect profile to adults. Drowsiness, nausea, and dizziness are the most common issues, and they typically show up when the medication is first started. Signs of too much baclofen in a child include excessive sleepiness, trouble breathing, and loss of consciousness. Giving the medication with food helps with tolerability, and any dose changes should be made slowly under medical guidance.
