Bactrim, the brand name for the fixed combination of trimethoprim and sulfamethoxazole (TMP-SMX), covers a surprisingly broad range of bacteria and even some non-bacterial pathogens. Its spectrum spans many common gram-positive and gram-negative organisms, and it holds a unique niche as first-line therapy for a handful of infections where few other oral antibiotics work well. That breadth, though, comes with clinically important gaps and a resistance picture that has shifted over the past two decades.
How Bactrim Works
The two components of Bactrim attack consecutive steps in the same metabolic pathway that bacteria use to make folate, a vitamin essential for building DNA. Sulfamethoxazole blocks an early enzyme that produces a folate precursor, while trimethoprim blocks the next enzyme down the line, the one that converts dihydrofolate into its active form, tetrahydrofolate.1PubMed Central. Mutual potentiation drives synergy between trimethoprim and sulfamethoxazole Hitting two points in the same pathway is what makes the combination more potent than either drug alone. Humans get their folate from food rather than synthesizing it, which is the basic reason TMP-SMX can poison the bacterial pathway without doing the same to human cells, though that selectivity is not absolute, as we will see later.
Gram-Positive Coverage
Bactrim’s gram-positive sweet spot is Staphylococcus aureus, including methicillin-resistant strains (MRSA). This makes it one of the relatively few oral options for outpatient MRSA infections. In a study of skin and soft tissue infections caused by MRSA, about three-quarters of patients treated with TMP-SMX at the standard dose achieved clinical resolution, and doubling the dose did not improve that rate.2PubMed Central. Dose of trimethoprim-sulfamethoxazole to treat skin and skin structure infections caused by methicillin-resistant Staphylococcus aureus For uncomplicated skin infections more broadly, including abscesses and cellulitis, TMP-SMX performed on par with clindamycin in a large randomized trial, with cure rates around 80% for both drugs.3PubMed Central. Sulfamethoxazole-Trimethoprim (Cotrimoxazole) for Skin and Soft Tissue Infections Including Impetigo, Cellulitis, and Abscess
Beyond staph, Bactrim has activity against many other gram-positive organisms. It covers most strains of Streptococcus pneumoniae, though resistance varies by region. Its role against group A streptococcus (GAS) is more debated. Some infectious-disease physicians remain cautious about relying on TMP-SMX for pure streptococcal infections like classic non-purulent cellulitis, but randomized data have shown comparable outcomes even in settings where GAS could be the cause.4PubMed Central. Sulfamethoxazole-Trimethoprim (Cotrimoxazole) for Skin and Soft Tissue Infections Including Impetigo, Cellulitis, and Abscess The observational and trial evidence taken together supports TMP-SMX for impetigo, purulent cellulitis, abscesses, and wound infections without needing to add a beta-lactam.5PubMed Central. Sulfamethoxazole-Trimethoprim (Cotrimoxazole) for Skin and Soft Tissue Infections Including Impetigo, Cellulitis, and Abscess
The Enterococcus Trap
One of the most clinically dangerous blind spots in Bactrim’s gram-positive coverage is Enterococcus. Standard lab testing can show enterococci as susceptible to TMP-SMX, but the lab result lies. Enterococci have a workaround: they can scavenge pre-formed folates from the body’s tissues, completely sidestepping the pathway that Bactrim blocks. Case reports have documented patients with enterococcal urinary tract infections who were treated with TMP-SMX based on apparently favorable lab results, only to develop bacteremia. They recovered only after switching to different antibiotics.6PubMed. In vivo v in vitro susceptibility of enterococcus to trimethoprim-sulfamethoxazole. A pitfall
The underlying problem is that standard testing media in the lab lack the folates that are present in human tissue and urine. When susceptibility testing is done in media supplemented with urine, the amount of drug needed to kill enterococci jumps dramatically, by as much as 360-fold.7Journal of Antimicrobial Chemotherapy. Trimethoprim and enterococci in urinary tract infections: new perspectives on an old issue This is a textbook example of why “susceptible on the report” does not always translate to “works in the patient.” If a urine culture grows Enterococcus, TMP-SMX should not be used regardless of what the sensitivity panel says.
Gram-Negative Coverage and Rising Resistance
For decades, TMP-SMX was a go-to antibiotic for uncomplicated urinary tract infections because it covers Escherichia coli and many other common gram-negative urinary pathogens. It also covers Klebsiella, Proteus mirabilis, and several other Enterobacteriaceae. That coverage remains valid in many cases, but resistance has eroded its reliability. In one study of E. coli isolates from young women with UTIs, roughly 30% were resistant to TMP-SMX.8PubMed Central. Antibiotic resistance in urinary isolates of Escherichia coli from college women with urinary tract infections Resistance rates vary geographically, and many guidelines now recommend checking local antibiograms before choosing TMP-SMX empirically for a UTI. In areas where resistance exceeds about 20%, other agents may be preferred as first-line therapy.
Beyond the urinary tract, Bactrim covers a handful of gram-negative respiratory and enteric pathogens. It has activity against Haemophilus influenzae and Moraxella catarrhalis, which makes it an option for some upper and lower respiratory tract infections. It also retains coverage against several Salmonella and Shigella species, though again resistance is climbing in many parts of the world.
Stenotrophomonas Maltophilia and Other Difficult Gram-Negatives
If there is one infection where Bactrim truly shines, it is Stenotrophomonas maltophilia. This gram-negative organism is intrinsically resistant to many broad-spectrum antibiotics, including carbapenems, making it a headache in hospital settings. TMP-SMX has long been the drug of choice for Stenotrophomonas infections, and it remains so today.9PubMed Central. Antimicrobial Treatment Strategies for Stenotrophomonas maltophilia: A Focus on Novel Therapies Fluoroquinolones, especially levofloxacin, are sometimes used as alternatives, but TMP-SMX is still regarded as the standard first-line option.10PubMed Central. Monotherapy with fluoroquinolone or trimethoprim-sulfamethoxazole for treatment of Stenotrophomonas maltophilia infections A multicenter study confirmed TMP-SMX’s continued place as the primary treatment for Stenotrophomonas.11Journal of Global Antimicrobial Resistance. Trimethoprim-sulfamethoxazole versus levofloxacin for the treatment of Stenotrophomonas maltophilia infections: A multicentre cohort study
Bactrim also covers Burkholderia cepacia to some degree, another naturally resistant organism encountered in cystic fibrosis patients and immunocompromised hosts. For Acinetobacter, TMP-SMX is occasionally considered when other options are exhausted, though it is not a primary choice.
What Bactrim Does Not Cover
No antibiotic covers everything, and Bactrim has several well-defined gaps. Pseudomonas aeruginosa is uniformly resistant to TMP-SMX. If Pseudomonas is suspected or confirmed, a completely different class of antibiotics is needed. Anaerobes are another major blind spot: Bactrim has essentially no useful activity against Bacteroides fragilis or other obligate anaerobic bacteria. For intra-abdominal infections, pelvic infections, or abscesses where anaerobes play a role, TMP-SMX is not appropriate as a stand-alone agent.
The enterococcal gap, covered above, also belongs in this category. And while Bactrim can cover many Enterobacteriaceae, resistance to it is common enough in some species (Serratia, Enterobacter, Citrobacter) that it should not be assumed to work without culture data.
Non-Bacterial Pathogens
One of Bactrim’s most distinctive features is its activity against organisms that are not bacteria at all. Its most important non-bacterial role is in preventing and treating Pneumocystis jirovecii pneumonia (PCP), a potentially fatal fungal lung infection in people with weakened immune systems, such as those with advanced HIV or transplant recipients on immunosuppressive drugs. TMP-SMX is the first-line agent for both PCP prophylaxis and treatment. Research into dosing strategies has found that lower doses of TMP-SMX can produce good outcomes while reducing side effects, which matters because treatment courses for PCP tend to be long and the drug’s toxicity accumulates.12PubMed Central. Trimethoprim-Sulfamethoxazole (Bactrim) Dose Optimization in Pneumocystis jirovecii Pneumonia (PCP) Management: A Systematic Review
When TMP-SMX is used as prophylaxis against PCP, it has the side benefit of also protecting against Toxoplasma gondii, Listeria monocytogenes, and Nocardia species. Patients switched to alternative PCP prophylaxis agents lose that broader protective umbrella.13Emerging Infectious Diseases. Nocardiosis in Immunocompromised Patients on Alternative Pneumocystis Prophylaxis This makes TMP-SMX a remarkably efficient single drug in immunosuppressed patients, covering several opportunistic threats at once.
Tissue Penetration and the Prostate
A drug’s spectrum on paper matters less if it cannot reach the site of infection at adequate concentrations. Bactrim’s two components behave quite differently in this regard. Trimethoprim is highly lipid-soluble and concentrates in many tissues at levels well above what is found in the bloodstream. The prostate is a particularly notable example: trimethoprim levels in prostatic tissue run two to three times higher than plasma levels.14Chemotherapy. Penetration of Antimicrobial Agents into the Prostate Sulfamethoxazole, on the other hand, penetrates prostatic tissue less effectively, reaching only about a third to half of plasma concentrations.15Chemotherapy. Penetration of Antimicrobial Agents into the Prostate This discrepancy means the synergistic ratio of the two drugs shifts when they reach certain tissues, but the strong trimethoprim penetration still makes TMP-SMX one of the better oral options for bacterial prostatitis. It also concentrates well in lung tissue, vaginal secretions, and middle-ear fluid, contributing to its usefulness across many infection types.
Resistance Mechanisms
Bacteria develop resistance to TMP-SMX through several routes, and understanding them helps explain why resistance rates vary so widely between settings. The most common mechanism involves acquiring altered versions of the target enzymes. For trimethoprim resistance, bacteria can pick up genes encoding modified dihydrofolate reductase enzymes that trimethoprim cannot bind to effectively. These resistance genes are frequently carried on mobile genetic elements called integrons, allowing them to hop between bacterial species. Researchers have identified numerous variants of these resistance genes in both pathogenic and commensal gut bacteria.16PubMed Central. New trimethoprim-resistant dihydrofolate reductase cassette, dfrXV, inserted in a class 1 integron The fact that these genes reside on transferable elements helps explain how resistance spreads through bacterial populations, particularly in environments with high antibiotic use like hospitals and long-term care facilities.
Sulfamethoxazole resistance follows a parallel pattern, with modified versions of its target enzyme. Some bacteria achieve resistance by simply overproducing the target enzyme, overwhelming the drug. Others reduce how much drug gets inside the cell in the first place. In practice, resistance to one component of the combination often co-occurs with resistance to the other, because the resistance genes tend to cluster together on the same mobile elements.
Side Effects Worth Knowing About
Bactrim is generally well tolerated for short courses, but it has a few side effects that are worth flagging because they are under-recognized or can become dangerous in the wrong context.
Hyperkalemia
Trimethoprim blocks sodium channels in the kidney’s distal tubule, reducing potassium excretion in a manner identical to the potassium-sparing diuretic amiloride.17PubMed. Trimethoprim-induced hyperkalaemia: clinical data, mechanism, prevention and management In healthy young people with normal kidneys, this rarely matters. But in older adults, people with chronic kidney disease, or anyone already taking drugs that raise potassium (ACE inhibitors, ARBs, potassium supplements, or actual potassium-sparing diuretics), the combination can push potassium to dangerous levels. Life-threatening hyperkalemia has been reported even at standard prophylactic doses of TMP-SMX.18Case Reports in Acute Medicine. Life-Threatening Hyperkalemia following Low-Dose Trimethoprim-Sulfamethoxazole: A Case Report If you are on Bactrim for more than a few days and have any risk factors for high potassium, your doctor may want to check your levels.
Creatinine Bump Without Kidney Damage
Trimethoprim competes with creatinine for secretion in the kidney tubules, which can cause serum creatinine to rise without any actual decline in kidney function.19PubMed. Effect of trimethoprim-sulfamethoxazole on the renal excretion of creatinine in man This is a pharmacological artifact, not kidney injury. The distinction matters because a rising creatinine often triggers alarm and additional workup. If you are on TMP-SMX and your creatinine goes up slightly, the drug itself is a likely explanation, but the change should still be evaluated in context rather than reflexively ignored.
Blood Cell Effects
Because TMP-SMX interferes with folate metabolism, prolonged use can suppress bone marrow production of blood cells. The trimethoprim component, through its antifolate activity, can inhibit the production of white blood cells (particularly neutrophils), and this effect is reversible with folinic acid.20PubMed. Studies of the effects of trimethoprim and sulfamethoxazole on human granulopoiesis In rare cases, TMP-SMX can cause pancytopenia, a simultaneous drop in red cells, white cells, and platelets. This has been documented after as little as two weeks of use, particularly in elderly patients.21PubMed Central. Trimethoprim-sulfamethoxazole Induced Pancytopenia: A Common Occurrence but A Rare Diagnosis Patients who are already folate-depleted (due to poor nutrition, alcoholism, or other medications) face higher risk.
Skin Reactions
Sulfonamides are among the most common drug classes associated with severe allergic skin reactions, including Stevens-Johnson syndrome (SJS), a potentially life-threatening condition where the skin and mucous membranes blister and peel. Case reports link TMP-SMX to SJS, sometimes beginning about a week into treatment.22PubMed Central. Stevens Johnson Syndrome Initiated by an Adverse Reaction to Trimethoprim-Sulfamethoxazole Incomplete forms of SJS, where the presentation is atypical and harder to recognize, have also been described after sulfonamide exposure.23PubMed Central. Incomplete Stevens-Johnson Syndrome Caused by Sulfonamide Antimicrobial Exposure Any new rash or mucosal blistering while taking Bactrim warrants immediate medical evaluation. People with a documented sulfa allergy should not take TMP-SMX.
Drug Interactions to Watch For
The interaction between TMP-SMX and warfarin deserves special attention. TMP-SMX raises warfarin levels through two mechanisms: it disrupts gut bacteria that produce vitamin K, and it inhibits the liver enzyme (CYP2C9) responsible for breaking down warfarin. In elderly patients with UTIs taking warfarin long-term, receiving a TMP-SMX prescription was associated with roughly a four-fold increased risk of being hospitalized for upper gastrointestinal bleeding.24PubMed Central. Warfarin and Antibiotics: Drug Interactions and Clinical Considerations If you take warfarin and need an antibiotic, this interaction should be part of the prescribing conversation. Closer INR monitoring or choosing a different antibiotic may be warranted.
TMP-SMX can also increase methotrexate toxicity through shared antifolate mechanisms and competition for kidney excretion. Patients on methotrexate for autoimmune conditions or cancer should flag this to their prescriber. Similarly, the combination of TMP-SMX with ACE inhibitors or ARBs compounds the hyperkalemia risk described above.
Pregnancy and Bactrim
TMP-SMX carries particular concerns during pregnancy. Because both components interfere with folate metabolism, there are theoretical and observed risks of birth defects when the drug is used in the first trimester, the period of most active organ formation. Concerns include possible links to neural tube defects and other congenital abnormalities.25JAMA Pediatrics. Antibacterial Medication Use During Pregnancy and Risk of Birth Defects: National Birth Defects Prevention Study Near the end of pregnancy, the sulfamethoxazole component can displace bilirubin from its binding sites in the newborn’s blood, raising the risk of jaundice. For these reasons, TMP-SMX is generally avoided in pregnancy whenever alternatives exist, especially during the first trimester and the final weeks before delivery.
When Bactrim Has an Edge Over Alternatives
Given all the caveats about resistance, side effects, and spectrum gaps, it is worth stepping back and recognizing the clinical niches where TMP-SMX genuinely outperforms or fills a role that few other oral antibiotics can. Outpatient MRSA skin infections are the most common scenario: the oral options here are limited, and TMP-SMX is inexpensive, well-studied, and generally effective. Stenotrophomonas infections, as discussed, essentially require TMP-SMX or a fluoroquinolone, and TMP-SMX remains preferred. PCP prophylaxis and treatment in immunocompromised patients is another domain where TMP-SMX has no true equal for breadth-of-coverage-per-pill. And bacterial prostatitis, where many antibiotics struggle to achieve adequate tissue levels, is a scenario that plays to trimethoprim’s pharmacokinetic strengths.
For uncomplicated UTIs, the picture is more competitive. Where local resistance rates remain below about 20%, TMP-SMX is still a reasonable empiric choice and is included in most UTI treatment guidelines. But nitrofurantoin and fosfomycin have gained ground as first-line agents in many regions precisely because their resistance rates have stayed lower. If you have a UTI and your doctor reaches for Bactrim, it is not the wrong call, but asking about local resistance patterns is a reasonable question.
