Bleeding gums almost always signal inflammation, and the most common cause is a buildup of bacterial plaque along the gumline. That inflammation, called gingivitis, is the earliest stage of gum disease and affects a huge portion of adults. The good news is that at this stage, the process is fully reversible. But bleeding gums can also point to hormonal changes, nutritional gaps, or underlying health conditions, so it’s worth understanding what’s behind the bleeding and what to do about it.
Plaque Buildup Is the Most Common Cause
Your gums bleed because bacteria in dental plaque trigger an immune response. When plaque sits undisturbed along the gumline, the bacteria release compounds that your immune system recognizes as threats. Your body responds by flooding the area with white blood cells and inflammatory molecules, which increases blood flow to the gum tissue and makes the tiny blood vessels there more fragile. That’s why even light brushing or flossing can break those capillaries and cause bleeding.
This cycle is self-reinforcing. As inflammation increases, the gums swell slightly and pull away from the teeth, creating small pockets where more bacteria collect. The protein-rich fluid that leaks from inflamed gums actually feeds the bacteria, helping them multiply faster. Severe periodontal disease affects more than 1 billion people worldwide, according to the World Health Organization, but it all starts with this same basic process: plaque, inflammation, bleeding.
Gingivitis vs. Periodontitis
Gingivitis is the mildest form of gum disease. Your gums look red and swollen and bleed easily, but no permanent damage has occurred. The bone and connective tissue holding your teeth in place are still intact. With better brushing and flossing habits, gingivitis typically clears up within a couple of weeks.
If gingivitis goes untreated, it can progress to periodontitis, which involves actual bone loss around the teeth. Dentists measure this progression by probing the pockets between your gums and teeth. Healthy pockets measure 1 to 3 millimeters deep. In early periodontitis, pockets deepen to around 4 millimeters with slight bone loss visible on X-rays. Moderate periodontitis shows pockets of 5 to 6 millimeters. In severe cases, pockets reach 6 to 8 millimeters or more, teeth become loose, and chewing becomes difficult. At the most advanced stage, teeth may need to be removed.
The key difference: gingivitis is reversible, periodontitis is not. You can stop periodontitis from getting worse, but you can’t regrow lost bone on your own. That’s why bleeding gums deserve attention early, before the damage becomes permanent.
Brushing Too Hard Can Cause It Too
Not all gum bleeding comes from disease. Brushing with too much force, using a hard-bristled toothbrush, or scrubbing back and forth aggressively can physically damage gum tissue. Over time, this kind of mechanical trauma can also cause gum recession, where the gum tissue wears away and exposes the root surface of the tooth.
The fix is straightforward: switch to a soft-bristled brush and use gentle, circular motions rather than sawing horizontally. If you’ve recently started flossing after a long break, some bleeding in the first week or two is normal as your gums adjust. Bleeding that persists beyond two weeks of consistent, gentle flossing is more likely inflammation than technique.
Hormonal Changes and Pregnancy
Pregnancy is one of the most well-known non-disease causes of bleeding gums. Rising levels of progesterone and estrogen increase blood flow to gum tissue and change how the immune system responds to plaque bacteria. The result is that even a small amount of plaque can cause an exaggerated inflammatory response, making gums swell and bleed much more easily than usual. This is common enough that it has its own name: pregnancy gingivitis.
Hormonal shifts during puberty, menstruation, and menopause can produce similar effects, though usually milder. In most cases, the bleeding improves once hormone levels stabilize, but maintaining good oral hygiene during these periods helps prevent the inflammation from progressing.
Low Vitamin C Plays a Role
Harvard Health has highlighted research showing that low vitamin C levels in the bloodstream are associated with increased gum bleeding, even with gentle probing. Vitamin C is essential for maintaining the connective tissue that supports blood vessel walls. When levels drop too low, capillaries become fragile and bleed more easily, not just in the gums but throughout the body. In its most extreme form, this is scurvy, though outright scurvy is rare today.
The recommended daily intake of vitamin C is 90 milligrams for adult men and 75 milligrams for women. Eating foods like bell peppers, kiwis, oranges, strawberries, and kale can help you meet that easily. If your diet is limited, a daily supplement of 100 to 200 milligrams covers the gap. Vitamin K deficiency can also contribute to bleeding problems since it plays a central role in blood clotting, though this is less commonly the culprit for gum bleeding specifically.
The Connection to Diabetes
Bleeding gums and diabetes have a two-way relationship. People with diabetes are three times more likely to develop gum disease, according to research from the Harvard School of Dental Medicine. Persistently high blood sugar impairs the body’s ability to fight infection and promotes chronic inflammation in the mouth. Higher glucose levels in saliva also feed harmful oral bacteria, accelerating plaque buildup and tissue breakdown.
The relationship works in reverse, too. Inflammation from gum disease can worsen blood sugar control, making diabetes harder to manage. Treating gum disease reduces that chronic inflammatory burden, which can improve the body’s response to insulin and help stabilize blood sugar. If you have diabetes and notice your gums bleeding regularly, it’s worth addressing both conditions together rather than treating them separately.
Less Common but Serious Causes
In rare cases, gum bleeding that seems out of proportion to plaque buildup can signal a blood disorder. Conditions that affect how your blood clots or how your body produces blood cells, including leukemia, can show up as unexplained oral bleeding. These situations typically come with other symptoms: unusual bruising, fatigue, frequent infections, or bleeding that’s hard to stop from minor cuts. Certain medications, particularly blood thinners, can also make gum bleeding more pronounced.
If your gums bleed heavily despite good oral hygiene, or if the bleeding is accompanied by symptoms elsewhere in your body, the cause may be systemic rather than dental.
What Healthy Gums Should Look Like
Healthy gum tissue is pale pink, firm, and fits snugly around the teeth like a tight collar. It doesn’t bleed when you brush or floss. Inflamed gums, by contrast, appear red or dark pink, feel puffy or spongy, and may be tender to the touch. You might also notice a bad taste in your mouth or persistent bad breath.
Dentists assess gum health partly by checking how many sites in your mouth bleed when gently probed. Research from the American Academy of Periodontology suggests that when more than 25% of probed sites bleed, the risk of progressive disease increases significantly. Below that threshold, gums are generally considered stable. This is why regular dental cleanings matter: they remove hardened plaque (tarite) that you can’t get off with brushing alone, and they catch early inflammation before it becomes irreversible bone loss.
