Most Botox side effects are mild and short-lived, clearing up within a day or two. Bruising, swelling, and redness at the injection site are the most common reactions. But the full range of possible effects extends from minor inconveniences to rare but serious complications, and the risk profile shifts depending on how much is injected and where.
Common Side Effects
The side effects most people experience are localized to where the needle goes in. Bruising is the most frequent complaint, especially in areas with many small blood vessels like the skin around the eyes. Mild swelling, redness, and tenderness at the injection site are also typical and usually resolve within 24 to 48 hours.
Headaches sometimes follow treatment, particularly after forehead injections. Some people also report brief flu-like symptoms: mild muscle aches, fatigue, or a low-grade fever. These tend to pass quickly and are more commonly reported with medical-dose treatments (like Botox for migraines, which involves more injection sites and higher doses) than with purely cosmetic use.
You can reduce your chances of bruising by avoiding alcohol, aspirin, ibuprofen, fish oil, and vitamin E for a few days before and after treatment. Applying a cold compress for 10 to 15 minutes during the first 24 hours helps constrict blood vessels and limit bruising. Keeping your head elevated and skipping strenuous exercise for the first four to six hours also makes a difference.
Eyelid and Eyebrow Drooping
About 2% of patients develop drooping of the eyelid or eyebrow after Botox injections in the upper face. This happens when the toxin migrates slightly from the injection site and weakens nearby muscles that hold the eyelid or brow in position. The drooping typically appears a few days to a week after treatment.
The good news is that it’s almost always temporary. While a full Botox treatment can last up to six months, ptosis (the clinical term for this drooping) usually resolves on its own within five to six weeks. If it’s bothersome enough to need treatment, prescription eye drops containing a muscle-stimulating ingredient can lift the lid by 1 to 2 millimeters while you wait for the effect to wear off.
Neck Pain and Stiffness
Neck pain is a particularly common side effect when Botox is used for migraines, since the injection protocol for chronic migraine involves multiple shots across the head, neck, and shoulders. Some patients describe stiffness or soreness in the neck that lasts a few days. For cosmetic treatments focused on the forehead or crow’s feet, neck pain is less common but can still occur if the treatment area extends to the lower forehead or between the brows.
When Botox Stops Working
Your immune system can learn to fight off Botox. Roughly 10% of patients who receive repeated injections develop neutralizing antibodies, proteins that bind to the toxin and block it from working. The risk is higher with frequent treatments: about 14.5% in patients getting repeated sessions compared to 4.5% after fewer exposures.
When this happens, the treatment gradually becomes less effective. Studies show patients with these antibodies experience about half the muscle-relaxing benefit compared to those without them. Higher doses or switching formulations are sometimes tried, but for some people the treatment simply stops delivering results. Spacing out your sessions and using the lowest effective dose can help reduce this risk.
Long-Term Muscle Changes
Repeated Botox use over months or years causes measurable changes in the targeted muscles. A systematic review of imaging and tissue studies found that injected muscles lose volume and thin out, a process called atrophy. In one study, the small muscle between the eyebrows (the one that creates the “11 lines” when you frown) lost approximately 50% of its volume. Another found that muscle fiber diameter shrank by nearly 20% after two to four years of treatment.
Under the microscope, researchers have observed muscle fibers replaced by fat cells and fibrous tissue. Human imaging studies using ultrasound, MRI, and CT scans confirmed muscle shrinkage appearing as early as six weeks after injection and persisting for up to two years, with no full recovery identified in the studies reviewed. For cosmetic users, this muscle thinning is partly why some people find they need less Botox over time, as the weakened muscles create fewer deep wrinkles even between treatments. But it also means the changes aren’t purely temporary.
Serious but Rare Complications
The FDA requires a boxed warning (its strongest safety alert) on all botulinum toxin products. The concern: in rare cases, the toxin can spread beyond the injection site and cause effects elsewhere in the body. Reported symptoms include generalized muscle weakness, double vision, difficulty speaking, loss of bladder control, and trouble swallowing or breathing. These symptoms have appeared anywhere from hours to weeks after injection.
Difficulty swallowing and breathing are the most dangerous of these effects, and there have been reported deaths, primarily in patients being treated for muscle spasticity rather than cosmetic purposes. The risk is highest in children treated for spasticity, but cases have occurred in adults across both medical and cosmetic uses.
Allergic reactions are also possible, though uncommon. Signs include rash, itching, and in rare cases breathing difficulty unrelated to muscle weakness.
Who Faces Higher Risk
Certain medical conditions significantly increase the chance of serious complications. People with neuromuscular disorders are at the top of this list. Conditions like ALS (Lou Gehrig’s disease), myasthenia gravis, and Lambert-Eaton syndrome affect how nerves communicate with muscles, and adding a muscle-paralyzing toxin on top of that can tip the balance toward dangerous weakness, severe swallowing problems, or respiratory compromise even at standard doses.
People receiving Botox for medical conditions like chronic migraine, overactive bladder, or cervical dystonia generally receive higher doses spread across more injection sites than cosmetic patients. This increases both the likelihood and severity of side effects. If you’re getting Botox for both cosmetic and medical reasons, spacing those treatments apart matters. Getting too much too close together raises your risk of side effects and can accelerate antibody formation, potentially making all your treatments less effective over time.
Medical vs. Cosmetic Side Effect Profiles
A typical cosmetic Botox session for forehead lines uses a relatively small amount of toxin concentrated in a few spots. The side effects tend to stay local: bruising, minor swelling, and occasionally a headache. A migraine treatment session, by contrast, involves 31 or more injections across seven areas of the head and neck. That wider distribution and higher total dose means more opportunities for neck stiffness, flu-like symptoms, and muscle weakness beyond what cosmetic patients typically experience.
The trade-off is straightforward. Lower doses in smaller areas carry fewer and milder side effects. Higher doses across larger areas are more likely to produce systemic symptoms, but they’re also treating more serious conditions where the benefit calculation is different. Regardless of the reason for treatment, the most common side effects remain mild and temporary for the vast majority of patients.
