A C-reactive protein (CRP) test is a blood test that measures inflammation in your body. It doesn’t pinpoint where the inflammation is coming from or what’s causing it, but it tells your doctor that something is triggering your immune system. That makes it one of the most widely used general screening tools in medicine, ordered for everything from diagnosing infections to monitoring autoimmune diseases to assessing heart disease risk.
How CRP Works in Your Body
CRP is a protein made by liver cells. When you have an infection, injury, or inflammatory condition, your immune system releases signaling molecules that travel to the liver and ramp up CRP production. The main signal driving this process is a molecule called IL-6, which essentially tells your liver to flood the bloodstream with CRP as part of the body’s first-line defense system.
What makes CRP especially useful as a diagnostic marker is how dramatically it responds. In a healthy person, CRP levels are barely detectable. During a serious infection or acute inflammation, those levels can spike a thousandfold or more within 24 to 48 hours. When the inflammation resolves, CRP drops back down quickly, unlike some other inflammation markers that linger for weeks. That rapid rise and fall gives doctors a real-time snapshot of how active inflammation is right now.
Detecting and Distinguishing Infections
One of the most common reasons doctors order a CRP test is to figure out whether an infection is bacterial or viral, since that distinction determines whether antibiotics will help. Viral infections tend to produce modest CRP elevations, while bacterial infections push levels much higher.
The thresholds aren’t perfectly standardized, but research published in BMJ Global Health found that a cutoff around 20 mg/L is a conservative choice that catches most serious bacterial infections, keeping the risk of missed diagnoses low. A higher cutoff around 40 mg/L is more specific, meaning fewer false alarms, and has been shown to meaningfully reduce unnecessary antibiotic prescriptions. Some studies have found optimal cutoffs clustering around 19 to 21 mg/L, while others land closer to 36 to 37 mg/L, depending on the patient population.
CRP isn’t a standalone diagnostic tool for infections. Your doctor will interpret the number alongside your symptoms, physical exam, and possibly other lab work. But it’s a fast, inexpensive way to lean the diagnosis in one direction.
Monitoring Autoimmune Conditions
If you have rheumatoid arthritis, inflammatory bowel disease, or another chronic inflammatory condition, CRP testing is likely a regular part of your care. Doctors use it to track disease activity over time: a rising CRP can signal a flare before symptoms fully develop, and a falling CRP after starting a new treatment suggests the medication is working.
In rheumatoid arthritis specifically, CRP serves double duty. It measures how much systemic inflammation your body is producing, and it also helps doctors assess your risk of related complications. Chronic inflammation from RA accelerates plaque buildup in arteries, and persistently elevated CRP is one indicator of that risk. Clinical trials sometimes stratify patients by CRP level to better understand how different treatments affect both joint inflammation and cardiovascular outcomes.
Assessing Heart Disease Risk
For heart disease screening, doctors use a more sensitive version of the test called high-sensitivity CRP (hs-CRP). Standard CRP tests are designed to detect the large spikes caused by infection or acute illness. The hs-CRP test measures much smaller elevations that reflect low-grade, chronic inflammation in blood vessel walls, the kind that contributes to plaque buildup over years.
Current guidelines from the American Heart Association and American College of Cardiology identify an hs-CRP level of 2 mg/L or higher as a risk-enhancing factor for cardiovascular disease. This is particularly useful for people whose risk falls in the intermediate zone, where traditional factors like cholesterol and blood pressure don’t clearly point toward or away from treatment. A high hs-CRP in that gray area can tip the decision toward more aggressive prevention.
The ACC recently went further, recommending universal screening of hs-CRP in both people who haven’t had a cardiac event and those who have. That’s a notable shift from earlier guidance, which limited hs-CRP testing to specific clinical scenarios.
Tracking Recovery After Surgery
CRP is routinely measured after major surgeries to monitor healing. A temporary spike is completely normal: levels typically peak two to three days after surgery, then steadily decline. This pattern holds across different types of procedures.
What doctors watch for is a CRP level that rises again after initially dropping, or one that stays elevated well past the expected timeline. Either pattern can signal a surgical site infection or other complication before visible symptoms like redness or fever appear. Because CRP responds within hours, it often flags problems earlier than clinical signs alone would.
CRP vs. ESR: Two Inflammation Tests
If you’ve had blood work for inflammation, you may have seen both CRP and ESR (erythrocyte sedimentation rate) on your results. They measure related but different things, and their timelines are quite different.
CRP peaks about 48 hours after inflammation begins and drops quickly once the trigger resolves. ESR rises slowly and can take weeks to return to normal. That makes CRP the better choice for detecting acute changes and monitoring treatment response in real time, while ESR is sometimes used alongside CRP to get a broader picture of chronic inflammation. In practice, many doctors order both.
What Can Affect Your Results
CRP is sensitive, which is its strength and its limitation. Plenty of things besides serious illness can nudge your levels up. Smoking, obesity, poor sleep, depression, gum disease, diabetes, pregnancy, and even a recent common cold can all cause mild elevations. A single high reading doesn’t necessarily mean something is wrong.
Intense exercise can also cause a temporary spike. If you’ve done a hard workout or a long run shortly before your blood draw, your results may be artificially elevated. Some doctors ask patients to avoid strenuous exercise before the test for this reason. Fasting usually isn’t required for a standalone CRP test, though if your blood sample is also being used for cholesterol testing (common when hs-CRP is ordered for cardiac risk), you may need to fast.
Certain medications lower CRP independently of treating the underlying condition. Statins are the most notable example. Research has shown that statins reduce CRP levels through anti-inflammatory effects separate from their cholesterol-lowering action. If you’re on a statin, your CRP results may read lower than they would otherwise, something your doctor should factor into the interpretation.
What Your Results Mean
A CRP result is reported in milligrams per liter (mg/L). For a standard CRP test used to evaluate infection or acute inflammation, levels below 10 mg/L are generally considered normal or mildly elevated. Levels above 10 mg/L suggest significant inflammation, and levels above 100 mg/L typically point to a serious infection or major inflammatory event.
For hs-CRP used in cardiac risk assessment, the scale is much finer. Below 2 mg/L is considered lower risk, while 2 mg/L and above is classified as a risk-enhancing factor for cardiovascular disease. Because so many everyday factors can influence these small numbers, doctors often repeat the test and average the results for a more reliable reading.
No CRP result, high or low, is a diagnosis on its own. It’s a signal that points your doctor toward further investigation, helps narrow the possibilities, or confirms that a treatment is reducing inflammation as expected.
