Can You Take Ear Drops While Pregnant? Safety Facts

Most ear drops are considered safe during pregnancy. Because the ear canal has limited blood supply compared to other body surfaces, very little of what you put in your ear actually reaches your bloodstream, and even less would reach your baby. That said, not all ear drops carry the same level of evidence, so the type of drop matters.

Why Ear Drops Pose Less Risk Than Oral Medications

The key factor with any medication during pregnancy is how much of it gets absorbed into your blood and crosses the placenta. Ear drops are applied to a tiny surface area inside a narrow canal, which limits how much your body takes in. The FDA notes that systemic absorption following topical ear application “appears to be minimal” and that maternal use of otic solutions “is not expected to result in fetal exposure to the drug.” This is a fundamentally different situation from swallowing a pill, where the full dose enters your digestive system and bloodstream.

That distinction is important because some ingredients that would raise concerns as oral medications, like certain antibiotics or steroids, are generally considered acceptable when delivered as ear drops. The dose is tiny, and the route of delivery keeps most of it local.

Antibiotic Ear Drops

Antibiotic ear drops are the standard treatment for outer ear infections (swimmer’s ear), and they’re widely used in pregnancy. The combination of polymyxin B, neomycin, and hydrocortisone is considered safe for pregnant women as long as your eardrum is intact. If there’s any chance your eardrum has a hole or tear, your provider will likely switch to ciprofloxacin or ofloxacin drops instead, since those are safe to use even with a perforated eardrum.

Fluoroquinolone antibiotics like ciprofloxacin have raised questions in pregnancy because oral versions were once thought to affect fetal joint development. But controlled observational data show that fluoroquinolone exposure during pregnancy is not associated with significant musculoskeletal problems. And again, the amount absorbed from ear drops is far smaller than what you’d get from an oral dose.

Steroid-Containing Ear Drops

Many prescription ear drops include a low-dose steroid like hydrocortisone or dexamethasone to reduce swelling and pain in the ear canal. A large Cochrane review of topical corticosteroid use in pregnancy found no link between maternal use and congenital abnormalities, preterm delivery, fetal death, or low Apgar scores, regardless of steroid potency.

The one nuance: very potent topical steroids used in large quantities over the course of a pregnancy were associated with a small risk of low birth weight. But this finding applied to extensive skin application, like treating widespread eczema with strong prescription creams over months. The tiny amount of mild steroid in ear drops is nowhere near that threshold.

Antifungal Ear Drops

Fungal ear infections (otomycosis) are sometimes treated with clotrimazole ear drops. Most studies have found that clotrimazole at low doses does not increase the chance of birth defects. One study suggested a small increased chance of miscarriage, but it had methodological problems, and other studies haven’t confirmed that finding. Since ear drops deliver a very small dose to a localized area, the amount reaching your bloodstream is expected to be minimal.

Pain-Relieving Ear Drops

Over-the-counter numbing drops containing benzocaine are sometimes used for ear pain. Benzocaine hasn’t been extensively studied in pregnancy, but when used as directed, very little is absorbed into the body. Two reports looking at first-trimester use found no greater chance of birth defects. The amount that would reach a developing baby from a few drops in the ear canal is expected to be very low.

Earwax Removal Drops

Carbamide peroxide drops (sold as Debrox and similar brands) are the most common over-the-counter option for softening earwax. These carry a pregnancy category C rating, meaning animal studies have shown some risk but human studies haven’t been done. There’s no strong evidence of harm, but there’s also no robust safety data confirming they’re fine. If earwax buildup is bothering you during pregnancy, a warm water rinse or having your provider flush the ear manually may be a simpler choice that avoids the question entirely.

How to Apply Ear Drops Correctly

Proper technique helps the drops work better and minimizes the chance of any unnecessary absorption elsewhere. Lie on your side or tilt your head so the affected ear faces the ceiling. For adults, gently pull the top of your ear backward and upward to straighten the ear canal, then let the drops fall in. Press the small flap of cartilage at the front of your ear (the tragus) gently inward to help the drops flow deeper into the canal. Stay tilted for a few minutes, then straighten up and wipe away any excess with a clean tissue.

The Eardrum Factor

One thing that changes the safety picture is whether your eardrum is intact. An intact eardrum acts as a barrier, keeping drops confined to the outer ear canal. If you have a perforation, drops can pass through to the middle ear, where absorption increases and certain ingredients (particularly aminoglycosides like neomycin) could potentially cause problems. This is why providers check your eardrum before prescribing drops, and why the recommended medication may differ if a perforation is suspected. If you have ear tubes, a history of eardrum rupture, or active drainage from your ear, make sure your provider knows before you start any drops.
