Cetirizine HCl 10 mg: Uses, Side Effects & How It Works

Cetirizine HCl 10 mg is an antihistamine tablet taken once daily to relieve symptoms of seasonal allergies (hay fever), year-round allergies, and chronic hives. It works within 20 minutes for about half of people who take it, and a single dose lasts a full 24 hours. As a second-generation antihistamine, it targets allergy symptoms without causing as much drowsiness as older options like diphenhydramine (Benadryl).

Allergy Symptoms It Treats

The 10 mg tablet is specifically approved to temporarily relieve upper respiratory allergy symptoms: runny nose, sneezing, itchy or watery eyes, and itching of the nose or throat. These symptoms can come from seasonal triggers like pollen or from year-round allergens like dust mites, pet dander, and mold. Cetirizine doesn’t treat the underlying allergy itself, but it blocks the chemical (histamine) your body releases during an allergic reaction, which is what causes those symptoms in the first place.

Chronic Hives

Beyond nasal and eye symptoms, cetirizine is widely used for chronic hives (also called chronic urticaria), the condition where itchy, raised welts appear on the skin repeatedly for six weeks or more. In clinical trials, cetirizine at 10 mg significantly reduced welts, redness, and itching compared to placebo. One study found that 74% of days were mild or symptom-free on cetirizine 10 mg, compared to 57% on placebo.

For people whose hives don’t fully respond to the standard 10 mg dose, doctors sometimes prescribe 20 mg daily (double the standard dose). Studies have shown this higher dose can continue to improve symptoms, and that dropping back down to 10 mg often allows hives to return. This higher dosing is considered off-label but is supported by guidelines from organizations like NICE.

How It Works

When your body encounters an allergen, immune cells release histamine, which binds to receptors throughout your nose, eyes, throat, and skin. Cetirizine blocks those receptors with high selectivity, meaning it sticks to the allergy-related receptors without interfering much with other systems in your body. It’s about 600 times more selective for these allergy receptors compared to other receptor types, which is part of why it causes fewer side effects than first-generation antihistamines.

One reason cetirizine is effective for a full day is that it dissociates from the receptor slowly. The active portion of the molecule stays bound with a half-life of about 142 minutes, essentially acting as a near-permanent blocker for the duration of its effect. This means you don’t need to re-dose mid-day.

How Quickly It Works

After taking a single 10 mg tablet, 50% of people feel relief within 20 minutes, and 95% experience relief within one hour. That effect persists for at least 24 hours, which is why once-daily dosing is standard. You can take it with or without food, and the timing doesn’t matter much as long as you’re consistent.

Who Can Take It

The 10 mg tablet is the standard adult dose and is also appropriate for children aged 6 and over. Children aged 12 and up take the same 10 mg dose as adults. Younger children (ages 2 to 5) typically use a lower dose in liquid form. You should not take more than one 10 mg tablet in a 24-hour period.

People with kidney problems may need a lower dose. If your kidney function is significantly reduced (a GFR of 50 or below), the typical recommendation is to cut the dose in half to 5 mg daily. This is because cetirizine is cleared through the kidneys, and reduced kidney function means the drug stays in your system longer.

Cetirizine is generally considered one of the safer antihistamine choices during pregnancy and breastfeeding. There’s no evidence it harms a developing baby, and only very small amounts pass into breast milk. It has been widely used by breastfeeding mothers for years without reports of significant effects on infants, though unusual sleepiness in a nursing baby is worth watching for.

Side Effects

Cetirizine causes less sedation than first-generation antihistamines, but drowsiness is still the most commonly reported side effect. In clinical trials using 20 mg (double the standard dose), about 24% of people reported feeling sleepy, compared to roughly 8% on placebo. At the standard 10 mg dose, drowsiness rates are lower, but still noticeable for some people. If sleepiness is an issue, taking your dose in the evening can help.

Other possible side effects include dry mouth, fatigue, and headache, though these are less common. Most people tolerate cetirizine well at the 10 mg dose. Unlike first-generation antihistamines, cetirizine doesn’t cross into the brain as readily, which is why it’s much less likely to cause the heavy, sedated feeling associated with older allergy medications.

Cetirizine vs. Other Antihistamines

Cetirizine sits in the same class as loratadine (Claritin) and fexofenadine (Allegra). All three are second-generation antihistamines with 24-hour dosing. Cetirizine tends to work slightly faster and is often considered more potent, particularly for hives and skin-related allergic reactions. The tradeoff is that it’s also slightly more likely to cause drowsiness than loratadine or fexofenadine. For people who need strong allergy relief and aren’t bothered by mild sleepiness, cetirizine is often the preferred choice. For those who are sensitive to drowsiness, fexofenadine is typically the least sedating option.
