Tadalafil, the active ingredient in Cialis, is approved for the treatment of benign prostatic hyperplasia (BPH) at a dose of 5 mg taken once daily. In clinical trials, this low daily dose consistently reduced urinary symptom scores by roughly two to three points more than placebo, with improvements noticeable in as little as one week. The drug works through a different pathway than traditional prostate medications, and because it also treats erectile dysfunction, it has become an appealing option for men dealing with both conditions at the same time.
How Tadalafil Works on the Prostate and Bladder
BPH medications generally fall into two camps: alpha-blockers, which relax muscle in the prostate and bladder neck, and 5-alpha reductase inhibitors, which shrink the prostate over months. Tadalafil does neither of those things. Instead, it blocks an enzyme called PDE5, which is found in smooth muscle throughout the lower urinary tract. By blocking that enzyme, tadalafil lets a signaling molecule called cGMP accumulate, and cGMP tells smooth muscle to relax. Lab studies have confirmed that this relaxation happens in smooth muscle strips taken from the human prostate, bladder, and the arteries that supply those organs.1PubMed. The mechanism of action of phosphodiesterase type 5 inhibitors in the treatment of lower urinary tract symptoms related to benign prostatic hyperplasia
But muscle relaxation is only part of the story. Research suggests tadalafil also improves blood flow to the prostate and bladder. In animal models, chronic pelvic ischemia (poor blood supply to the pelvis) led to structural damage in prostate tissue, including excess collagen buildup and increased muscle contractility. Tadalafil prevented all of those changes.2PubMed Central. Protective effect of tadalafil on the functional and structural changes of the rat ventral prostate caused by chronic pelvic ischemia Separate experiments showed that tadalafil maintained blood flow to both the bladder and prostate during conditions that would otherwise reduce it.3PubMed. Effect of a single treatment with tadalafil on blood flow in lower urinary tract tissues in rat models of bladder overdistension/emptying and abdominal aorta clamping/release
A third mechanism involves inflammation. BPH tissue tends to be chronically inflamed, and lab studies on human BPH tissue found that tadalafil reduced the secretion of inflammatory markers when the tissue was exposed to metabolic stressors. The effect appeared to run through the same cGMP pathway that drives muscle relaxation.4PubMed. PDE5 inhibitors blunt inflammation in human BPH: a potential mechanism of action for PDE5 inhibitors in LUTS So tadalafil likely helps through at least three routes: relaxing smooth muscle, improving blood flow, and dampening inflammation. Which of these matters most in a given patient is still an open question, but collectively they explain why the drug relieves symptoms even though it does not shrink the prostate.
What the Clinical Trials Actually Show
The effectiveness of tadalafil for BPH has been measured across multiple large, placebo-controlled trials. The standard yardstick is the International Prostate Symptom Score, a questionnaire that captures how often you experience weak stream, incomplete emptying, frequency, urgency, and nighttime urination. Scores run from 0 (no symptoms) to 35 (severe), and a drop of about three points is considered clinically meaningful.
In one pivotal international trial, men taking tadalafil 5 mg daily saw their symptom score drop by 5.6 points from baseline, compared with 3.6 points for placebo, a statistically significant difference. The reduction was already visible after one week and reached significance by the four-week mark.5PubMed. Efficacy and safety of tadalafil once daily in the treatment of men with lower urinary tract symptoms suggestive of benign prostatic hyperplasia: results of an international randomized, double-blind, placebo-controlled trial Across trials, tadalafil improved not just total symptom scores but also the voiding subscore (weak stream, hesitancy), the storage subscore (urgency, frequency), and patients’ self-reported quality of life.6PubMed Central. A review of the use of tadalafil in the treatment of benign prostatic hyperplasia in men with and without erectile dysfunction
A pooled analysis of four pivotal trials put a finer point on the timeline. Roughly two-thirds of men on tadalafil 5 mg achieved a clinically meaningful improvement. Among those responders, more than half got there within the first week, and over 70% within four weeks.7PubMed. Time to onset of clinically meaningful improvement with tadalafil 5 mg once daily for lower urinary tract symptoms secondary to benign prostatic hyperplasia: analysis of data pooled from 4 pivotal, double-blind, placebo controlled studies That speed of onset is quicker than 5-alpha reductase inhibitors like finasteride, which typically require months to show their full effect.
The Flow Rate Paradox
Here is where the story gets interesting and sometimes confusing. Despite clear improvements in how men feel, tadalafil does not meaningfully improve peak urinary flow rate (the objective measurement of how fast urine comes out). In one analysis, there were dose-dependent trends toward better flow, but none reached statistical significance compared with placebo.8PubMed. Changes in peak urinary flow and voiding efficiency in men with signs and symptoms of benign prostatic hyperplasia during once daily tadalafil treatment A dose-finding study confirmed the same pattern: symptom scores improved at every dose tested, but peak flow did not budge significantly at any dose.9PubMed. Tadalafil administered once daily for lower urinary tract symptoms secondary to benign prostatic hyperplasia: a dose finding study
This is worth flagging because urologists sometimes rely on flow rate as an objective marker of obstruction. The disconnect between subjective symptom relief and objective flow rate suggests that tadalafil’s benefit comes mainly from its effects on bladder smooth muscle, blood flow, and sensory nerve signaling rather than from physically opening the channel through the prostate. In practical terms, your symptoms get better even if a uroflow test would look about the same. That distinction matters when you are deciding whether to add tadalafil to a regimen or switch from an alpha-blocker, especially if your doctor is monitoring you with flow studies.
How It Compares to Tamsulosin
Tamsulosin (Flomax) has been the default first-line medication for BPH for years. A head-to-head randomized trial compared tadalafil 5 mg, tamsulosin 0.4 mg, and placebo over 12 weeks. Both active drugs significantly improved symptom scores versus placebo, with tadalafil producing a 2.1-point advantage over placebo and tamsulosin producing a 1.5-point advantage. Both drugs started working within one week.10PubMed. Monotherapy with tadalafil or tamsulosin similarly improved lower urinary tract symptoms suggestive of benign prostatic hyperplasia in an international, randomised, parallel, placebo-controlled clinical trial
A meta-analysis pooling data from randomized controlled trials confirmed there was no significant difference in symptom score reduction between the two drugs when used alone.11PubMed Central. Comparative Effectiveness of Tadalafil versus Tamsulosin in Treating Lower Urinary Tract Symptoms Suggestive of Benign Prostate Hyperplasia: A Meta-Analysis of Randomized Controlled Trials A separate case-control study similarly found comparable symptom improvements, with tadalafil adding a marked improvement in sexual function that tamsulosin did not provide.12PubMed Central. Comparing monotherapy with tadalafil or tamsulosin in men with benign prostatic hyperplasia: A case-control study
The side-effect profiles differ in ways that influence prescribing decisions. Tamsulosin is more likely to cause dizziness and retrograde ejaculation (semen flowing backward into the bladder during orgasm). Tadalafil tends toward headache, back pain, and flushing. Neither drug has serious safety issues in most men, but those differences can steer a choice. If sexual function matters to you, tadalafil has a built-in advantage. If you are already on nitrates for heart disease, tadalafil is off the table entirely.
The Dual Benefit for Erectile Dysfunction
This is arguably tadalafil’s biggest selling point for BPH. A large percentage of men with prostate enlargement also have erectile dysfunction, and the overlap is not coincidental. The same vascular and smooth-muscle mechanisms that contribute to BPH symptoms are involved in achieving erections. Tadalafil, originally developed for ED, treats both problems with a single pill.
A randomized, placebo-controlled trial enrolled men with both conditions and found that tadalafil 5 mg daily significantly improved both erectile function scores and urinary symptom scores over 12 weeks, with the drug well tolerated.13The Journal of Sexual Medicine. Tadalafil 2.5 or 5 mg Administered Once Daily for 12 Weeks in Men with Both Erectile Dysfunction and Signs and Symptoms of Benign Prostatic Hyperplasia: Results of a Randomized, Placebo-Controlled, Double-Blind Study The meta-analysis comparing tadalafil to tamsulosin showed that while symptom-score improvements were equivalent, tadalafil was significantly better at improving erectile function scores.14PubMed Central. Comparative Effectiveness of Tadalafil versus Tamsulosin in Treating Lower Urinary Tract Symptoms Suggestive of Benign Prostate Hyperplasia: A Meta-Analysis of Randomized Controlled Trials
For men who would otherwise need two separate prescriptions (say, tamsulosin for the prostate and sildenafil as needed for erections), daily tadalafil consolidates that into one medication. This simplification can also reduce cost and pill burden, though insurance coverage and out-of-pocket pricing vary widely, especially now that generic tadalafil is available.
Combining Tadalafil with Other BPH Drugs
Some men find that a single medication does not go far enough. Research has explored adding tadalafil to tamsulosin or to finasteride.
A systematic review and meta-analysis of trials comparing tamsulosin plus tadalafil against tamsulosin alone found that the combination significantly improved symptom scores, quality of life, peak urinary flow rate, and erectile function scores. Both regimens were well tolerated.15PubMed Central. A Systematic Review and Meta-Analysis of the Efficacy and Safety of Tamsulosin Plus Tadalafil Compared With Tamsulosin Alone in Treating Males With Lower Urinary Tract Symptoms Secondary to Benign Prostrate Hyperplasia A prospective observational trial found that while total symptom scores and storage symptoms were similar between tadalafil alone and the combination, voiding symptoms and peak flow rate were significantly better with the combined approach.16PubMed Central. Tadalafil 5 mg Alone or in Combination with Tamsulosin 0.4 mg for the Management of Men with Lower Urinary Tract Symptoms and Erectile Dysfunction: Results of a Prospective Observational Trial That improvement in peak flow is notable, because it addresses the one area where tadalafil alone falls short.
The finasteride angle is different. Finasteride shrinks the prostate over time and is used in men with larger glands, but it commonly worsens erectile function. A 26-week trial showed that adding tadalafil 5 mg to finasteride preserved and improved erectile function scores throughout the study period, while placebo plus finasteride showed no erectile improvement and even trended toward decline.17PubMed. Efficacy and safety of the coadministration of tadalafil once daily with finasteride for 6 months in men with lower urinary tract symptoms and prostatic enlargement secondary to benign prostatic hyperplasia So for men who need finasteride for a large prostate, tadalafil acts as an antidote to finasteride’s sexual side effects while adding its own urinary symptom benefit.
Safety, Side Effects, and Drug Interactions
At the 5 mg daily dose used for BPH, tadalafil’s side-effect profile is generally mild. The most common complaints across trials are headache, indigestion, back pain, nasal congestion, and flushing. These effects tend to be mild and often decrease over the first few weeks. Serious adverse events are uncommon.
The critical safety concern is the interaction with nitrate medications, which are used for chest pain (angina). Both tadalafil and nitrates lower blood pressure through overlapping pathways, and combining them can cause a dangerous, sometimes life-threatening drop in blood pressure. This combination is strictly contraindicated.18PubMed. Pharmacology and drug interaction effects of the phosphodiesterase 5 inhibitors: focus on alpha-blocker interactions If you are taking nitroglycerin tablets, nitrate patches, or isosorbide in any form, tadalafil is not an option.
The interaction with alpha-blockers deserves separate attention. Many men with BPH are already on tamsulosin or another alpha-blocker when they start tadalafil. Both drug classes can lower blood pressure, and combining them can sometimes cause orthostatic hypotension, the feeling of lightheadedness or near-fainting when you stand up quickly.19PubMed. Pharmacology and drug interaction effects of the phosphodiesterase 5 inhibitors: focus on alpha-blocker interactions In practice, this combination is used regularly under medical supervision, but your doctor will typically want you stable on the alpha-blocker before adding tadalafil, and may monitor your blood pressure during the transition.
Nighttime Urination
Nocturia, getting up multiple times at night to urinate, is one of the most bothersome BPH symptoms. Sleep fragmentation affects mood, energy, and overall health, and it is often the symptom that drives men to seek treatment in the first place. A Japanese study specifically looked at tadalafil’s effect on nocturia and found that it significantly improved both the number of hours of undisturbed sleep and nocturia-related quality of life. Patients new to treatment saw the largest improvements in overall nocturia quality-of-life scores.20PubMed. Tadalafil Improves Nocturia and Nocturia-Related Quality of Life in Patients with Benign Prostatic Hyperplasia (KYU-PRO Study)
This is worth knowing because nocturia is often the hardest BPH symptom to treat. Alpha-blockers help with daytime voiding symptoms but are less consistent at reducing nighttime trips. The fact that tadalafil specifically improved undisturbed sleep hours, not just symptom questionnaire scores, adds practical weight to the finding.
Does It Work in Older Men
BPH is overwhelmingly a condition of aging, and many patients are in their seventies or older by the time symptoms become disruptive enough to treat. A reasonable concern is whether tadalafil works as well in older age groups, given that cardiovascular health, medication load, and kidney function all change with age. Pooled analyses from multinational trials that specifically broke out results for men aged 75 and older found that tadalafil maintained its efficacy across age groups.21PubMed. Efficacy and safety of tadalafil 5 mg once daily in the treatment of lower urinary tract symptoms associated with benign prostatic hyperplasia in men aged ≥75 years: integrated analyses of pooled data from multinational, randomized, placebo-controlled clinical studies The drug did not lose its symptom-lowering effect in older men, and the safety profile stayed consistent.
That said, older men are more likely to be on medications that interact with tadalafil, particularly nitrates and certain blood pressure drugs. The conversation with your doctor matters more, not less, as the prescription list grows longer.
Why the 5 mg Dose
Tadalafil is available for ED in higher on-demand doses (10 mg and 20 mg taken before sexual activity), so it is reasonable to wonder why BPH treatment uses only 5 mg daily. The dose-finding trial tested 2.5, 5, 10, and 20 mg daily and found a clear dose-response pattern for symptom improvement. All doses outperformed placebo, but the jump from 5 mg to 10 or 20 mg added only modest additional symptom benefit while increasing side-effect exposure.22PubMed. Tadalafil administered once daily for lower urinary tract symptoms secondary to benign prostatic hyperplasia: a dose finding study The 5 mg dose sat at the best balance point between efficacy and tolerability, which is why it became the approved dose for BPH.
The daily dosing matters too. Tadalafil has a half-life of about 17.5 hours, considerably longer than sildenafil or vardenafil. At 5 mg daily, the drug reaches a steady-state concentration in the bloodstream within about five days, providing around-the-clock smooth muscle relaxation rather than the peaks and troughs of on-demand dosing. This pharmacokinetic profile is what makes once-daily dosing feasible and is why tadalafil, rather than one of the other PDE5 inhibitors, ended up with the BPH indication.
What Tadalafil Does Not Do for BPH
It is important to be clear about the limitations. Tadalafil does not shrink the prostate. If you have a significantly enlarged gland, you will still have a significantly enlarged gland after a year of treatment. It reduces the symptoms caused by that enlargement, but it does not address the underlying growth the way finasteride or dutasteride does. For men with very large prostates who are at risk of acute urinary retention or who are heading toward surgery, tadalafil alone may not be enough.
Tadalafil also does not appear to reduce the long-term risk of BPH progression (meaning the likelihood of needing surgery, developing urinary retention, or seeing a substantial worsening of symptoms over years). The trials that established its efficacy were mostly 12 to 26 weeks long. There is no strong evidence that staying on tadalafil indefinitely prevents the disease from advancing the way combination therapy with an alpha-blocker and a 5-alpha reductase inhibitor has been shown to do in large, multi-year trials.
For men with mild to moderate symptoms, particularly those who also have erectile dysfunction, tadalafil is a genuinely useful option that addresses two problems with one daily pill. For men with severe obstruction, large prostates, or high risk of disease progression, it is better positioned as part of a combination strategy rather than a standalone solution.
Generic Availability and Practical Considerations
The brand-name Cialis patent for BPH expired in the United States in 2018, and generic tadalafil 5 mg is now widely available. This has changed the practical calculus considerably. When brand-name Cialis could cost several hundred dollars per month, it was hard to justify for many patients, especially given that inexpensive generic tamsulosin worked about equally well for urinary symptoms. Now that generic tadalafil is comparably priced, the decision between the two comes down more to side-effect preferences and whether erectile dysfunction is also in the picture.
One practical note: tadalafil for BPH must be taken daily to maintain its effect. Skipping doses or taking it only when symptoms feel bad does not work the same way. The drug depends on reaching a steady concentration in your blood. If you stop taking it, symptom relief typically fades within a few days. This is different from how many men think of Cialis, since the on-demand ED version is used only before sexual activity. For BPH, consistency is the whole point.
