Clomipramine Side Effects: Common and Serious Risks

Clomipramine carries a wider range of side effects than most modern antidepressants, largely because it acts on many receptor systems at once rather than targeting serotonin alone. The most common complaints are dry mouth, constipation, drowsiness, weight gain, and sexual difficulties, but the drug also poses rarer risks to the heart, liver, and nervous system that make monitoring important. Because clomipramine remains a first-line treatment for obsessive-compulsive disorder and is also prescribed off-label for conditions like premature ejaculation and certain anxiety disorders, understanding the full picture of what it can do to your body is worth more than a glance at the pharmacy leaflet.

The Everyday Side Effects Most People Notice

The side effects you’re most likely to encounter with clomipramine are the ones tied to its strong anticholinergic activity, meaning the drug blocks a neurotransmitter called acetylcholine that helps regulate saliva, gut movement, bladder function, and sweating. In a head-to-head trial with sertraline for OCD, half the clomipramine group reported dry mouth and half reported weight gain, with constipation affecting about a quarter of patients.

That same trial found sedation in roughly one in five clomipramine users and dizziness in about one in ten.

1Psychiatry and Clinical Psychopharmacology. Clomipramine Versus Sertraline in the Treatment of Obsessive Compulsive Disorder A separate comparison between clomipramine and sertraline in OCD outpatients reported a similar profile: dry mouth in 20%, constipation in 16%, nausea in 15%, and somnolence in 11% of clomipramine patients.2European Psychiatry. A double-blind comparison of sertraline and clomipramine in outpatients with obsessive-compulsive disorder The anticholinergic burden of clomipramine is not subtle: animal research comparing it to SSRIs found that its ratio of anticholinergic effect to antidepressant activity was tighter than that of paroxetine and far tighter than fluvoxamine’s, meaning the anticholinergic side effects show up at doses close to the therapeutic dose rather than well above it.3PubMed. Comparison of the anticholinergic effects of the serotonergic antidepressants, paroxetine, fluvoxamine and clomipramine

Sweating, blurred vision, and urinary hesitancy round out the anticholinergic package. Many of these symptoms are worst in the first few weeks of treatment and ease somewhat as your body adjusts, but dry mouth and constipation tend to persist for as long as you take the drug.

Why So Many People Drop Out of Treatment

Side-effect burden is not just an annoyance with clomipramine; it’s a practical barrier to treatment. In the sertraline comparison trial, 26% of clomipramine patients withdrew because of adverse events, compared with 11% on sertraline. The researchers concluded that the difference in apparent effectiveness between the two drugs was “almost wholly accounted for” by clomipramine’s higher dropout rate.4European Psychiatry. A double-blind comparison of sertraline and clomipramine in outpatients with obsessive-compulsive disorder A second OCD trial found a nearly identical pattern, with clomipramine withdrawals for side effects running about double those of sertraline.5Psychiatry and Clinical Psychopharmacology. Clomipramine Versus Sertraline in the Treatment of Obsessive Compulsive Disorder

This dropout gap matters because OCD treatment typically needs to continue for months or years to remain effective. A drug that works well in theory but makes a quarter of patients quit early is less useful in the real world than one people can actually tolerate. If you’re struggling with clomipramine’s side effects, it’s worth discussing the tradeoff with your prescriber rather than stopping on your own, since abrupt discontinuation carries its own risks.

Sexual Side Effects and Off-Label Use

Sexual dysfunction is one of the most commonly reported problems with clomipramine and, paradoxically, the reason some clinicians prescribe it deliberately. The drug powerfully inhibits serotonin reuptake, which delays orgasm. A systematic review and meta-analysis pooling data from over 300 patients found that clomipramine significantly increased the risk of adverse events compared with placebo when used for premature ejaculation, with a risk ratio of about 3.5.6Sexual Medicine. Tolerability and Optimal Therapeutic Dosage of Clomipramine for Premature Ejaculation: A Systematic Review and Meta-Analysis Those adverse events include delayed ejaculation (the intended therapeutic effect in that context), reduced libido, erectile difficulties, and anorgasmia.

For people taking clomipramine for OCD or depression, these same sexual effects are unwelcome. They can appear at low doses and tend to persist throughout treatment. Unlike sedation or nausea, sexual side effects rarely improve with time, which is one reason they drive discontinuation even when other symptoms have settled.

Heart Rhythm and Blood Pressure Concerns

Like other tricyclic antidepressants, clomipramine can affect the electrical conduction system of the heart. The most serious risk is QT prolongation, a lengthening of the interval between heartbeats on an electrocardiogram that can predispose to a dangerous arrhythmia called torsades de pointes. A recent case report described a patient with acute heart failure and low potassium who developed a QTc interval of 550 milliseconds on clomipramine, experienced multiple episodes of torsades de pointes, and ultimately died from ventricular fibrillation.7PubMed Central. Clomipramine potentially induced fatal torsades de pointes in a patient with acute decompensated heart failure: a case report That case involved a patient with significant cardiac disease and an electrolyte imbalance, which are major compounding risk factors. In otherwise healthy individuals at standard doses, the cardiac risk is low, but it is not zero.

Orthostatic hypotension, the lightheaded feeling when you stand up too quickly, is a more everyday cardiovascular concern. Clomipramine blocks alpha-1 adrenergic receptors, which help maintain blood pressure when you change position. This effect can cause falls, particularly in older adults, and tends to be more pronounced at higher doses or when combined with other blood-pressure-lowering drugs.

Seizure Threshold

Among antidepressants, clomipramine has a relatively high potential to trigger seizures. A review of psychotropic drugs and seizure risk specifically singled out clomipramine (alongside maprotiline) as carrying a higher seizurogenic potential than other antidepressants.8PubMed. Effects of psychotropic drugs on seizure threshold The risk is dose-dependent: at standard therapeutic doses it remains uncommon, but it climbs steeply at high doses or in overdose. People with a history of seizures, head injury, or alcohol withdrawal are at higher risk. This is one reason prescribers generally avoid pushing clomipramine above 250 mg per day and are cautious about combining it with other drugs that also lower the seizure threshold.

Serotonin Syndrome

Because clomipramine is one of the most potent serotonin-reuptake inhibitors among the tricyclics, it carries a meaningful risk of serotonin syndrome when combined with other serotonergic drugs or taken in excess. Serotonin syndrome involves a triad of mental-status changes (agitation, confusion), autonomic instability (fever, sweating, rapid heart rate), and neuromuscular abnormalities (clonus, tremor, rigidity). A case report highlighted how excessive clomipramine use contributed to serotonin syndrome in a young man, and that collateral information about his overuse of the drug was not obtained early enough to guide diagnosis.9PubMed Central. Identifying Early Presentation of Serotonin Syndrome in a 20-Year-Old Man Prescribed Clomipramine and Quetiapine: A Case Report

The practical lesson is that clomipramine should not be combined with MAO inhibitors, other tricyclics, SSRIs, SNRIs, triptans, tramadol, or St. John’s wort without very careful supervision. Even switching from clomipramine to an SSRI (or the reverse) requires a washout period. If you develop a high fever, muscle stiffness, and confusion while on clomipramine, especially after a dose increase or after adding a new medication, seek emergency care.

Mania and Mood Switching

Clomipramine can flip some patients from depression into mania or hypomania. In a controlled trial of hospitalized patients with unipolar depression, six out of 25 clomipramine-treated patients developed manic behavior, compared with just one of 25 on amitriptyline.10PubMed. Clomipramine-induced mania in unipolar depression A systematic review confirmed that high-dose antidepressants, clomipramine and SSRIs in particular, are a recognized trigger for manic or hypomanic episodes in people being treated for OCD.11PubMed. First manic/hypomanic episode in obsessive-compulsive disorder patients treated with antidepressants: A systematic review

This risk is highest in people who have an underlying bipolar spectrum disorder that hasn’t been diagnosed yet. OCD and bipolar disorder overlap more often than commonly appreciated, and clomipramine’s potent serotonergic action can unmask latent bipolar vulnerability. If you notice uncharacteristic euphoria, drastically reduced need for sleep, racing thoughts, or impulsive behavior after starting clomipramine, contact your prescriber promptly.

Liver Effects

Drug-induced liver injury from clomipramine is uncommon but documented. A large drug-surveillance program found that clomipramine had one of the highest rates of liver injury among antidepressants, at 0.23%, and was associated with elevations in multiple liver enzymes.12PubMed Central. Drug-Induced Liver Injury during Antidepressant Treatment: Results of AMSP, a Drug Surveillance Program While 0.23% sounds small, it places clomipramine among the top three antidepressants for this particular adverse event. Laboratory research in rat liver tissue has confirmed that clomipramine can directly impair mitochondrial energy production in liver cells, providing a plausible biological mechanism for the clinical liver injury reports.13PubMed. The harmful acute effects of clomipramine in the rat liver: Impairments in mitochondrial bioenergetics

Routine liver-function blood tests are reasonable during the first months of treatment, especially if you drink alcohol regularly, take other medications processed by the liver, or have pre-existing liver disease. Symptoms like unexplained fatigue, dark urine, or yellowing of the skin warrant immediate evaluation.

Skin Reactions and Photosensitivity

Skin-related side effects of clomipramine are rarely mentioned but can be distressing when they occur. A documented case involved a farmer who developed severe photodermatitis with liver involvement while taking tricyclic antidepressants; patch testing confirmed both photoallergy and contact allergy to clomipramine specifically.14PubMed. A case of photosensitivity and contact allergy to systemic tricyclic drugs, with unusual features What made this case unusual was that the contact sensitivity could also be triggered by oral administration of the drug without any sun exposure. If you notice unexplained rashes, sunburn-like reactions at lower-than-normal UV exposure, or worsening skin irritation while on clomipramine, let your doctor know.

Eye Pressure and Angle Closure

Clomipramine’s anticholinergic properties can dilate the pupil, which in people with anatomically narrow drainage angles in the eye can increase intraocular pressure and, in rare cases, trigger an acute angle-closure glaucoma attack. A study comparing patients on long-term psychotropic medications found that those on tricyclic antidepressants (including clomipramine) had narrower anterior chamber angles than patients on lithium, suggesting a higher baseline risk for angle closure.15PubMed Central. Comparing the effect of sympathomimetic/anticholinergic psychotropic drugs with that of non-sympathomimetic/anticholinergic psychotropic drugs on IOP and angle parameters among patients on long-term therapy If you have a family history of glaucoma or have been told you have narrow angles, mention it before starting clomipramine.

What Happens When You Stop

Discontinuing clomipramine abruptly can produce a withdrawal syndrome even at modest doses. Case reports describe a 45-year-old man who developed headache, muscle pain, fatigue, and insomnia after suddenly stopping 75 mg per day, and a 47-year-old woman who experienced severe insomnia, anxiety, and agitation after discontinuing a dose as low as 25 mg per day.16PubMed. Withdrawal reactions after clomipramine Symptoms can include flu-like body aches, nausea, dizziness, vivid dreams, and rebound anxiety. Gradual tapering over several weeks is the standard approach to minimize these effects. If you and your prescriber decide to stop clomipramine, plan the taper in advance rather than running out of pills and going cold turkey.

Overdose Safety Compared to Other Tricyclics

One relative bright spot in clomipramine’s safety profile is that it appears to cause fewer deaths per prescription in overdose than the average antidepressant. A BMJ study analyzing mortality from antidepressant overdose found that clomipramine, along with lofepramine and trimipramine, had a significantly lower death rate per million prescriptions than expected when compared to all antidepressants as a class.17BMJ. Relative mortality from overdose of antidepressants This does not mean clomipramine overdose is safe, because tricyclic overdoses in general can be fatal through cardiac arrhythmias and seizures. But it suggests clomipramine is somewhat less lethal in overdose than older tricyclics like amitriptyline or dothiepin.

The Role of Blood Level Monitoring

Clomipramine is metabolized by liver enzymes that vary considerably from person to person. Some people are “poor metabolizers,” meaning they break down the drug slowly and can accumulate unexpectedly high blood concentrations even at standard doses. A case series found that patients with excessive blood levels of clomipramine typically failed to respond to treatment and experienced more side effects. When their doses were reduced to bring blood concentrations down, they improved.18PubMed. High blood concentrations of imipramine or clomipramine and therapeutic failure: a case report study using drug monitoring data Some of these patients were identified as poor metabolizers of debrisoquine, a marker for a specific enzyme pathway. Others had been prescribed a phenothiazine alongside their antidepressant, which competed for the same metabolic pathway and effectively raised drug levels.

The takeaway is that if you’re experiencing significant side effects at a dose your prescriber considers moderate, or if the drug doesn’t seem to be working despite dose increases, a blood-level measurement can clarify whether the issue is too much drug in your system rather than too little.

Older Adults and Clomipramine

You might expect older adults to have a distinctly worse side-effect profile with clomipramine, but the picture is more nuanced. A study comparing younger and older depressed patients on clomipramine found no significant differences across 44 recorded side effects, with one exception: older patients had more pronounced orthostatic hypotension.19PubMed. Age-related adverse drug reactions to clomipramine That single finding is clinically important, though, because orthostatic hypotension in older adults leads to falls, which can lead to hip fractures and head injuries. Older adults also tend to take more medications, increasing the chance of drug interactions that affect clomipramine levels or compound its anticholinergic and cardiac effects.

Clomipramine in Veterinary Medicine

Clomipramine isn’t only prescribed to humans. It’s approved in several countries for the treatment of separation anxiety in dogs, sold under brand names like Clomicalm. The side-effect profile in dogs differs from humans in some ways. A large randomized trial in dogs found that the most common side effect was mild, transient vomiting in a small number of animals.20Applied Animal Behaviour Science. Treatment of separation anxiety in dogs with clomipramine: results from a prospective, randomized, double-blind, placebo-controlled, parallel-group, multicenter clinical trial However, a case report linked clomipramine to acute pancreatitis in a dog, and the authors suggested that the drug should be considered as a potential cause when investigating pancreatitis in susceptible breeds.21PubMed.
