Neither Coke Zero nor Diet Coke is meaningfully healthier than the other. Both contain zero calories, zero sugar, and share most of the same base ingredients. The differences between them are small enough that choosing one over the other won’t change your health outlook in any significant way. That said, they aren’t identical, and the details are worth understanding if you drink either one regularly.
Where the Ingredients Actually Differ
The two drinks share a core formula: carbonated water, caramel color, phosphoric acid, aspartame, potassium benzoate, natural flavors, and caffeine. The differences come down to two things: sweeteners and acids.
Diet Coke uses aspartame alone. Coke Zero uses aspartame plus a second sweetener called acesulfame potassium (sometimes listed as Ace-K). This combination is what helps Coke Zero taste closer to regular Coca-Cola, while Diet Coke has its own distinct flavor profile that doesn’t try to mimic the original.
The acid systems differ too. Diet Coke contains citric acid alongside phosphoric acid. Coke Zero swaps in potassium citrate instead. This is a minor formulation choice that affects flavor balance more than health.
Caffeine and Nutrition
Diet Coke has noticeably more caffeine: 46 mg per 12-ounce can compared to 34 mg in Coke Zero. That’s about a 35% difference. If you’re drinking multiple cans a day or you’re sensitive to caffeine, this gap could matter. For reference, a standard cup of coffee has roughly 95 mg, so both sodas are still on the lower end.
Everything else on the nutrition label is essentially identical. Both have zero calories, zero protein, zero fat, and zero sugar. Neither one provides any nutritional value.
What the Sweeteners Do in Your Body
Aspartame, the primary sweetener in both drinks, doesn’t raise blood sugar. The same goes for acesulfame potassium in Coke Zero. If you have diabetes and you’re wondering whether one is safer for blood glucose management, neither will directly spike your levels.
The more complicated picture involves what happens over time. The World Health Organization released a guideline in 2023 advising against using non-sugar sweeteners for weight control. Their systematic review found that replacing sugar with these sweeteners doesn’t reduce body fat in the long term. The review also flagged potential links to increased risk of type 2 diabetes and cardiovascular disease with long-term use, though the WHO noted these findings could be influenced by the overall health patterns of people who tend to drink diet sodas. Both aspartame and acesulfame potassium fall under this guidance.
On gut health, early research suggests the sweeteners in these drinks may affect your gut bacteria differently. A 2025 study in Frontiers in Microbiology found that acesulfame potassium (the extra sweetener in Coke Zero) increased the diversity of gut microbes but disrupted the structural relationships between bacterial communities, potentially weakening the microbiome’s long-term resilience. This is preliminary research, and the doses used in lab studies don’t always reflect what you’d get from a can of soda. Still, it’s one area where Coke Zero’s additional sweetener introduces a variable that Diet Coke doesn’t have.
The Aspartame Safety Question
Both drinks contain aspartame, so any safety concerns about this sweetener apply equally to both. In 2023, the International Agency for Research on Cancer classified aspartame as “possibly carcinogenic to humans” (Group 2B), based on limited evidence linking it to liver cancer. That classification sits in the same category as aloe vera and pickled vegetables. It signals that the evidence isn’t strong enough to confirm a cancer risk but warrants attention.
At the same time, a joint WHO and FAO expert committee reaffirmed the long-standing safe intake level of 40 mg of aspartame per kilogram of body weight per day. For a 150-pound person, that works out to roughly 13 to 14 cans of Diet Coke or Coke Zero daily. Most people don’t come close to that threshold.
One group does need to pay attention: people with phenylketonuria (PKU), a genetic condition that prevents the body from properly processing phenylalanine. Aspartame breaks down into phenylalanine during digestion, and for people with PKU, this can cause serious neurological harm. Both drinks carry a warning label for this reason, and both are equally off-limits if you have PKU.
Effects on Your Teeth
Sugar-free doesn’t mean tooth-safe. Both Coke Zero and Diet Coke are acidic enough to erode tooth enamel over time. The phosphoric acid they share is a primary driver of this erosion, and research shows diet sodas cause roughly the same amount of dental damage as their sugary counterparts. The different secondary acids (citric acid in Diet Coke, potassium citrate in Coke Zero) don’t create a clear winner here. Both sit at low pH levels that put enamel at risk with frequent exposure.
If you’re concerned about your teeth, how you drink matters more than which one you pick. Sipping slowly throughout the day bathes your teeth in acid repeatedly. Drinking with meals or using a straw reduces contact with enamel.
So Which One Should You Choose?
If you’re choosing between these two for health reasons, the honest answer is that it doesn’t matter much. The nutritional profiles are nearly identical, the sweeteners are both FDA-approved, and neither drink offers any health benefit beyond being a zero-calorie alternative to regular soda.
The small differences that do exist: Diet Coke has more caffeine. Coke Zero has an additional sweetener that may interact with gut bacteria in ways we don’t fully understand yet. Neither difference is large enough to make one drink clearly better or worse. Pick whichever one you prefer the taste of, and if you’re drinking several cans a day, the more relevant health question isn’t which zero-calorie soda to choose but how much of either one you’re consuming over time.
