Different Types of Dyslexia and How They’re Diagnosed

Dyslexia isn’t a single condition with one set of symptoms. It shows up in several distinct patterns, each affecting reading through a different mechanism. Some people struggle to connect letters with sounds, others can’t recognize familiar words on sight, and still others read accurately but painfully slowly. Understanding which type is at play matters because it shapes what kind of support actually helps. Dyslexia affects roughly 20 percent of the population and accounts for 80 to 90 percent of all learning disabilities.

Phonological Dyslexia

This is the most common and most widely recognized form. Phonological dyslexia involves difficulty matching sounds to letter symbols and breaking words down into their individual sounds. If you have this type, sounding out unfamiliar words feels like trying to crack a code without a key. The word “fantastic,” for example, doesn’t naturally split into “fan-tas-tic” in your mind the way it does for other readers.

The underlying issue sits in how the brain processes the sound structure of language. Two brain regions involved in this processing, located in the left hemisphere near the temples and behind the forehead, show different activation patterns in people with phonological dyslexia. This isn’t about intelligence or effort. The wiring for linking printed letters to spoken sounds simply works differently.

Because so much early reading instruction relies on “sounding it out,” phonological dyslexia tends to surface early. Children in kindergarten and first grade may not associate letters with their sounds, can’t sound out simple words like “cat” or “map,” and make reading errors that seem disconnected from the letters on the page, like saying “puppy” when the word is “dog” because they’re guessing from a picture instead.

Surface Dyslexia

Surface dyslexia is almost the mirror image of the phonological type. A person with surface dyslexia can sound out new words without much trouble but fails to recognize familiar words by sight. The brain doesn’t store what a word “looks like” in a way that allows quick, automatic recognition.

This creates a specific problem with irregular words, the ones that don’t follow standard spelling rules. English is full of them: “yacht,” “colonel,” “enough.” These words can’t be decoded purely by sounding them out, so they need to be memorized as visual patterns. A reader with surface dyslexia has to laboriously sound out every word, even ones they’ve encountered hundreds of times. The part of the brain responsible for storing these visual word templates, located at the back of the left hemisphere, doesn’t build that instant-recognition library the way it typically would.

Reading with surface dyslexia is slow and exhausting, not because the person lacks decoding skills, but because nothing ever becomes automatic. Imagine having to spell out every word on a highway sign instead of recognizing it at a glance.

Rapid Naming Dyslexia

Some people can decode words and recognize them by sight but still read far more slowly than expected. Rapid naming dyslexia involves difficulty quickly retrieving the names of letters, numbers, colors, or objects when they appear visually. A person with this type can identify a letter or number correctly, but it takes noticeably longer to come up with the name.

This delay compounds across a sentence, a paragraph, a page. Reading speed drops, and the mental effort required to keep retrieving word after word leads to fatigue. Adults with this pattern often describe reading as draining. They rarely read for pleasure, take much longer with books or manuals, and may avoid subtitles in films because the text disappears before they’ve finished processing it.

Double Deficit Dyslexia

When phonological difficulties and rapid naming problems occur together, the result is called double deficit (or mixed) dyslexia. A person with this type struggles both to decode unfamiliar words and to quickly name the symbols they see. Neither route to reading, sounding out or rapid recognition, works smoothly.

This combination tends to produce more significant reading challenges than either deficit alone, because the two systems that typically compensate for each other are both compromised. Someone with phonological dyslexia alone might lean on memorized word shapes to get by; someone with rapid naming issues alone can still sound words out. With a double deficit, both backup strategies are weakened.

Acquired Dyslexia

All the types above are developmental, meaning they’re present from birth and show up as a child learns to read. Acquired dyslexia is fundamentally different. It occurs in people who previously read normally but lose that ability after brain damage from a stroke, head injury, or other neurological event.

The medical term for this is alexia, and it takes different forms depending on which part of the brain is affected. Damage to the back of the left hemisphere can leave a person unable to read but still able to write, a strange disconnect where their hand knows how to form words their eyes can no longer interpret. Damage to a region near the left temple and parietal lobe can impair both reading and writing simultaneously. Damage to the left frontal lobe produces yet another pattern, typically alongside broader language difficulties.

The key distinction is that acquired dyslexia represents a loss of skills that were already established, while developmental dyslexia reflects a difference in how those skills develop in the first place.

Directional Difficulties

You may have heard the term “directional dyslexia” used to describe persistent left-right confusion, trouble following directions, or getting lost easily, even in familiar places. This is not a formal diagnosis. It describes a set of spatial awareness challenges that sometimes co-occur with dyslexia but can also exist independently.

People with these difficulties may consistently turn the wrong way when given directions, struggle to read maps, have trouble judging distances, or find it hard to remember a sequence of turns on a route. These spatial issues can overlap with reading difficulties, but they involve a distinct set of skills. If spatial confusion is your primary challenge, it’s worth exploring whether it connects to dyslexia or to a separate issue with spatial processing.

Visual Stress and Reading

Some people experience visual discomfort while reading: words seem to shimmer, blur, or move on the page. This has been called scotopic sensitivity syndrome or visual stress, and it has been proposed as a visual subtype of dyslexia. However, the scientific support for this as a distinct syndrome is thin. The Royal College of Ophthalmologists found no scientific evidence confirming the condition exists as described, and treatments like colored overlays or tinted lenses have not shown reliable effectiveness in well-designed studies.

That doesn’t mean the discomfort isn’t real. Visual fatigue during reading is a genuine experience for many people. But current evidence suggests it’s better understood as a symptom that may accompany dyslexia rather than a separate type of it.

How Dyslexia Is Officially Diagnosed

Clinically, dyslexia falls under the umbrella of “specific learning disorder with impairment in reading.” A diagnosis requires that reading difficulties have persisted for at least six months despite targeted help, that reading skills fall substantially below what’s expected for the person’s age, and that the struggles aren’t better explained by other conditions like vision problems, intellectual disability, or lack of adequate instruction.

The diagnosis can specify which reading skills are affected: word reading accuracy, reading speed and fluency, or reading comprehension. This framework doesn’t use the subtype labels like “phonological” or “surface” that researchers and clinicians use informally, but the specifiers map onto similar territory. A person whose primary difficulty is word reading accuracy likely has a phonological profile, while someone whose main issue is reading speed may fit the rapid naming pattern.

Signs Across Different Ages

Dyslexia looks different depending on when you’re looking for it. In preschoolers, the signs are pre-reading: trouble learning nursery rhymes, difficulty remembering letter names, not recognizing rhyming patterns, and persistent mispronunciation of familiar words. A family history of reading difficulties is one of the strongest early indicators, since dyslexia runs in families.

By kindergarten and first grade, the signs become more concrete. A child can’t sound out simple three-letter words, doesn’t connect letters to their sounds, and may complain that reading is hard or try to avoid it entirely. In later elementary years through high school, reading remains slow and awkward. Unfamiliar words prompt wild guesses rather than systematic decoding, and reading aloud becomes something to avoid at all costs.

Adults with dyslexia often describe a lifetime of compensating. Reading skills may have improved over the years, but reading still requires enormous effort and happens slowly. Spoken language can be affected too: difficulty retrieving words, frequent “tip of the tongue” moments, mispronouncing names and places, and a smaller spoken vocabulary compared to what they understand when listening. Many adults with dyslexia report extreme fatigue after reading, sacrificing social time to keep up with work or study, and a persistent private worry about being perceived as less capable, despite evidence to the contrary.
