Does Apple Cider Vinegar Help With PCOS Symptoms?

Apple cider vinegar shows some early promise for PCOS, particularly for improving insulin sensitivity and restoring ovulation, but the evidence is still limited to very small studies. It’s not a replacement for established treatments, though it may offer modest benefits as part of a broader approach to managing the condition.

What the Research Actually Shows

The most cited study on this topic is a small Japanese trial that gave a daily vinegar beverage to seven women with PCOS. Within 40 days, four of the seven women resumed ovulatory menstrual cycles. That’s a striking result, but seven participants is an extremely small sample. No large, well-controlled trials have replicated this finding yet, so it’s best treated as encouraging but preliminary.

A broader systematic review and meta-analysis published in BMC Complementary Medicine and Therapies looked at apple cider vinegar’s effects on blood sugar and blood fat levels across multiple clinical trials (not limited to PCOS). The review found that vinegar can improve blood sugar control through several pathways: slowing the rate at which food leaves the stomach, helping cells absorb and use glucose more efficiently, reducing the liver’s production of new glucose, and supporting insulin release. These mechanisms are particularly relevant to PCOS because insulin resistance drives many of the condition’s symptoms, from irregular periods to excess androgen production.

Why Insulin Resistance Matters in PCOS

Up to 70% of women with PCOS have some degree of insulin resistance. When your body doesn’t respond well to insulin, the pancreas produces more of it to compensate. High insulin levels signal the ovaries to produce more testosterone, which disrupts the hormonal balance needed for regular ovulation. This is the central loop that connects weight gain, irregular periods, acne, and excess hair growth in PCOS.

Anything that improves insulin sensitivity has the potential to interrupt this cycle. That’s why the blood sugar effects of acetic acid (the active compound in vinegar) are relevant. By slowing digestion and helping your body process glucose more effectively, vinegar may reduce the insulin spikes that worsen PCOS symptoms. The effect is modest compared to medications designed for this purpose, but it’s a plausible mechanism.

How It Compares to Standard Treatment

A 2024 animal study compared apple cider vinegar to metformin (the most commonly prescribed insulin-sensitizing drug for PCOS) in rats with induced PCOS. Both treatments individually reversed hormonal imbalances, inflammation, high blood sugar, and elevated blood fats. The combination of apple cider vinegar and metformin together produced more promising results than either treatment alone, reducing oxidative stress in ovarian tissue and correcting hormonal disruption more effectively.

This is animal research, so the results don’t translate directly to humans. But the finding that vinegar and metformin may complement each other is noteworthy. It suggests vinegar works through partially different pathways than metformin rather than simply being a weaker version of the same drug.

How People Typically Use It

Most studies and practitioners reference a dose of one to two tablespoons (15 to 30 mL) of apple cider vinegar diluted in a full glass of water, taken before meals. The timing before eating is intentional, since much of the benefit relates to slowing digestion and blunting blood sugar spikes from food. Some people split this into two or three smaller doses across the day rather than taking it all at once.

Always dilute it. Undiluted vinegar is acidic enough to erode tooth enamel over time and irritate your throat and esophagus. Drinking through a straw and rinsing your mouth with plain water afterward can reduce contact with your teeth.

Risks and Interactions

Apple cider vinegar slows the rate at which food moves from the stomach into the small intestine. For most people this is harmless, even beneficial. But if you have gastroparesis (delayed stomach emptying, which is more common in people with diabetes), vinegar can make symptoms like nausea and bloating significantly worse. One controlled study found that two tablespoons of vinegar in water measurably increased the time food stayed in the stomach compared to plain water in patients with gastroparesis.

If you take insulin or medications that stimulate insulin production, adding vinegar could push blood sugar or potassium levels dangerously low. The same blood sugar-lowering effect that makes vinegar potentially helpful can become a problem when stacked on top of medication designed to do the same thing. This is especially relevant for PCOS patients already on metformin or other insulin-sensitizing drugs.

People taking diuretics should also be cautious, since vinegar may further deplete potassium levels. If you’re on any of these medications, it’s worth discussing vinegar intake with whoever manages your prescriptions before adding it to your routine.

Putting It in Perspective

Apple cider vinegar is not a PCOS treatment in the way that medications, dietary changes, or exercise are. The biological mechanisms are plausible, the early human data is encouraging, and the animal research suggests it may work well alongside conventional treatment. But the total body of evidence in women with PCOS specifically remains very small.

Where vinegar fits best is as one component of a broader strategy focused on improving insulin sensitivity. Regular physical activity, a diet that minimizes blood sugar spikes (lower in refined carbohydrates, higher in fiber, protein, and healthy fats), adequate sleep, and stress management all have stronger evidence bases for PCOS. Adding a tablespoon of diluted vinegar before meals is unlikely to hurt and may offer a small additional benefit on top of those foundations, but it’s not going to override poor sleep or a sedentary lifestyle on its own.
