Fish oil has not been proven to fade existing acne scars. No clinical trial has directly tested whether omega-3 supplements improve the appearance of acne scarring. However, the biological properties of fish oil, particularly its anti-inflammatory effects and influence on wound healing, suggest it could play a modest supporting role in how your skin heals after acne and how visible those marks become over time.

Most acne scars form because of prolonged, deep inflammation. The more inflamed a breakout, the more tissue damage occurs, and the worse the eventual scar. Fish oil attacks this problem at its source by shifting the body’s chemical signaling toward an anti-inflammatory state. Specifically, omega-3s block the production of leukotriene B4, a compound that fuels the kind of intense inflammation responsible for cystic and nodular acne lesions.
In a small clinical study, 13 people with inflammatory acne took 3 grams of fish oil (containing 930 mg of EPA) daily for 12 weeks. By week 6, measurable skin redness had decreased, and by week 12, 62% of participants had improved by at least one acne severity grade. Fewer and less severe active breakouts translates directly into fewer new scars forming. If your primary concern is preventing additional scarring from ongoing acne, this is where fish oil has the strongest case.
Fish Oil and Dark or Red Acne Marks
Many people searching about “acne scars” are actually dealing with post-inflammatory marks: the flat red or brown spots left behind after a pimple heals. These aren’t true scars (there’s no change in skin texture), but they can linger for months and are cosmetically frustrating.
DHA, one of the two main fatty acids in fish oil, has been shown in cell studies to reduce melanin production by speeding up the breakdown of tyrosinase, the enzyme your skin uses to make pigment. Related fatty acids like alpha-linolenic acid (ALA, a plant-based omega-3) demonstrated measurable skin-lightening effects in animal studies: after three weeks of topical application, skin lightness values increased significantly, and melanin content dropped by as much as 84% compared to untreated skin.
These are promising signals, but there’s a critical caveat. Most of this research used topical application of fatty acids directly on skin, not oral fish oil capsules. Whether swallowing fish oil delivers enough of these compounds to your skin cells to produce a visible lightening effect on dark marks is unknown. The biological pathway exists, but the delivery method matters enormously.
Atrophic vs. Raised Scars
Acne scars come in two broad categories. Atrophic scars are the indented, pitted kind (ice pick, boxcar, rolling) caused by tissue loss during healing. Don’t expect dramatic changes to existing scars within that window. The realistic benefit is a gradual reduction in redness and inflammation from active acne, potentially fewer new breakouts, and possibly less aggressive scarring from lesions that are still healing. For scars that are already mature (six months or older), fish oil alone is unlikely to produce noticeable improvement.
Side Effects to Know About
Fish oil is generally well tolerated. The most common complaints are digestive: fishy aftertaste, heartburn, nausea, or diarrhea. Taking capsules with food and choosing enteric-coated versions helps with these. At higher doses, fish oil can increase bleeding risk, which is worth noting if you take blood thinners or are planning any skin procedures like microneedling or laser treatments for your scars. A skin rash is a less common side effect, and people with seafood allergies should be cautious since the safety of fish oil in that group isn’t well established.
Where Fish Oil Fits in Scar Treatment
Fish oil is not a scar treatment. It’s a supplement with anti-inflammatory properties that may modestly support your skin’s healing environment. For existing indented acne scars, procedures like microneedling, chemical peels, laser resurfacing, and dermal fillers have far stronger evidence. For post-inflammatory red or dark marks, topical ingredients like vitamin C, niacinamide, retinoids, and sunscreen are more directly effective.
Where fish oil fits is as a background player: reducing systemic inflammation, potentially calming active breakouts, and creating slightly better conditions for your skin to heal cleanly from new lesions. If you’re dealing with ongoing inflammatory acne and want to reduce the chance of future scarring, adding fish oil to your routine is a reasonable, low-risk choice. If you’re looking to fade scars you’ve had for years, your results will come from other treatments.
