Nitroglycerin is best known as a heart medication, but the same mechanism that relieves chest pain also applies to erections. When absorbed through the skin, nitroglycerin breaks down into nitric oxide, a molecule that signals smooth muscle tissue to relax. In the penis, this relaxation opens up blood vessels and allows them to fill with blood, which is the basic physical process behind an erection.

This is actually the same pathway your body uses naturally during arousal. The difference is that nitroglycerin delivers nitric oxide directly through the skin rather than relying on nerve signals to trigger its release. For men whose natural nitric oxide production is reduced (a common issue with aging, diabetes, or cardiovascular disease), the topical gel essentially supplements what the body is no longer producing on its own.

What the Clinical Evidence Shows

A randomized, double-blinded trial of 232 men tested a 0.2% nitroglycerin gel called MED2005 against a placebo. Men using the nitroglycerin gel saw their erectile function scores rise from a baseline of 17.1 to 19.6, compared to 18.5 with placebo. That’s a statistically significant difference, but the gap is relatively small. About 23% of men experienced a clinically meaningful improvement (defined as a 4-point or greater increase in erectile function scores) with the gel alone, compared to 14% who improved on placebo only. Nearly half of participants, 48.4%, did not respond to either treatment.

The clearest benefits showed up in men with mild ED. For those with moderate or severe dysfunction, the 0.2% concentration didn’t move the needle much. Some researchers have tested concentrations ranging from 0.2% to 0.8% for more severe cases, though the higher concentrations come with a greater risk of side effects.

One genuine advantage over oral medications is speed. In the trial, 44% of men noticed the start of an erection within 5 minutes of applying the gel, and nearly 70% noticed effects within 10 minutes. Oral ED pills typically take 30 to 60 minutes to kick in, so the fast onset is a meaningful practical difference for some couples. Partners in the study also reported noticing the improvement, which lined up with the men’s self-reported results.

Over-the-Counter Availability

A related topical gel called MED3000 (sold under the brand name Eroxon) received FDA authorization for marketing as a device for ED treatment in June 2023. This makes it available over the counter in the United States without a prescription, which is a first for an ED product in the U.S. market. MED3000 uses glyceryl trinitrate (the chemical name for nitroglycerin) in a gel formulation designed specifically for penile application before sex.

The over-the-counter status is notable because all other widely used ED treatments, including sildenafil and tadalafil, require a prescription. For men who want to try something without a doctor visit, or who have mild symptoms that don’t justify stronger medications, this is a lower-barrier option.

Side Effects to Expect

Headache is the most common side effect, and it’s essentially built into how the drug works. Nitroglycerin dilates blood vessels throughout the body, not just in the penis, and that dilation in the head causes headaches. Other common reactions include dizziness, lightheadedness, facial flushing, and skin irritation at the application site.

There’s also a practical concern about transfer to a partner during sex. Because nitroglycerin is absorbed through skin contact, a partner could experience headaches or lightheadedness if the gel hasn’t been fully absorbed before intercourse begins. Using a condom or waiting long enough for the gel to absorb can reduce this risk.

The Critical Interaction With ED Pills

This is the most important safety issue with nitroglycerin for ED: you cannot combine it with common oral ED medications like sildenafil (Viagra), tadalafil (Cialis), or vardenafil. These drugs amplify the blood pressure-lowering effects of nitrates, and the combination can cause a dangerous, potentially fatal drop in blood pressure.

This isn’t a theoretical risk. The interaction has caused deaths. Sildenafil is absolutely contraindicated in anyone taking any form of nitrate, whether long-acting daily nitrates for heart disease or a one-time topical application for ED. The danger works in both directions: if you’ve recently used nitroglycerin gel, you should not take an oral ED pill, and if you’ve recently taken an oral ED pill, you should not use nitroglycerin.

If you currently take nitrate medications for a heart condition, this interaction also means oral ED pills are off the table for you. Ironically, topical nitroglycerin might be one of the few pharmacological options available in that situation, since you’re already on nitrates and the topical form simply adds a local dose rather than introducing a conflicting drug class.

Who It Works Best For

Nitroglycerin gel occupies a specific niche. It’s best suited for men with mild ED who want a fast-acting, non-prescription option, or for men who can’t take oral ED medications due to nitrate use or other contraindications. The clinical data consistently shows that men with mild dysfunction see the most benefit, while those with moderate to severe ED are less likely to notice a meaningful difference at the standard 0.2% concentration.

Men who’ve had good results with oral medications and are looking for something stronger will likely find nitroglycerin gel underwhelming. The response rates are lower, and the magnitude of improvement is smaller than what’s typical with prescription pills. But for the right candidate, particularly someone with mild symptoms who values the quick onset and over-the-counter access, it fills a gap that didn’t have a good solution before.
