Tretinoin can improve skin quality around marionette lines, but it cannot eliminate them. Chemical peels and microneedling work in a similar category: they can improve the appearance of early lines and skin texture but cannot address deep folds.
For moderate to deep marionette lines, dermal fillers are the most direct treatment. Hyaluronic acid fillers can be injected into the fold itself to physically fill the crease, or placed strategically in the cheeks and jawline to restore volume that has been lost with age. This addresses the structural cause of marionette lines in a way that no topical product can. Results are immediate and typically last several months to over a year depending on the filler used.
Thread lifts and surgical options target the sagging and ligament laxity that contribute to deeper lines. These are more invasive but address the gravitational component that topicals and even fillers cannot fully correct.
A Realistic Approach
Think of tretinoin as a skin-quality treatment rather than a wrinkle eraser for marionette lines. It builds stronger, thicker skin that holds up better over time, and it can meaningfully soften shallow creases in the lower face. Used consistently for six months or more, it makes the area look healthier and can reduce how severe marionette lines appear. For deeper lines, it works best as one piece of a broader approach rather than a standalone solution. Many people use tretinoin alongside periodic filler treatments, getting structural correction from the filler and long-term skin improvement from the retinoid.
