Yes, heartburn is a recognized side effect of Wegovy. The FDA lists gastroesophageal reflux disease among the most common adverse reactions, occurring in about 5% of adults taking the medication compared to 3% of those on placebo in clinical trials. While that’s a relatively modest increase in risk, it’s consistent enough that the Wegovy medication guide specifically names heartburn as a side effect to watch for.
How Common Heartburn Is on Wegovy
In the three major clinical trials (known as the STEP trials) that led to Wegovy’s approval, researchers tracked 2,116 adults taking the full 2.4 mg dose. Five percent of them reported reflux symptoms, versus 3% in the placebo group. That means roughly 1 in 20 people on Wegovy experienced notable heartburn or acid reflux.
The pattern holds in younger patients too. In a separate 68-week trial of 201 adolescents aged 12 and older, 4% of those on Wegovy reported reflux compared to 2% on placebo. Heartburn from Wegovy isn’t age-specific; it’s tied to how the drug works in the body.
Why Wegovy Causes Heartburn
Wegovy (semaglutide) works partly by slowing down how fast your stomach empties food into your small intestine. This delayed gastric emptying is actually one of the reasons the drug helps with weight loss: food sits in your stomach longer, so you feel full sooner and stay full for more time. But that same mechanism creates the conditions for heartburn.
When food and stomach acid linger in the stomach longer than usual, acid has more opportunity to splash upward into the esophagus. That backwash of acid is what produces the burning sensation in your chest or throat. If you already had mild reflux before starting Wegovy, the slower digestion can make it noticeably worse. And even people who never had heartburn before may develop it for the first time on the medication.
Foods and Habits That Make It Worse
Because Wegovy slows digestion, certain foods become more problematic than they’d normally be. Greasy and fried foods are the biggest culprits. Fried chicken, burgers, heavy cream sauces, and pizza all sit in your stomach even longer when digestion is already delayed, giving acid more time to irritate your esophagus. Cutting back on high-fat, fried, and sugary foods is one of the most effective ways to reduce heartburn while on the medication.
Portion size matters more on Wegovy than it does off it. Even healthy foods can trigger discomfort if you eat too much in one sitting, because your stomach stays full for longer and overeating creates pressure that can push acid upward. A few practical changes that help:
- Eat smaller meals spread throughout the day rather than two or three large ones
- Eat slowly and stop as soon as you feel satisfied, not stuffed
- Prioritize protein at each meal, which tends to cause less reflux than fatty foods
- Try bland foods like bananas, eggs, chicken breast, and mashed sweet potatoes during flare-ups
Timing also plays a role. Lying down shortly after eating gives acid an easier path to your esophagus. Staying upright for at least two to three hours after meals, and elevating the head of your bed slightly if nighttime heartburn is an issue, can make a noticeable difference.
Using Heartburn Medications With Wegovy
If dietary changes aren’t enough, over-the-counter heartburn remedies are generally compatible with Wegovy. No drug interactions have been identified between semaglutide and common acid-reducing medications like omeprazole (Prilosec). Standard antacids, H2 blockers, and proton pump inhibitors all remain options while you’re on the medication. That said, it’s worth mentioning any new medications to your prescriber so they can keep track of your full picture.
When Heartburn Needs Attention
For most people, Wegovy-related heartburn is mild and manageable, especially with the dietary adjustments above. It also tends to be most noticeable during dose increases, since the drug is titrated up gradually over several months. Some people find their symptoms ease as their body adjusts to each new dose level.
Severe or persistent stomach problems are a different story. Novo Nordisk, the maker of Wegovy, advises contacting your prescriber if you develop stomach symptoms that are severe or won’t go away. Warning signs that go beyond typical heartburn include intense abdominal pain, vomiting that doesn’t stop, difficulty swallowing, or reflux that worsens steadily over weeks despite changes to your diet and over-the-counter treatment. These could signal complications like esophageal irritation that may need direct evaluation.
Heartburn that responds to smaller meals, fewer greasy foods, and an occasional antacid is a manageable trade-off for many people on Wegovy. Heartburn that disrupts your sleep, limits what you can eat, or keeps getting worse is worth bringing up at your next appointment rather than waiting it out.
