Hiccups that drag on for an entire day are almost always caused by something irritating the nerves that control your diaphragm, the dome-shaped muscle under your lungs that drives each breath. Most hiccup bouts last a few minutes, so when yours have stuck around for hours, your body is telling you that something is keeping that nerve pathway fired up. The good news: hiccups lasting less than 48 hours are usually triggered by everyday habits and tend to resolve on their own or with simple techniques.
What Actually Happens During a Hiccup
A hiccup is an involuntary spasm of your diaphragm and the small muscles between your ribs, followed by a sudden snap of your vocal cords shutting. That snap is the “hic” sound. The whole event is driven by a reflex arc, a loop of nerve signals that starts with two key nerves: the vagus nerve (which runs from your brain down through your neck, chest, and abdomen) and the phrenic nerve (which directly controls the diaphragm). When something irritates either of these nerves, the loop can fire over and over without a clear off switch.
The brain regions that process this reflex sit in the brainstem and upper spinal cord, roughly at the level of your neck. That’s why so many different triggers, from your stomach to your throat to your ears, can set hiccups off. Anything that touches or irritates any part of this long nerve pathway can keep the spasms going.
The Most Likely Reasons Your Hiccups Won’t Stop
For hiccups lasting less than 48 hours, the triggers are overwhelmingly lifestyle-related. The most common culprits include drinking carbonated beverages, eating too much or too fast, drinking too much alcohol, emotional stress or excitement, sudden temperature changes, and swallowing air from chewing gum or smoking. If any of these sound like your day, that’s probably your answer.
Carbonation and overeating are especially common because they distend the stomach, which sits right below the diaphragm. A bloated stomach physically pushes against the diaphragm and irritates the vagus nerve where it passes through the area. Alcohol works similarly: it relaxes the valve between your stomach and esophagus, promotes stomach distension, and can directly irritate the nerve lining.
Hot food followed by a cold drink (or vice versa) is a classic setup. The sudden temperature shift stimulates the vagus nerve in the throat and esophagus, kicking off the reflex. Stress works through a different path, likely by altering breathing patterns and activating the sympathetic nervous system, which is also part of the hiccup reflex arc.
Acid Reflux: A Hidden Driver
If your hiccups keep coming back day after day, acid reflux deserves a closer look. Stomach acid backing up into the esophagus irritates the vagus nerve along its entire length, and this irritation can trigger hiccup bouts that feel random but are actually tied to meals, lying down, or bending over. Many people with reflux don’t experience classic heartburn. Instead, they notice throat clearing, a sour taste, or persistent hiccups.
A related condition, hiatal hernia, can amplify this problem. A hiatal hernia happens when the upper part of the stomach pushes through the diaphragm into the chest cavity. About 95% of hiatal hernias are the sliding type, meaning they shift as you eat and swallow. Burping and hiccupping are recognized symptoms. Because the herniated stomach directly contacts the diaphragm, it provides a constant source of mechanical and chemical irritation that can keep hiccups cycling.
Medications That Cause Hiccups
If you recently started or changed a medication, it could be the trigger. Corticosteroids (commonly prescribed for inflammation, allergies, and autoimmune conditions) and benzodiazepines (used for anxiety and sleep) are the drugs most frequently linked to hiccups. Steroids like dexamethasone are particularly notorious, especially when combined with chemotherapy drugs.
The list extends well beyond those two classes, though. Pain medications including ibuprofen-type drugs and opioids, certain antibiotics, some antidepressants, anti-seizure medications, and even blood pressure drugs have all been reported to cause hiccups. If your all-day hiccups started around the same time as a new prescription, that connection is worth raising with your pharmacist or doctor.
How to Stop Them
Most home remedies for hiccups work on the same principle: they either stimulate or override the vagus nerve, breaking the reflex loop. The techniques that have the most physiological basis include:
- Holding your breath for 10 to 20 seconds. This raises carbon dioxide levels in your blood, which changes the signaling in your brainstem’s breathing center and can interrupt the hiccup reflex.
- Bearing down as if you’re having a bowel movement. This is called the Valsalva maneuver. It increases pressure in your chest and abdomen, directly stimulating the vagus nerve.
- Drinking ice-cold water slowly. The cold stimulates the vagus nerve in the throat, potentially resetting the reflex.
- Swallowing a teaspoon of granulated sugar. The gritty texture on the back of the throat stimulates the vagus nerve at a different point than the hiccup trigger.
- Pulling your knees to your chest and leaning forward. This compresses the diaphragm and changes the mechanical environment around the nerves involved.
For hiccups that have been going all day, you may need to try several of these in sequence rather than relying on one. Combining breath-holding with the bearing-down maneuver tends to be more effective than either alone. Avoid things that worsen stomach distension while you’re trying to stop them: skip the soda, eat small portions, and stay upright after eating.
When All-Day Hiccups Signal Something Bigger
Hiccups that last less than 48 hours are almost never a sign of serious disease. But if yours cross the 48-hour mark, the potential causes shift from lifestyle triggers to medical ones. Persistent hiccups (lasting more than 48 hours) can result from damage or irritation to the vagus or phrenic nerves caused by a sore throat or laryngitis, a growth on the thyroid gland, or even something as simple as a hair touching the eardrum. Central nervous system conditions, metabolic imbalances like electrolyte disturbances or kidney problems, and chronic alcohol use can also drive hiccups that won’t quit.
Surgical procedures and tumors near the chest or neck can irritate the phrenic nerve specifically, sometimes causing hiccups that persist for weeks. In these cases, medical treatment may include nerve-blocking techniques to interrupt the signals triggering the diaphragm contractions.
