High Cortisol Symptoms: How to Know If You Have It

High cortisol produces a recognizable pattern of symptoms that affects your body shape, skin, mood, and sleep. A single symptom like weight gain or anxiety isn’t enough to point to cortisol on its own, but when several of these signs show up together, they paint a clearer picture. Here’s what to look for and how doctors confirm it.

Changes in Body Shape and Fat Distribution

Cortisol directs fat storage to very specific places. The most telling pattern is rapid weight gain concentrated in your face and midsection while your arms and legs stay relatively thin, or even lose muscle. Three signature changes stand out:

  • Moon face: Your face becomes noticeably round and puffy as fat accumulates along the sides. People often notice this in photos before they notice it in the mirror.
  • Central weight gain: Fat builds around your abdomen and trunk, sometimes quickly enough that it’s clearly not explained by diet changes alone.
  • Buffalo hump: A pad of fat develops between your shoulder blades at the base of your neck. This one is distinctive enough that it almost always prompts a doctor to check cortisol levels.

These changes happen because cortisol actively signals your body to store visceral fat (the kind packed around your organs) rather than distributing it evenly. If you’re gaining weight in these areas despite no major changes in how you eat or move, cortisol could be involved.

Skin and Tissue Signs

High cortisol breaks down the proteins that keep skin strong and elastic. Over time, this produces visible changes that are different from normal aging or typical stretch marks.

The most specific sign is wide, pink or purple stretch marks on your stomach, hips, thighs, breasts, or underarms. Regular stretch marks from pregnancy or weight gain tend to be pale or silvery. Cortisol-driven stretch marks are wider and distinctly colored because the skin has thinned so much that blood vessels show through.

You may also notice that your skin bruises from minor bumps that wouldn’t have left a mark before, and that cuts and scrapes take noticeably longer to heal. Acne can flare up as well, sometimes in people who haven’t dealt with breakouts since adolescence. These skin changes tend to develop gradually, so they’re easy to dismiss individually. Together, though, they’re a meaningful clue.

Muscle Weakness and Bone Loss

Cortisol breaks down muscle tissue, particularly in your upper arms and thighs. You might notice that everyday tasks like climbing stairs, standing from a chair, or lifting things overhead feel harder than they used to, even if you haven’t changed your activity level. This weakness is different from general fatigue. It’s a measurable loss of strength in specific muscle groups.

Over longer periods, high cortisol also weakens bones by interfering with the process that rebuilds bone tissue. Some people discover elevated cortisol only after a bone density scan reveals unexpected osteoporosis, or after a fracture from a minor fall that shouldn’t have caused one.

Mood, Sleep, and Cognitive Changes

Cortisol directly affects brain function, and excess levels create a cycle that’s hard to break. Anxiety, irritability, and depression are common, and they often don’t respond well to typical treatments because the underlying hormonal driver is still present.

Sleep disturbances are particularly telling. Cortisol normally follows a daily rhythm: it peaks in the early morning to help you wake up and drops to its lowest levels around midnight. When cortisol is chronically elevated, that rhythm flattens or inverts. You may find yourself wired at night, waking in the middle of the night for no clear reason, or feeling both exhausted and unable to fall asleep. Poor sleep then worsens anxiety and impairs memory and concentration, creating a feedback loop where sleep deprivation itself pushes cortisol even higher.

Difficulty concentrating and feeling mentally foggy are also common. If you’re experiencing mood or cognitive changes alongside the physical symptoms described above, that combination is more significant than either category alone.

Blood Sugar and Blood Pressure

Cortisol raises blood sugar by signaling your liver to release more glucose and making your cells less responsive to insulin. Over time, this can tip into full type 2 diabetes, sometimes in people who have no other risk factors for it. If you’ve been diagnosed with high blood sugar or diabetes that seems out of proportion to your diet and weight, it’s worth asking whether cortisol could be contributing.

High blood pressure follows a similar path. Cortisol causes your body to retain sodium and water, which increases blood volume and puts more pressure on your blood vessels. Unexplained high blood pressure, especially in someone younger or without a family history, can be a sign.

What Causes Cortisol to Stay High

The single most common cause of persistently high cortisol is long-term use of corticosteroid medications, the kind prescribed for asthma, rheumatoid arthritis, lupus, or after organ transplants. These drugs mimic cortisol, so taking them at high doses for months or years produces the same effects as your body overproducing it. If you’re on these medications and noticing symptoms, that connection is worth discussing with your prescriber.

When the body itself overproduces cortisol, the cause is usually a small, noncancerous tumor. Pituitary tumors account for about 8 out of 10 cases of internally driven high cortisol (called Cushing’s syndrome). These tumors send excess signals to the adrenal glands, telling them to keep making cortisol. Less commonly, tumors on the adrenal glands themselves or in other organs like the lungs can drive overproduction.

There’s also a gray zone called pseudo-Cushing’s, where cortisol runs high without progressing to full Cushing’s syndrome. This can happen with chronic depression, anxiety, heavy alcohol use, poorly controlled diabetes, or obesity. The symptoms overlap, which makes testing important for sorting out what’s actually going on.

How Doctors Test Cortisol Levels

No single symptom confirms high cortisol, and no single test does either. Doctors typically use a combination of approaches to build a reliable picture.

Blood Tests

A standard cortisol blood test is drawn in the morning, when levels are naturally highest. Normal morning cortisol (drawn between 6 and 8 a.m.) falls between 10 and 20 mcg/dL. By late afternoon, it should drop to 3 to 10 mcg/dL. Values consistently above these ranges raise suspicion, but a single high reading isn’t diagnostic on its own because cortisol fluctuates throughout the day and spikes with stress, illness, or poor sleep.

Late-Night Salivary Cortisol

This is one of the most useful screening tests. You collect a saliva sample at home between 11 p.m. and midnight, when cortisol should be at its daily low. A normal result is below 100 ng/dL. In confirmed Cushing’s patients, late-night salivary cortisol runs far higher, often between 100 and 6,000 ng/dL. This test works well because it captures whether your cortisol rhythm is intact. Doctors usually want at least two late-night samples on separate nights.

24-Hour Urine Collection

You collect all your urine over a full day into a container, which gives an average cortisol output rather than a single snapshot. Values above 100 mcg per 24 hours suggest high cortisol, though there’s enough variation between individuals that no single cutoff is absolute.

Dexamethasone Suppression Test

You take a low dose of a synthetic steroid pill at night, then have your blood drawn the next morning at 8 a.m. In a healthy system, the pill signals your brain to temporarily dial back cortisol production. If your morning cortisol still comes back above 1.8 mcg/dL after taking the pill, your body isn’t responding to that “stop” signal the way it should, which points toward excess cortisol production.

Recognizing the Pattern

The key to knowing whether your symptoms point to high cortisol is looking at the full picture rather than any one sign. Weight gain alone doesn’t suggest cortisol problems. But weight gain concentrated in your face and trunk, combined with purple stretch marks, easy bruising, muscle weakness, sleep disruption, and mood changes, is a pattern that warrants testing. The more of these signs you recognize in yourself, especially if they developed over the same time period, the stronger the case for getting your levels checked. A combination of the salivary and blood tests described above can give you a clear answer relatively quickly.
