Antipsychotics affect how you feel in ways that go well beyond their intended purpose. Most people notice sedation first, often within hours of the first dose, followed by a range of physical, emotional, and cognitive changes that shift over the coming weeks. The experience varies significantly depending on which medication you’re taking, your dose, and your individual biology, but certain patterns are remarkably consistent across the millions of people who use these drugs.
Sedation Hits First and Hardest
The most immediate sensation most people report is sedation. This isn’t just feeling sleepy at bedtime. It encompasses excessive daytime sleepiness, difficulty thinking or concentrating, and oversleeping. Sedation peaks on the first or second day of treatment, and for many people it’s the dominant experience of the early days on medication.
The good news is that sedation resolves in about half of patients within the first week. By four weeks, most people have adjusted. But roughly 25% of patients continue to experience persistent sedation beyond that point, which may signal the need for a dose change or a switch to a different medication. During those early days, the side effects often arrive well before the therapeutic benefits, which can make the experience feel especially discouraging.
The “Zombie” Feeling
One of the most commonly described experiences is emotional blunting, sometimes called the “zombie effect.” This is a real clinical phenomenon known as neuroleptic-induced deficit syndrome. It resembles the negative symptoms of schizophrenia itself: flattened emotions, apathy, social withdrawal, slowed movement, and reduced speech. You may find it harder to feel excitement, sadness, or motivation. Things that used to interest you may feel distant or irrelevant.
This happens because antipsychotics work by blocking dopamine receptors in the brain, particularly in areas that control motivation and reward. Dopamine is central to how your brain registers pleasure and drives you toward goals. When medication occupies too many of those receptors, the result can feel like the volume on your inner life has been turned way down. The degree of emotional dulling tends to correlate with how strongly a given drug blocks dopamine, which is why older, more potent antipsychotics are more likely to produce this effect.
Restlessness That Comes From Inside
One of the most distressing sensations antipsychotics can cause is akathisia, an intense internal restlessness that makes it nearly impossible to sit still. People with akathisia describe nervousness, an inability to relax, and a strong, almost uncontrollable compulsion to move, especially in their lower body. You might find yourself crossing and uncrossing your legs, pacing back and forth, rocking in your chair, or shifting from foot to foot while standing in line.
What makes akathisia particularly difficult is that it doesn’t look like much from the outside, but internally it causes extreme anxiety and distress. It’s sometimes mistaken for worsening psychiatric symptoms rather than a medication side effect, which can lead to a dose increase that makes things worse. If you develop a new, intense sense of physical unease after starting or increasing an antipsychotic, that distinction matters.
How Thinking and Memory Change
The cognitive effects of antipsychotics are complicated and depend heavily on the specific drug. In the short term, older antipsychotics (sometimes called “typicals” or first-generation drugs) can impair sustained attention and immediate memory. These effects tend to lessen with continued use, and verbal memory may actually improve over time.
Newer antipsychotics generally have a better cognitive profile. Some improve working memory, the ability to hold information in mind and manipulate it. Others show benefits for verbal memory or the ability to filter out distracting information. For example, certain newer medications have been shown to improve selective attention and alertness with long-term use, though effects on sustained attention remain inconsistent. The net cognitive experience for many people is a tradeoff: the medication may slow certain types of processing while clearing away the disorganized thinking that psychosis or mania causes.
Physical Side Effects You Can Feel
Beyond sedation, antipsychotics produce a range of physical sensations that vary by drug class. Older antipsychotics are more likely to cause movement-related effects: increased muscle tone, rigidity, and a loss of fine motor precision. Your handwriting might change. Your movements may feel stiff or mechanical. These physical changes are a major reason people stop taking these medications.
Newer antipsychotics trade some of those movement problems for metabolic effects. Weight gain is common with certain drugs, and it can be substantial. Some people report increased appetite that feels almost compulsive. Changes in sexual function are also widespread. Decreased libido, difficulty with arousal, and other sexual side effects are frequently reported but rarely discussed proactively during treatment.
Older vs. Newer Antipsychotics Feel Different
The two generations of antipsychotics create noticeably different experiences. First-generation drugs block dopamine aggressively and are more likely to cause that stiff, slowed-down, emotionally flat feeling along with movement problems. These physical side effects often produce such negative subjective responses that people stop taking the medication regardless of whether it’s helping their symptoms.
Second-generation (atypical) antipsychotics interact with a broader set of brain chemicals, which generally means fewer movement side effects and, for some patients, improvements in cognitive function and emotional range. However, they come with their own set of concerns, particularly weight gain and metabolic changes. The specific drug matters enormously. Within the newer class, some are more sedating, some are more likely to cause weight gain, and some are relatively neutral on both counts. The subjective experience of one atypical antipsychotic can be very different from another.
What Stopping Feels Like
If you’ve been on antipsychotics and try to stop, there’s a high chance you’ll feel withdrawal effects. In a survey of 585 people who attempted to stop their antipsychotic medication, 72% reported withdrawal symptoms including nausea, tremors, anxiety, agitation, headaches, and insomnia. Over half of those who experienced withdrawal rated it as severe. Insomnia, nervousness, and extreme emotional swings were the most frequently described experiences.
About 18% of people in that survey experienced a return of psychotic symptoms during withdrawal, which makes it difficult to distinguish between withdrawal effects and relapse. But the picture isn’t entirely negative. Around 26% reported positive changes after stopping: feeling more alive, more like themselves, having more energy, and thinking more clearly. These reports align with what many people describe while on the medication, that it suppresses something essential about how they experience the world.
The Timeline of What You’ll Feel
The sequence matters. Side effects typically arrive before benefits. Sedation and physical side effects can start within hours of the first dose. Emotional blunting and akathisia often develop in the first week or two. The actual therapeutic effects on hallucinations, delusions, or mania generally take longer to fully establish, which creates a difficult window where you’re dealing with the costs of the medication before you’re experiencing the benefits. Understanding that timeline can make the early period more tolerable, because for many people, the side effect profile at week one is not what it will be at week six.
