How Bad Is Pre-Workout for You? What Science Says

Most pre-workout supplements aren’t dangerous for healthy adults when used as directed, but they’re not harmless either. The real answer depends on what’s in your specific product, how much caffeine it packs, when you take it, and whether you’re stacking it on top of other stimulants throughout the day. Some ingredients are well-studied and safe. Others ride on minimal evidence, and the industry itself operates with far less oversight than most people assume.

What’s Actually in Pre-Workout

Pre-workout formulas vary widely, but most contain some combination of caffeine, creatine, beta-alanine, branched-chain amino acids (BCAAs), and B-vitamins. Of these, caffeine does the heavy lifting. It slows the process of fatigue in your muscles, which can help you push harder and longer during a session. Creatine is the most researched ingredient in the mix and is effective for increasing strength and lean body mass when paired with regular training. Beta-alanine and BCAAs round out the formula by supporting muscular endurance and protein synthesis.

The combination of these ingredients has been shown to delay fatigue while improving reaction time and muscular endurance. So on the performance side, there’s legitimate benefit. The problems start when you look at dosing, added stimulants, and what else might be hiding in the formula.

Caffeine Is the Biggest Risk Factor

A single serving of pre-workout contains between 150 and 300 milligrams of caffeine. The FDA considers 400 milligrams per day safe for most adults, which means one scoop of a high-caffeine pre-workout already gets you to 75% of that ceiling. If you’re also drinking coffee, energy drinks, or tea throughout the day, you can easily blow past the limit without realizing it.

Too much caffeine causes jitteriness, anxiety, a racing heart, and digestive problems. In clinical testing, caffeine raised systolic blood pressure by 17% and mean arterial pressure by 11% before exercise even started. During a workout, when your cardiovascular system is already under load, that additional spike matters. For people with undiagnosed heart conditions or high blood pressure, this is where pre-workout moves from “not great” to genuinely risky.

Caffeine also reduces blood flow to the heart muscle during exercise. A study published in The American Journal of Medicine found measurably decreased myocardial blood flow in participants who took caffeine before physical activity. That doesn’t mean every person who takes pre-workout is damaging their heart, but it’s a meaningful physiological trade-off that most users never consider.

How Pre-Workout Affects Your Sleep

This is one of the most underappreciated downsides. Caffeine has a half-life of about five to six hours, meaning half the stimulant is still circulating in your body long after your workout ends. If you train in the afternoon or evening, a 300-milligram dose can easily interfere with falling asleep, staying asleep, or both. A systematic review of nine randomized controlled trials found that while results varied based on timing, dose, and habitual caffeine use, insomnia following caffeine ingestion is likely to occur.

This creates a frustrating cycle. Poor sleep impairs recovery, muscle repair, and hormone regulation. You wake up tired, feel like you need pre-workout to get through your next session, and the pattern repeats. Over time, the sleep disruption can undermine the very fitness goals you’re using the supplement to achieve. If you do use pre-workout, timing it at least six hours before bed significantly reduces this risk.

The Tingling Sensation Is Harmless

That prickling, itchy feeling on your skin after taking pre-workout is called paresthesia, and it comes from beta-alanine. It’s one of the most common reasons people worry about safety, but a systematic risk assessment and meta-analysis found that paresthesia was the only reported side effect of beta-alanine supplementation. The review concluded that beta-alanine, at the doses used in available research, does not adversely affect those consuming it. Participants taking beta-alanine dropped out of studies at the same rate as those on a placebo, suggesting the sensation is more annoying than harmful.

What About Your Liver and Kidneys

This is a common concern, especially for people who use pre-workout regularly over months or years. A cross-sectional study of 594 gym users in Brazil measured liver and kidney function markers in a subset of 242 participants. No one in the study showed strongly altered liver or kidney markers. Supplement users did show a higher chance of slight alterations in one liver enzyme (AST) and in urea, a kidney marker. But the researchers cautioned that these changes likely reflect normal physiological adaptations to training and supplementation rather than actual organ impairment, and they found no clinical relevance.

That said, “no clinical relevance in a study of gym users” is not the same as “safe for everyone in every scenario.” People with pre-existing kidney disease, liver conditions, or those taking medications that stress these organs should be more cautious. And the long-term data on daily, high-dose pre-workout use spanning years is still limited.

The Regulation Problem

Perhaps the most important thing to understand about pre-workout supplements is how they reach store shelves. The FDA regulates dietary supplements as food, not as drugs. That means manufacturers don’t have to prove their products are safe or effective before selling them. There’s no pre-market testing requirement, no mandatory clinical trials, and no guarantee that what’s on the label matches what’s in the tub.

This regulatory gap is why third-party certification matters. Programs like NSF Certified for Sport screen supplements for over 280 substances banned by major athletic organizations, along with undeclared ingredients like stimulants, steroids, narcotics, and masking agents. If a product doesn’t carry certification from NSF, Informed Sport, or a similar third-party program, you have no independent verification of what you’re actually consuming. Proprietary blends, which list ingredients without specifying individual amounts, make this even murkier.

Who Should Avoid It

Pre-workout is not equally risky for everyone. People who are sensitive to caffeine, have anxiety disorders, or experience heart palpitations should steer clear of stimulant-based formulas entirely. The same goes for anyone with high blood pressure, heart conditions, or kidney and liver issues. Pregnant and breastfeeding women should avoid these products. Teenagers, whose cardiovascular and neurological systems are still developing, face higher risk from high-dose stimulant intake.

For healthy adults who train regularly, a well-formulated pre-workout used at the recommended dose and taken early enough in the day is unlikely to cause serious harm. The real danger comes from doubling scoops, combining it with other caffeine sources, using products with undisclosed ingredients, or relying on it so heavily that you can’t train without it. If your pre-workout habit is costing you sleep, raising your resting heart rate, or giving you digestive issues, those are signs to cut back or switch to a stimulant-free option.
