At this point, the fertilized egg traveled down the fallopian tube, divided into a ball of cells, and arrived in the uterus. Successful implantation happens around 8 to 10 days after fertilization. By week 4, that process is complete: the blastocyst has shed its outer membrane in a step called hatching, and sticky proteins on its surface have latched onto the uterine lining. The tiny cluster of cells is now fully embedded and drawing nutrients from your body.

Even though the embryo is barely a speck, the internal activity at 4 weeks is enormous. A small group of cells called the inner cell mass is splitting into two layers: the epiblast and the hypoblast. These two layers are the starting blueprint for every tissue your baby will eventually have.

The upper portion of this tube will become the brain and skull. The lower portion will become the spinal cord and the bones of the back. This is why folic acid intake matters so much in early pregnancy: the neural tube is forming before many people even know they’re pregnant, and adequate folate reduces the risk of defects in this structure.

What You Can and Can’t See on an Ultrasound

If you’re hoping to see something on an ultrasound at 4 weeks, you’ll likely be disappointed. At best, a transvaginal ultrasound may show a small collection of fluid within the uterine lining, representing the very earliest stage of a gestational sac. There is no visible heartbeat, no recognizable shape, and no embryo to measure yet. Most providers wait until at least 6 to 7 weeks for a first ultrasound because there’s simply not enough to see before then.

What’s Happening in Your Body

The moment the blastocyst implants, it starts releasing a hormone called hCG. This is the hormone that pregnancy tests detect. At 4 weeks, hCG levels in your blood can range anywhere from 0 to 750 µ/L, a wide spread that reflects how variable early pregnancy timing can be. Some people will get a positive home pregnancy test right at 4 weeks, while others may need to wait a few more days for hCG to rise enough for detection. A faint line on a home test at this stage is completely normal and simply reflects low but rising hormone levels.

You may not feel noticeably different yet. Some people experience light spotting around implantation, mild cramping, or breast tenderness, but many feel nothing at all. The more recognizable symptoms like nausea and fatigue typically ramp up over the next two to four weeks as hCG levels climb.
