Yeast infections happen when a fungus called Candida, which normally lives in small amounts in the vagina, grows out of control. Up to 75% of women will experience at least one vaginal yeast infection in their lifetime, and 138 million women worldwide deal with recurrent infections each year. The overgrowth isn’t usually caused by catching something new. It’s almost always a shift in the vaginal environment that lets a fungus already living there take over.

How the Vagina Normally Keeps Yeast in Check

A healthy vagina hosts a community of bacteria, with Lactobacillus species running the show. These bacteria produce large amounts of lactic acid, keeping vaginal pH below 4.5. That acidic environment does two important things: it slows yeast growth directly, and it prevents Candida from shifting into its aggressive form. In its harmless state, Candida exists as round, single-celled yeast. Even women without diabetes can notice this effect. Periods of very high sugar intake or blood sugar spikes from illness or stress can tip the balance. For women who get frequent yeast infections, checking blood sugar levels is often part of the workup.

Your Immune System’s Role

Your body has a specialized immune response designed specifically to control Candida on mucosal surfaces like the vaginal lining. Immune cells recognize the fungus through pattern recognition receptors, triggering an inflammatory response that recruits neutrophils (a type of white blood cell) and activates a branch of immunity called the Th17 pathway. This pathway produces signaling molecules that are critical for keeping mucosal Candida under control.

When this system is compromised, yeast infections become more frequent and harder to resolve. HIV, chemotherapy, organ transplant medications, and long-term corticosteroid use all suppress immune function in ways that reduce the body’s ability to keep Candida in check. People with rare genetic immune deficiencies affecting the Th17 pathway suffer from chronic, persistent fungal infections of the mucous membranes, which underscores how important this specific immune response is.

Douching and Scented Products Disrupt the Balance

Douching washes away the normal bacteria that protect against infection and changes the vagina’s natural acidity. The U.S. Office on Women’s Health notes that douching removes beneficial bacteria and can cause an overgrowth of harmful organisms, including yeast. Despite being marketed as a hygiene practice, douching actually increases infection risk.

Scented tampons, pads, powders, and sprays can cause similar disruption. These products introduce chemicals that irritate vaginal tissue and alter the microbial balance. Even mild soaps can cause dryness and irritation in women with sensitive skin or existing infections. The vagina is self-cleaning, and its acidic environment is its own best defense. Moisture and Clothing Choices

Candida thrives in warm, dark, moist conditions. Sitting in a wet bathing suit after swimming, staying in sweaty workout clothes, or wearing damp underwear for extended periods creates exactly the environment yeast needs to proliferate. The groin area already tends to be warm, and trapping moisture against the skin accelerates fungal growth.

Tight, non-breathable fabrics compound the problem by preventing airflow and holding heat and moisture close to the body. Cotton underwear and loose-fitting clothing allow better ventilation. Moisture-wicking athletic wear helps sweat evaporate faster during exercise. The simplest preventive step is changing out of wet or sweaty clothes promptly rather than letting them sit against the skin for hours.

What Happens When Candida Overgrows

Once conditions favor its growth, Candida shifts from its harmless yeast form into an invasive filamentous form. These filaments produce specialized proteins that help them stick to and penetrate the vaginal lining. As the filaments push into cells, they release a toxin called candidalysin that directly damages tissue and triggers the inflammatory response responsible for the classic symptoms: itching, burning, swelling, and thick white discharge.

Instead, it’s often a combination of factors: a genetic variation in immune response, subtle hormonal patterns, ongoing medication use, or a vaginal microbiome that never fully recovers its protective Lactobacillus population after the first infection.

Recurrent infections affect quality of life significantly, causing chronic discomfort and frustration. Women in this situation benefit from identifying and addressing their specific contributing factors rather than simply treating each episode as it arises. Keeping blood sugar stable, avoiding unnecessary antibiotics, choosing breathable clothing, and skipping products that disrupt vaginal chemistry can all reduce the frequency of flare-ups over time.
