Deaf people talk through a combination of speech training, technology, visual cues, and alternative communication methods. The approach varies widely depending on when hearing loss occurred, its severity, and personal preference. Someone who lost hearing at age 30 may speak fluently because they learned speech before going deaf, while someone born deaf might develop spoken language through years of therapy, use sign language as their primary mode of communication, or combine multiple methods.

Why Timing of Hearing Loss Matters

The single biggest factor in whether a deaf person uses spoken language is when they lost their hearing. People who become deaf after learning to speak (called post-lingual deafness) already have the neural wiring for language in place. Their brains built those pathways during childhood, and speech often persists even without the ability to hear their own voice. Over time, though, speech can drift in pitch and clarity without auditory feedback, which is why many adults who lose hearing later in life use hearing aids or cochlear implants to maintain their spoken communication skills.

For people born deaf or who lose hearing in infancy, the situation is fundamentally different. They never heard the sounds they’re being asked to produce, so learning to speak requires intensive training using senses other than hearing. Touch, sight, and vibration become the tools for building speech from scratch.

How Speech Training Works

Speech therapy for deaf children typically follows a structured path. Early stages focus on sound awareness: getting the child curious about environmental sounds and helping them notice differences in rhythm, pitch, and volume. From there, training moves to identifying simple words from a small set of options, then recognizing words and sentences without any visual help.

These programs use playful, reward-based learning to keep young children engaged. A therapist might use toys, games, or tangible rewards to reinforce correct responses. One well-known approach, Auditory-Verbal Therapy, deliberately restricts visual cues like lip reading during sessions. The goal is to push children to rely entirely on whatever hearing they have (usually amplified through hearing aids or cochlear implants) so they develop the strongest possible auditory skills for everyday life.

Tactile feedback is another key tool. Children learn to feel the vibrations of their own throat, chest, or face while producing sounds. A therapist might place the child’s hand on their neck to feel the difference between a voiced sound like “b” and an unvoiced one like “p.” This hands-on approach dates back over a century, and it remains a practical way to teach sound production when hearing alone isn’t enough.

Cochlear Implants and Hearing Aids

Cochlear implants have transformed speech outcomes for many deaf individuals. Unlike hearing aids, which amplify sound, cochlear implants bypass damaged parts of the ear and directly stimulate the auditory nerve. For children, timing is critical. Research consistently shows that children implanted before age two develop speech and language at significantly higher rates than those implanted later. One study found that 43 percent of children who received implants at age two achieved speech and language abilities comparable to hearing children of the same age. Children implanted before nine months showed the most natural spoken language development.

For adults who lose hearing later in life, cochlear implants also work well, but again, timing matters. About 94 percent of post-lingually deaf adults with implants achieved sentence recognition scores above 80 percent. However, if someone went more than 10 years without hearing before getting an implant, outcomes dropped significantly. The brain gradually repurposes unused auditory pathways for other functions, and after a decade, some of those changes become difficult to reverse.

Modern hearing aids, meanwhile, have become remarkably sophisticated. Current models use directional microphones that suppress background noise while focusing on the speaker’s voice. Some use binaural beamforming, wirelessly combining signals from both ears to create a highly focused listening experience. Newer models even run onboard artificial intelligence that analyzes the acoustic environment in real time and adjusts settings for gain, noise management, and microphone direction to optimize speech clarity.

Lip Reading and Its Limits

Many deaf people rely on lip reading (also called speechreading) to follow conversations, and it plays a major role in how they participate in spoken exchanges. But lip reading is far harder than most hearing people realize. Many speech sounds look identical on the lips. The consonants “p,” “b,” and “m,” for example, all involve the same lip movement, making them impossible to tell apart visually. Context helps, but longer sentences can overwhelm working memory, while shorter ones may not provide enough context to fill in the gaps.

This is where a system called Cued Speech becomes useful. Developed specifically to solve the ambiguity of lip reading, Cued Speech pairs mouth movements with hand signals near the face. Each handshape represents a group of consonants, and each hand position near the mouth represents a group of vowels. Sounds that look the same on the lips get different handshapes. So “p,” “b,” and “m” are each assigned a distinct hand signal, making them instantly distinguishable. When a hearing person uses Cued Speech while talking, a deaf person can follow spoken language with near-complete accuracy. It’s not sign language. It’s a visual code for spoken sounds.

Speech-to-Text Technology

Smartphones have opened up a practical workaround for face-to-face conversations. Apps like Google Live Transcribe, Ava, Otter.ai, and Microsoft Translator convert spoken words into text on screen in real time, allowing a deaf person to read what someone is saying as they say it. Apple’s Live Captions feature, available on iPhone 11 and later, works similarly without needing a separate app.

Accuracy varies depending on background noise, speaker clarity, and how close the microphone is to the person talking. These tools work best in quieter, one-on-one settings. In noisy restaurants or group conversations, accuracy drops. Still, for many deaf people, these apps have become an everyday tool for navigating spoken interactions at work, in stores, and in social settings where sign language interpreters aren’t available.

The Role of Personal Choice

Not every deaf person wants to speak, and the question of whether they should has a complicated history. By 1900, most schools for deaf children in the United States required speech training. Many placed students in “oral” tracks where signing was prohibited. In some schools, children were forced to sit on their hands or wear mittens as punishment for signing. This oralist tradition caused significant harm, and its legacy still shapes how many Deaf people (capital-D Deaf, referring to the cultural community) view spoken language.

Today, many Deaf people use sign language as their primary language and see no reason to speak. They communicate fluently and fully through ASL or other sign languages, and they view deafness not as a deficit but as a cultural identity. Others prefer spoken language, especially those who grew up with cochlear implants or in hearing families. Many use a mix: signing with Deaf friends, speaking at work, texting in group chats, and pulling up a transcription app at the coffee shop. The reality is that most deaf people navigate between multiple communication methods depending on the situation, choosing whatever works best in the moment.
