How Common Is Carbon Monoxide Poisoning in the U.S.

Carbon monoxide poisoning is more common than most people realize. In the United States alone, more than 100,000 people visit an emergency department each year for CO exposure not linked to fires, over 14,000 are hospitalized, and more than 400 die. Globally, unintentional CO poisoning killed an estimated 28,900 people in 2021. While those numbers have been declining over the past two decades, CO remains one of the leading causes of poisoning death in the developed world.

U.S. Numbers in Context

The 400-plus annual deaths in the U.S. only count unintentional cases unrelated to house fires. When you include fire-related CO deaths and intentional exposures, the total is significantly higher. But even focusing on accidental, non-fire cases, the scale is striking: roughly one person dies every day from CO they didn’t know was building up around them.

The more than 100,000 annual emergency visits represent the cases severe enough to send someone to a hospital. Many milder exposures likely go undiagnosed because CO poisoning symptoms, including headache, dizziness, nausea, and confusion, overlap with the flu, food poisoning, and other common illnesses. Some researchers believe the true number of people affected each year is considerably higher than what shows up in hospital data.

Who Is Most at Risk

Men are roughly three times more likely to die from CO poisoning than women, with an average annual death rate of 0.22 per 100,000 compared to 0.07 for women. This gap likely reflects higher occupational exposure and greater use of generators and fuel-burning tools. Globally, nearly 70% of CO poisoning deaths occur in males.

Age matters, too. Death rates are highest among adults 65 and older, at 0.42 per 100,000 for men and 0.18 for women in that age group. People aged 85 and older face the highest mortality rate of any age bracket at nearly 2 deaths per 100,000. Older adults are more vulnerable both because their bodies handle CO less efficiently and because they may not recognize symptoms or evacuate quickly. The lowest rates are among people under 25.

Alcohol also plays a role. A global analysis found that high alcohol use contributed to about 3.5% of CO poisoning deaths, likely because intoxication makes it harder to notice symptoms or respond to alarms. Occupational injuries accounted for roughly 14% of deaths worldwide.

The Biggest Sources

Portable generators are the single consumer product most commonly linked to fatal CO poisoning. In 2021, the U.S. Consumer Product Safety Commission recorded an estimated 204 non-fire CO deaths from consumer products. Of those, 54% involved engine-driven tools, and generators alone accounted for 102 of those deaths. People typically run generators indoors or in attached garages during power outages, not realizing how quickly CO can accumulate in an enclosed space.

Heating systems were the second-largest category, responsible for about 31% of consumer-product CO deaths. This includes furnaces, boilers, space heaters, and other fuel-burning appliances that malfunction or vent improperly. Charcoal and charcoal grills caused about 4% of deaths, usually when people bring grills inside for warmth or cooking.

Winter and Storms Drive Seasonal Spikes

CO poisoning follows a clear seasonal pattern. Nearly half of all weather-related CO deaths occur between November and February, with December through February alone accounting for about 35%. The connection is straightforward: winter storms and extreme cold knock out power, and people turn to backup sources like portable generators, kerosene heaters, and gas stoves to stay warm. These devices produce CO that builds to dangerous levels in poorly ventilated homes.

Major weather events create acute spikes. After hurricanes, ice storms, and severe cold snaps, emergency departments regularly see surges in CO cases. The pattern repeats reliably: the disaster itself may pass in hours, but the days of power outage that follow are when CO poisoning clusters appear.

Global Trends

Worldwide, the CO poisoning mortality rate was 0.37 per 100,000 people in 2021. That represents a 53.5% drop in the age-standardized rate since 2000, driven largely by better building codes, improved heating technology, and wider use of CO detectors. But the decline hasn’t been even. Eastern Europe has the highest regional mortality rate at 2.12 per 100,000, more than five times the global average, likely reflecting older housing stock, coal-based heating, and less widespread detector use.

The 50-to-54 age group had the largest absolute number of deaths globally, with an estimated 2,210 in 2021. This likely reflects a combination of occupational exposure and the demographic realities of who operates generators, maintains heating systems, and works around combustion engines.

What Happens to Survivors

Surviving CO poisoning doesn’t always mean a full recovery. Between 15% and 40% of survivors experience long-term neurological problems. These can appear days or even weeks after the initial poisoning and include memory difficulties, trouble concentrating, personality changes, and movement disorders. One study found a 12% incidence of delayed neurological symptoms and a 20% incidence of heart muscle injury among hospitalized CO patients.

The severity depends on how much CO was in the blood and how long the exposure lasted. Doctors measure this through a blood test that shows the percentage of hemoglobin bound to CO instead of oxygen. In nonsmokers, anything above 2% suggests CO exposure. Levels above 25-30% are considered severe and may require treatment in a pressurized oxygen chamber. But even people with lower levels can develop lasting complications if exposure was prolonged.

CO Detectors and Prevention Laws

Most U.S. states now require CO alarms in residential buildings, though the specifics vary. New York, for example, mandates CO detection in both new and existing residential and commercial buildings, with alarms tested periodically according to manufacturer guidelines. These laws have contributed to the long-term decline in CO deaths, but compliance is uneven. Detectors expire (typically after 5 to 7 years), batteries die, and rental properties don’t always meet code.

A working CO detector is the single most effective protection against poisoning. If you use any fuel-burning appliance, including a gas furnace, water heater, fireplace, or attached garage, a detector on every level of your home is essential. During power outages, generators should only run outdoors, at least 20 feet from windows, doors, and vents. The same applies to charcoal grills and camp stoves, which should never be used indoors for any reason.
