Hyperthyroidism is the most common hormonal disorder in cats, particularly in middle-aged and older animals. It affects roughly 10% of cats over the age of 10, making it something most owners of senior cats should have on their radar. The condition occurs when the thyroid glands produce too much thyroid hormone, speeding up the body’s metabolism and gradually affecting the heart, kidneys, and other organs.
Which Cats Are Most at Risk
Hyperthyroidism overwhelmingly affects older cats. It rarely appears before age 8, and the typical cat is diagnosed around 12 to 13 years old. Because cats are living longer than previous generations thanks to better nutrition and veterinary care, the disease has become far more prevalent over the past few decades. It was first formally described in cats in the late 1970s and has risen steadily since.
Certain factors influence a cat’s risk. A study examining breed, sex, and coat type found that long-haired cats, female cats, and non-purebred (mixed breed) cats had higher rates of hyperthyroidism. Several purebred breeds, including Siamese, Burmese, Tonkinese, Persian, British Shorthair, and Abyssinian, showed reduced rates. If your cat is a domestic shorthair or longhair mix over the age of 10, the odds of eventually developing this condition are meaningful enough to justify routine screening.
What Causes It
In more than 98% of cases, feline hyperthyroidism is caused by a benign overgrowth of thyroid tissue, either a non-cancerous tumor (adenoma) or general enlargement of the gland. Thyroid cancer accounts for fewer than 2% of cases. This is good news: the benign nature of the underlying problem means most cats respond well to treatment.
Researchers have investigated whether household chemicals, particularly flame retardants found in furniture, carpets, and electronics, might contribute to the rising rates. Cats groom themselves constantly and ingest dust particles that settle on their fur, so the theory made sense. Studies have confirmed that cats carry flame retardant levels roughly 10 times higher than the general human population. However, direct comparisons between hyperthyroid and healthy cats have not found a significant difference in chemical levels between the two groups, so a clear environmental cause has not been established.
Signs to Watch For
The classic combination is a cat that loses weight despite eating more than usual. This happens because excess thyroid hormone pushes the metabolism into overdrive, burning calories faster than the cat can take them in. Increased thirst and urination are also common early signs.
Other symptoms include vomiting, diarrhea, restlessness or unusual hyperactivity (especially noticeable in an older cat that was previously calm), and a dull or unkempt coat. Some cats become more vocal or irritable. Over time, the elevated thyroid hormones force the heart to beat faster and contract harder, which can thicken the heart muscle. Left untreated, this leads to a form of heart disease that is often reversible once thyroid levels return to normal.
The tricky part is that many of these signs develop gradually. Owners sometimes assume their cat is simply aging, losing a bit of weight or drinking more water than before. Routine blood work during annual or semi-annual veterinary visits is the most reliable way to catch it early, since a simple blood test measuring thyroid hormone levels can confirm the diagnosis.
The Kidney Connection
One complication that catches many owners off guard is the relationship between hyperthyroidism and kidney disease. Excess thyroid hormone increases blood flow through the kidneys, which can temporarily make them appear to function better than they actually are. This masks underlying chronic kidney disease that may already be developing. Once hyperthyroidism is treated and thyroid levels normalize, kidney function can appear to decline because the “boost” is gone. Your vet will typically monitor kidney values closely after starting treatment to get an accurate picture of how the kidneys are truly performing.
Treatment Options and Success Rates
There are four main approaches to managing feline hyperthyroidism, and the right choice depends on the cat’s overall health, age, and whether kidney disease is also present.
- Radioactive iodine therapy is considered the gold standard. A single injection destroys the overactive thyroid tissue while leaving healthy tissue intact. In a study of 524 cats treated this way, 94.2% had a good response, and by six months after treatment, only 1.5% still had elevated thyroid levels. It requires a short hospital stay (typically a few days) because the cat is mildly radioactive afterward, but it is a one-time treatment with no daily medication needed.
- Daily medication controls thyroid hormone production but does not cure the disease. It requires giving pills or applying a topical gel to the ear once or twice daily for the rest of the cat’s life. It works well for cats who are not candidates for other treatments or whose owners want to monitor kidney function before committing to a permanent option.
- Surgery to remove the affected thyroid gland is effective but carries anesthesia risks in older cats and is less commonly chosen now that radioactive iodine is widely available.
- Prescription diet severely restricts iodine intake, which limits the thyroid’s ability to produce hormones. It can work for some cats but requires that the cat eat absolutely nothing else, which is difficult in multi-cat households.
Why Early Detection Matters
Cats diagnosed and treated early generally do very well. The heart changes caused by excess thyroid hormone often reverse within weeks to months of bringing levels back to normal. Weight stabilizes, appetite normalizes, and many owners report their cat seems years younger. Without treatment, hyperthyroidism progressively damages the heart, can worsen hidden kidney disease, and significantly shortens a cat’s life. For any cat over 8 or 9 years old, annual thyroid screening as part of routine blood work is a straightforward way to catch it before serious damage occurs.
