Bladder stone surgery in dogs is considered a safe, routine procedure with a low complication rate of around 4%. Most dogs recover fully within two weeks. That said, no surgery is risk-free, and the specifics depend on your dog’s age, overall health, the type and size of the stones, and whether less invasive options were considered first.
What the Surgery Involves
The standard surgery for bladder stones is called a cystotomy. Your vet makes an incision through the abdomen and into the bladder wall, removes the stones, flushes the bladder, and closes everything with sutures. It’s one of the most commonly performed surgeries in veterinary practice, and most general practice vets have extensive experience with it.
The procedure itself is relatively quick. Your dog will be under general anesthesia, which carries its own small set of risks, particularly for older dogs or those with heart, liver, or kidney conditions. For a healthy dog, the anesthesia risk is minimal.
Complication Rates Are Low but Real
In a study of 128 dogs that underwent cystotomy, complications developed in about 4% of cases. The most common issues include infection at the incision site, urine leaking into the abdomen, and prolonged bleeding in the urine. Incision site infections that progress to serious abdominal infection are rare, occurring in roughly 7% of dogs who had any postoperative complication at all.
One risk that often surprises owners: stones get left behind more often than you’d expect. In the same study, 20% of dogs whose surgical outcomes could be evaluated had incomplete stone removal. Dogs with stones lodged in both the bladder and the urethra (the tube that carries urine out of the body) were more likely to have stones missed. This is why post-surgical imaging, such as X-rays or ultrasound, matters. Despite this, only about 15% of dogs in the study received appropriate follow-up imaging.
Leftover stone fragments can cause continued irritation, recurring infections, and the need for a second procedure. If your vet doesn’t mention post-operative imaging, it’s worth asking about.
Sutures Can Trigger New Stones
An underappreciated risk of traditional surgery is that the sutures used to close the bladder can themselves become a starting point for new stones. Minerals in the urine can crystallize around suture material, and this may be a primary factor in roughly 9% of recurrent bladder stones. This is one reason veterinary specialists increasingly recommend non-surgical alternatives when they’re feasible.
Recovery: What to Expect
Most dogs go home within a day or two of surgery. The first one to two weeks are the critical recovery window. It’s normal for your dog’s urine to look slightly bloody during this period. You’ll need to check the incision daily for redness, swelling, or discharge, and keep an eye on your dog’s appetite, water intake, and urination patterns. An e-collar (cone) is typically necessary to prevent your dog from licking or chewing at the incision.
Activity restriction is important. Running, jumping, and rough play can stress the healing bladder wall and abdominal incision. Most dogs are back to normal within 10 to 14 days, with a follow-up visit to remove skin sutures and confirm healing.
Surgery Isn’t Always the Best Option
Veterinary consensus guidelines now recommend that surgery should not be the automatic first choice for every bladder stone. The right approach depends heavily on the type of stone your dog has.
Struvite stones, one of the two most common types in dogs, can often be dissolved with a prescription diet and, if infection is present, antibiotics. In a study of 50 dogs treated with dietary dissolution, 58% achieved full stone dissolution in a median of 35 days. Some dogs’ stones dissolved in as little as 13 days. If the stones haven’t started shrinking after about a month of dietary therapy, they’re likely not struvite, and your vet will need to consider other approaches.
Calcium oxalate stones, the other common type, cannot be dissolved with diet. These do require physical removal, but that doesn’t necessarily mean open surgery.
Minimally Invasive Procedures
For stones small enough to pass through the urethra, several non-surgical options exist. These include flushing techniques that push stones out, basket retrieval through a scope, and laser lithotripsy, which breaks stones into tiny fragments using laser energy delivered through a small scope inserted through the urethra.
Laser lithotripsy has been directly compared to traditional cystotomy in dogs. The two approaches showed no significant difference in stone removal rates, anesthesia time, cost, or complication rates. The key advantage of lithotripsy was shorter hospitalization, meaning dogs went home sooner. The tradeoff is that the procedure itself takes longer than a cystotomy.
These incision-free options eliminate the risk of suture-related stone recurrence and generally mean faster recovery. They’re not available at every veterinary clinic, though. They typically require a veterinary specialty hospital with the right equipment and training.
Which Dogs Face Higher Risk
The 4% complication rate reflects the general population of dogs undergoing this surgery. Your dog’s individual risk may be higher if they are very old, have kidney disease or other organ dysfunction, are obese, or have a urinary tract infection that hasn’t been controlled before surgery. Dogs with stones in multiple locations (bladder plus urethra) also face a higher chance of incomplete removal and the complications that follow.
Breeds prone to bladder stones, such as Dalmatians, Miniature Schnauzers, Shih Tzus, and Bichon Frises, often deal with recurrence regardless of how well the initial surgery goes. For these dogs, long-term dietary management and regular monitoring with urine tests and imaging are just as important as the initial treatment.
Questions Worth Asking Your Vet
Before agreeing to surgery, a few questions can help you make a more informed decision:
- What type of stone is suspected? If struvite is likely, dietary dissolution may be worth trying first.
- Are minimally invasive options available? If not at your clinic, a referral to a specialty hospital may be worthwhile.
- Will post-operative imaging be done? X-rays or ultrasound after surgery can catch any fragments left behind before they cause problems.
- What’s the prevention plan? Removing the stones solves the immediate problem, but without dietary or medical prevention, many dogs form new stones within months to years.
