Blood in your stool can look bright red, dark red, or black and tarry, depending on where the bleeding originates in your digestive tract. Sometimes it’s obvious on the toilet paper or in the bowl. Other times, the blood is invisible to the naked eye and only shows up on a screening test. Knowing what to look for, and what can mimic blood, helps you figure out your next step.
What Blood in Stool Actually Looks Like
The color of blood in your stool tells you a lot about where it’s coming from. Bright red blood typically originates lower in the digestive tract, in the colon, rectum, or anus. You might see it on the surface of your stool, on toilet paper after wiping, or dripping into the toilet bowl. This type of bleeding is often noticeable and hard to miss.
Black, tarry stools point to bleeding higher up, in the stomach, esophagus, or the first section of the small intestine. The blood turns dark because it gets broken down by digestive enzymes as it travels through your system. These stools often have a sticky consistency and a distinctly foul smell that’s different from normal bowel movements. If your stool looks like coffee grounds or has that tar-like quality, that’s a sign of digested blood.
Dark red or maroon-colored blood mixed into the stool can signal bleeding somewhere in the middle of the tract, like the small intestine or the right side of the colon. This is less common but worth noting because it doesn’t fall neatly into the “bright red” or “black” categories.
Hidden Blood You Can’t See
Not all gastrointestinal bleeding is visible. Slow, low-volume bleeding can go unnoticed for weeks or months. In these cases, the first clue is often how you feel rather than what you see. Chronic hidden blood loss leads to iron-deficiency anemia, which shows up as persistent fatigue, shortness of breath during normal activity, pale skin, and feeling lightheaded when you stand up. If you’ve developed these symptoms without an obvious explanation, hidden blood loss is one possibility worth investigating.
Stool-based screening tests can detect this invisible blood. The fecal immunochemical test (FIT) is the most commonly used option and is more accurate than older guaiac-based tests. In head-to-head comparisons, FIT has a specificity around 92% for detecting advanced growths, meaning it produces fewer false alarms. The older guaiac test sits closer to 76% specificity. The U.S. Preventive Services Task Force recommends annual FIT screening starting at age 45 for colorectal cancer detection, even if you have no symptoms.
Foods and Medications That Mimic Blood
Before you panic, consider what you’ve eaten or taken recently. Beets and foods with red coloring can make stools appear reddish, closely mimicking bright red blood. On the dark side, black licorice, blueberries, iron supplements, activated charcoal, and bismuth-containing medications like Pepto-Bismol can all turn your stool black. These changes are harmless and typically resolve within a day or two after you stop consuming the culprit.
A simple way to test this: if you suspect a food is responsible, stop eating it for two to three days. If your stool color returns to normal, food was likely the cause. If the unusual color persists, something else is going on.
Common Causes of Visible Blood
Hemorrhoids are the most frequent reason for bright red blood on toilet paper or in the bowl. These are swollen blood vessels around the anus that can cause itching, soreness, swelling, and bleeding during bowel movements. They’re extremely common and usually not dangerous, though they can be uncomfortable and alarming.
Anal fissures, small tears in the lining of the anus, cause a sharp, stinging pain during bowel movements along with bright red bleeding. They’re often triggered by passing hard or large stools and typically heal on their own within a few weeks. Fissures are also relatively common in people with Crohn’s disease, sometimes appearing before other symptoms of inflammatory bowel disease develop.
Colon polyps and diverticular disease are lower GI sources of bleeding that tend to be painless. Polyps are small growths on the colon lining, and while most are benign, some can become cancerous over time. Diverticulosis, where small pouches form in the colon wall, can cause sudden, painless bleeding that sometimes stops on its own.
Higher up in the digestive tract, peptic ulcers in the stomach or upper small intestine are a leading cause of black, tarry stools. Ulcer-related bleeding can range from slow and chronic to sudden and heavy.
What the Color and Pain Tell You
Matching what you see with what you feel narrows things down considerably:
- Bright red blood with sharp pain during bowel movements: most likely an anal fissure.
- Bright red blood with itching or swelling but less sharp pain: hemorrhoids are the usual suspect.
- Bright red or dark red blood with no pain: could indicate polyps, diverticular bleeding, or other colon issues that deserve investigation.
- Black, tarry stools with upper abdominal pain or nausea: suggests a stomach or duodenal ulcer.
- Bloody stool with diarrhea, cramping, and urgency over weeks: raises the possibility of inflammatory bowel disease like ulcerative colitis or Crohn’s disease.
Signs That Need Urgent Attention
Most causes of blood in stool are manageable and not immediately dangerous. But certain combinations of symptoms signal significant blood loss that needs prompt medical evaluation. Feeling dizzy or lightheaded when you stand, a racing heartbeat, confusion, or passing large amounts of blood all suggest your body is losing blood faster than it can compensate. Vomit that looks like coffee grounds or contains bright red blood alongside bloody stools points to active upper GI bleeding.
Black, tarry stools that persist for more than a day or two (and aren’t explained by iron pills or bismuth medications) should also be evaluated promptly. Upper GI bleeds can escalate quickly, and the tarry appearance means a meaningful amount of blood has passed through your system.
How Doctors Confirm the Source
If you report blood in your stool, the evaluation usually starts with basic questions about what you’ve seen, how long it’s been happening, and whether you have pain or other symptoms. A blood count checks for anemia, which reveals both the severity and duration of any bleeding.
For visible lower GI bleeding, a colonoscopy lets doctors examine the entire colon and rectum directly, spot polyps, identify inflammation, and take tissue samples if needed. For suspected upper GI bleeding (black, tarry stools or vomiting blood), an upper endoscopy examines the esophagus, stomach, and upper small intestine. In cases where the bleeding source isn’t clear from either approach, imaging like CT scans with contrast can pinpoint active bleeding.
If you’re not having an acute episode but want to check for hidden blood, a take-home FIT kit is straightforward. You collect a small stool sample, send it to a lab, and get results within a few days. A positive result doesn’t automatically mean cancer or a serious condition. It means further evaluation, usually a colonoscopy, is the logical next step to find the source.
