How Do I Know If I Have BV? Signs and Testing

The most telling sign of bacterial vaginosis is a thin, grayish or white vaginal discharge with a strong fishy smell, especially noticeable after sex. But roughly half of people with BV have no symptoms at all, which makes it tricky to self-diagnose with certainty. Understanding what to look for, and what BV can be confused with, helps you figure out your next step.

The Main Symptoms of BV

BV changes the balance of bacteria in your vagina, and those changes produce a few recognizable signs. The discharge is typically thin, sometimes foamy, and grayish or off-white. It’s different from your normal discharge in both appearance and volume.

The odor is the most distinctive clue. People often describe it as fishy, and it tends to get stronger after unprotected sex. That’s because semen is alkaline, which releases the same odor-causing compounds that BV bacteria produce. Some people notice the smell during their period for the same reason: menstrual blood raises vaginal pH temporarily.

Mild itching or irritation around the vaginal opening is possible but not the hallmark of BV. If intense itching or burning is your primary symptom, something else may be going on.

BV vs. Yeast Infection: Key Differences

These two conditions are easy to confuse because both involve unusual discharge, but they look and feel quite different in practice.

Yeast infections produce thick, white, clumpy discharge that’s often compared to cottage cheese. It typically has no odor. The defining feature is intense itching and irritation of the vulva and vaginal opening, sometimes with visible redness or swelling. You might also notice a white coating in and around the vagina.

BV discharge, by contrast, is thin, grayish, and foamy, with that characteristic fishy smell. Itching is less prominent. If you’re dealing with a strong odor but minimal itching, BV is more likely. If you have heavy itching with thick, odorless discharge, a yeast infection is the better guess. That said, overlap exists, and both can show up at the same time.

Why You Can Have BV Without Knowing

A significant portion of people with BV are completely asymptomatic. The bacterial shift is happening, but it hasn’t produced noticeable discharge or odor. This matters because untreated BV, even without symptoms, can increase your susceptibility to sexually transmitted infections like chlamydia, gonorrhea, and HIV. Those STIs can in turn lead to pelvic inflammatory disease, which may affect fertility.

If you’re pregnant, the stakes are higher. BV during pregnancy raises the risk of preterm delivery and low birth weight (under 5.5 pounds). BV sometimes resolves on its own, but treatment reduces these risks significantly.

What Happens at a Doctor’s Visit

There’s no blood test for BV. Diagnosis happens through a vaginal swab and a few straightforward checks. Your provider is looking at a combination of signs to confirm what’s going on.

The most commonly used clinical method looks at four things: the appearance of your discharge, the pH of your vaginal fluid, how your discharge responds to a chemical test, and what the cells look like under a microscope. A healthy vagina is acidic, with a pH below 4.5. In BV, the pH rises above 4.5 because the protective acid-producing bacteria have been overtaken by other types. Your provider may also add a drop of potassium hydroxide to a sample of your discharge. If it releases a fishy smell, that’s a positive result, confirming the presence of odor-causing compounds produced by BV bacteria.

Under the microscope, your provider is checking whether the normal rod-shaped bacteria have been replaced by a mix of other bacterial types. A lab scoring system grades this on a 0 to 10 scale: 0 to 3 is normal, 4 to 6 is intermediate, and 7 to 10 confirms BV. Some clinics also use rapid DNA-based tests that detect high concentrations of the primary BV-associated bacterium in about 30 minutes.

At-Home pH Tests: Helpful but Limited

You can buy over-the-counter vaginal pH test strips for around $30 or less. These are simple swabs that change color based on your vaginal acidity. If your pH is above 4.5, it suggests something is off.

The catch is that an elevated pH doesn’t tell you why it’s elevated. BV, trichomoniasis, and certain other infections all raise vaginal pH. So does menstruation, recent sex, and perimenopause. A pH strip can’t distinguish between these causes, which means it can point you in the right direction but can’t replace a proper diagnosis. Think of it as a useful first step: if the result is abnormal, it’s worth following up with a provider who can identify the specific cause and recommend the right treatment.

These tests are also unreliable if you’re in perimenopause or menopause, since vaginal pH naturally rises during that transition regardless of infection status.

Signs That Warrant a Closer Look

A few scenarios should prompt you to get tested rather than waiting it out. New or persistent fishy odor is the most obvious one, especially if it’s been present for more than a few days. A change in your discharge color or texture that doesn’t match your normal cycle patterns is another. If you’re pregnant or planning to become pregnant and notice any of these changes, testing is particularly important given the link between BV and preterm birth.

Recurrent BV is also common. If you’ve been treated before and recognize the same symptoms returning, that pattern itself is useful information for your provider in choosing a treatment approach that may be more durable.
