Low estrogen shows up in your body long before a blood test confirms it. The most recognizable signs are hot flashes, vaginal dryness, irregular or missing periods, and sleep problems, but the full picture can include everything from dry skin to brain fog to joint pain. If you’re noticing a cluster of these symptoms, especially during perimenopause or after changes to your menstrual cycle, low estrogen is a likely explanation.
The Physical Symptoms That Show Up First
Hot flashes and night sweats are the hallmark signs. As estrogen levels drop, your body’s internal thermostat becomes less stable, triggering sudden waves of heat that can last anywhere from a few seconds to several minutes. Night sweats are the same mechanism, just happening while you sleep. Some people experience them a few times a week, others multiple times a night.
Skin changes are another early signal. Estrogen helps maintain moisture and elasticity in your skin, so when levels fall, your skin can become noticeably drier and thinner. You might also notice your hair becoming more brittle or your joints feeling stiff and achy, particularly in the morning. These symptoms often get attributed to aging or stress, but they can be directly tied to declining estrogen.
Vaginal and Urinary Changes
One of the most specific indicators of low estrogen is a group of symptoms affecting the vaginal and urinary tissues. Lower estrogen causes these tissues to become thinner, drier, and less elastic. The result can include vaginal dryness, burning, or itching; pain during sex from reduced lubrication; and sometimes light bleeding afterward. Some people notice an unusual thin or watery discharge.
Urinary symptoms often accompany these changes. You might feel the need to urinate more frequently or urgently, experience a burning sensation during urination, or find yourself dealing with recurrent urinary tract infections. These symptoms tend to be progressive, meaning they worsen over time without treatment, unlike hot flashes which often improve on their own after several years. Roughly half of postmenopausal women experience some degree of these genitourinary changes.
Brain Fog, Mood Changes, and Sleep
Estrogen receptors exist in virtually every organ in your body, and the brain is no exception. When estrogen drops, many people experience what’s commonly called brain fog: reaching for a word that won’t come, forgetting why you walked into a room, or having trouble retaining new information. The Study of Women’s Health Across the Nation, a large long-term trial, found that perimenopausal women temporarily had more difficulty learning new information compared to pre- and postmenopausal women. The good news is that this cognitive cloudiness appears to be temporary for most people.
Mood symptoms follow a pattern tied to where you are in the hormonal transition. Anxiety and hot flashes tend to be most common in the first five years after menopause, while depression and low libido become more prominent further out. Irritability, tearfulness, and a general sense that your emotional baseline has shifted are all common.
Sleep disruption ties many of these symptoms together. Beyond night sweats waking you up, the decline in progesterone that typically accompanies falling estrogen has its own sleep effects. Progesterone has natural sedative properties, so losing it contributes to lighter sleep, trouble falling asleep, and more frequent nighttime awakenings. Poor sleep then amplifies the brain fog, mood changes, and fatigue, creating a cycle that can be hard to untangle.
Bone Loss: The Silent Symptom
Unlike the symptoms above, bone loss from low estrogen doesn’t produce obvious warning signs until it’s advanced. Estrogen plays a critical role in maintaining bone density, and the drop at menopause is one of the strongest risk factors for osteoporosis. Women who go through early menopause or have their ovaries removed at a younger age face an even higher risk of accelerated bone loss. You won’t feel your bones thinning, which is why a bone density scan becomes important if you’ve had low estrogen for an extended period, particularly if it started before the typical age of menopause.
Low Estrogen Before Menopause
Menopause isn’t the only reason estrogen drops. If you’re younger and experiencing these symptoms, several other causes are worth considering. Extreme exercise or very low body weight can suppress the hormonal signals from your brain that trigger estrogen production, a condition sometimes called hypothalamic amenorrhea. Eating disorders, chronic stress, and certain autoimmune conditions can do the same thing. Some genetic conditions, like Turner syndrome, affect ovarian function from birth. Surgical removal of one or both ovaries, certain chemotherapy drugs, and radiation therapy to the pelvic area can also cause estrogen to fall sharply, sometimes overnight.
If your period has become irregular or disappeared entirely and you’re not in the typical age range for perimenopause (mid-40s to early 50s), that’s a strong signal to get your hormone levels checked rather than assuming it will resolve on its own.
How Estrogen Levels Are Tested
A blood test measuring estradiol (the primary form of estrogen) is the standard way to confirm low estrogen. Normal estradiol ranges are roughly 10 to 300 pg/mL for premenopausal women and below 10 pg/mL for postmenopausal women. That premenopausal range is wide because estrogen fluctuates dramatically throughout your menstrual cycle. For the most accurate reading, testing is typically done on days 2 to 3 of your cycle (shortly after your period starts) or on days 21 to 23.
Your doctor will often order follicle-stimulating hormone (FSH) alongside estradiol. When estrogen is low, FSH rises as your brain tries to signal the ovaries to produce more. Seeing both numbers together gives a much clearer picture than either one alone.
What About At-Home Hormone Tests?
At-home hormone kits that use saliva or finger-prick blood samples have become widely available, but their reliability is questionable. Hormone levels are transient, fluctuating throughout the day and across the menstrual cycle, which makes a single snapshot easy to misinterpret. Mail-in samples can also be affected by contamination or improper handling during shipping. If you’re trying to confirm whether low estrogen is behind your symptoms, a clinical blood draw timed to the right day of your cycle will give you far more useful information than an at-home kit.
Patterns That Point to Low Estrogen
No single symptom proves low estrogen on its own. Hot flashes can come from thyroid problems, brain fog can come from poor sleep or stress, and vaginal dryness can have other causes. What makes low estrogen likely is the pattern: multiple symptoms from different categories showing up around the same time, particularly alongside menstrual changes. If you’re experiencing three or four of the symptoms described above, and especially if your periods have become irregular, shorter, or absent, low estrogen is high on the list of explanations. A blood test can confirm what your body is already telling you.
