Insulin resistance often develops silently over years, but your body does leave clues. Some are visible in the mirror, others show up in routine blood work, and a few you can estimate at home with a tape measure. The tricky part is that standard lab ranges are wide enough to miss early-stage resistance, so knowing what to look for beyond the basics matters.
Visible Signs on Your Body
One of the most reliable physical markers is a skin condition called acanthosis nigricans: patches of dark, velvety skin that typically appear in body folds. The neck, armpits, groin, and under the breasts are the most common spots. The patches can be brown or black, sometimes itchy, and occasionally have an odor. Small skin tags in the same areas often accompany them. These changes are driven directly by high circulating insulin levels stimulating skin cells to grow, so their presence is a strong signal that insulin resistance is already underway.
For women, another major clue is polycystic ovary syndrome (PCOS). The CDC notes that women with PCOS often have insulin resistance, and the two conditions fuel each other. Excess insulin drives the ovaries to produce more androgens (male hormones), which can cause irregular periods, acne along the jawline, and excess facial or body hair. If you’ve been diagnosed with PCOS or notice these patterns, insulin resistance is worth investigating even if your blood sugar looks normal.
What Standard Blood Tests Reveal
A basic metabolic panel at your annual checkup usually includes fasting glucose. If that number comes back at 100 mg/dL or higher, it’s flagged as elevated. But fasting glucose is a late-stage marker. Your body can keep blood sugar in a normal range for years by pumping out extra insulin, so glucose alone doesn’t catch early resistance.
The more telling number is fasting insulin, and this is where things get confusing. Most labs report anything under 25 μIU/mL as “normal.” That range was designed to catch severe hyperinsulinemia, not subtle metabolic dysfunction. Many clinicians focused on metabolic health consider optimal fasting insulin to be much lower, in the range of roughly 2.6 to 5 μIU/mL. Someone with a fasting insulin of 18 would technically be “normal” by standard lab criteria but could already be compensating hard for tissue-level resistance. If your doctor doesn’t routinely order fasting insulin, you can ask for it specifically.
When both fasting glucose and fasting insulin are available, they can be combined into a score called HOMA-IR. The calculation multiplies fasting insulin (in μU/mL) by fasting glucose (in mg/dL), then divides by 405. A healthy result is around 1.0, and values above 2.5 generally indicate insulin resistance. This single number gives a clearer picture than either glucose or insulin alone.
Your Cholesterol Panel Holds a Hidden Clue
A standard lipid panel measures triglycerides and HDL cholesterol, and the ratio between these two numbers is a surprisingly good proxy for insulin resistance. To calculate it, divide your triglycerides by your HDL (both in mg/dL). Research published in PLOS ONE found that the cutoff suggesting resistance varies by sex and ethnicity: roughly 3.8 for white European men and 2.0 for white European women, with lower thresholds (2.8 and 2.5) for South Asian men and women respectively.
In practical terms, if your triglycerides are climbing while your HDL is dropping, that pattern points toward insulin resistance even when your glucose is still in range. High triglycerides paired with low HDL is one of the five markers used to diagnose metabolic syndrome.
The Five Markers of Metabolic Syndrome
Metabolic syndrome is essentially a clinical snapshot of insulin resistance at work. Meeting three or more of these five criteria earns the diagnosis:
- Waist circumference: 40 inches or more in men, 35 inches or more in women
- Triglycerides: 150 mg/dL or higher
- HDL cholesterol: below 40 mg/dL in men, below 50 mg/dL in women
- Blood pressure: 130/85 mmHg or higher
- Fasting glucose: 100 mg/dL or higher
You may already know most of these numbers from a recent physical. If you hit three, insulin resistance is very likely the underlying driver. Even meeting two is worth paying attention to.
A Tape Measure Test You Can Do at Home
You don’t need a lab to get useful information. Your waist-to-height ratio is a simple, well-studied predictor of the visceral fat that’s closely linked to insulin resistance. Measure your waist at your navel (not at your belt line), then divide that number by your height, using the same units for both.
A ratio above roughly 0.5 signals elevated risk. Research in Frontiers in Endocrinology found that a cutoff of about 0.52 effectively predicted insulin resistance in women with PCOS, and the 0.5 threshold is widely used as a general benchmark. If your waist is more than half your height, visceral fat accumulation is likely contributing to metabolic problems regardless of what the scale says. This is one reason some people at a “normal” BMI can still be insulin resistant: it’s where the fat sits, not just how much there is.
The Glucose Tolerance Test: A Deeper Look
If your doctor wants a more detailed picture, they may order an oral glucose tolerance test (OGTT). You drink a standardized glucose solution, then have blood drawn at intervals over two or more hours. The standard version tracks glucose, but adding insulin measurements at each time point reveals much more.
In a healthy response, insulin rises moderately within 30 to 60 minutes after the glucose load, then drops back down quickly. In insulin resistance, the pattern shifts: insulin peaks higher, peaks later, or takes much longer to come back down. Research based on the work of Joseph Kraft, who analyzed thousands of these curves, identified distinct patterns. One particularly telling sign is when insulin doesn’t peak until the two-hour mark. A study of Japanese American men found that this delayed insulin peak significantly increased the risk of developing type 2 diabetes over the following decade.
This extended test isn’t routine, and many labs only measure glucose during an OGTT. But if you’re getting borderline results on simpler tests and want clarity, asking for insulin to be measured alongside glucose during a tolerance test can catch resistance that fasting labs miss.
Symptoms That Are Easy to Overlook
Beyond lab results and skin changes, insulin resistance often produces symptoms people attribute to aging, stress, or poor sleep. Persistent fatigue after meals, especially carbohydrate-heavy ones, is common. So is intense hunger that returns quickly after eating, brain fog in the afternoon, and difficulty losing weight around the midsection despite consistent effort.
None of these symptoms alone is diagnostic, but when several cluster together alongside even one or two of the measurable markers above, the picture becomes clearer. The challenge with insulin resistance is that it occupies a gray zone: you don’t feel sick enough to seek help, and standard screening often labels you as fine. Knowing which specific numbers to request and what thresholds actually matter gives you a much sharper lens than the typical annual checkup provides.
