The most common sign of low iron is a persistent, heavy fatigue that doesn’t improve with sleep. But iron deficiency is tricky because it develops in stages, and many of its symptoms overlap with other conditions. The only way to confirm it is with a blood test. That said, your body does send signals worth paying attention to, and knowing what to look for can help you decide whether it’s time to get checked.
Why Iron Matters in Your Body
Iron is a core building block of hemoglobin, the protein in red blood cells that carries oxygen from your lungs to every tissue in your body. It also plays a critical role inside your cells’ energy-producing centers (mitochondria), where it helps convert oxygen into usable energy. When iron levels drop, your body can’t make enough functional red blood cells, and your tissues start running on less oxygen. That’s why the symptoms of low iron affect so many different parts of your life, from your energy to your mood to how well you think.
Common Symptoms to Watch For
Iron deficiency doesn’t announce itself all at once. In the earliest stage, your stored iron drops but your body compensates, so you may feel completely fine. As stores get lower, your body struggles to keep up with demand, and symptoms begin to appear. By the time you’ve progressed to full iron deficiency anemia, the signs are harder to miss.
The symptoms most people notice first are:
- Unusual tiredness. Not the kind you fix with a good night’s sleep. This is a deep, persistent exhaustion that makes normal activities feel draining.
- Pale skin, nail beds, or inner eyelids. With fewer healthy red blood cells, your skin loses some of its color. Pulling down your lower eyelid and checking whether the inner rim looks pale pink or whitish is a quick self-check many doctors use.
- Shortness of breath during routine activity. Walking up stairs or carrying groceries may leave you winded in a way it didn’t before.
- Cold hands and feet. Less oxygen reaching your extremities means your body has trouble regulating temperature there.
- Brittle nails or hair loss. Your body prioritizes vital organs when oxygen is limited, so hair and nails suffer early.
- Headaches or dizziness. Especially when standing up quickly.
Symptoms You Might Not Expect
Some of the most telling signs of low iron are ones people rarely connect to it. Restless legs syndrome, that uncomfortable urge to move your legs especially at night, has a well-established link to low brain iron. Scientists have found that iron levels in the brain directly affect dopamine signaling, and when brain iron drops, dopamine activity decreases in a pattern that worsens at night. Importantly, even people with significant iron deficiency in the brain may not be anemic on standard blood tests, so restless legs can appear long before your red blood cell counts look abnormal.
Another unusual sign is pica: strong cravings for non-food items like ice, dirt, or starch. Compulsive ice chewing (pagophagia) is one of the more common forms and is closely associated with iron deficiency. If you find yourself craving and crunching through cups of ice daily, that’s worth mentioning to your doctor. Difficulty concentrating, brain fog, and irritability are also common but frequently blamed on stress or poor sleep instead.
Who Is Most Likely to Be Low on Iron
Certain groups are significantly more likely to develop iron deficiency, and knowing whether you fall into one of them can help you gauge your risk.
Women with heavy menstrual periods are among the most affected. Heavy bleeding during periods may account for 33% to 41% of iron deficiency anemia cases in women of reproductive age. Pregnancy also dramatically increases iron needs because of expanding blood volume and the demands of the growing fetus. Without supplementation, many pregnant women develop iron deficiency, which raises the risk of premature birth and low birthweight.
Frequent blood donors are another high-risk group that often gets overlooked. About 25% to 35% of regular donors develop iron deficiency. One study found that men who donated three or more times in a year and women who donated twice or more were over five times as likely to have depleted iron stores compared to first-time donors.
Gastrointestinal conditions like celiac disease, Crohn’s disease, and ulcerative colitis impair iron absorption, as does any surgery that removes or bypasses part of the small intestine. Slow, hidden blood loss from conditions like peptic ulcers, colon polyps, or colorectal cancer can also drain iron stores gradually. Up to 60% of people with colon cancer have iron deficiency at the time of diagnosis.
If you follow a vegetarian or vegan diet, your iron needs are about 1.8 times higher than someone who eats meat. That’s because plant-based (nonheme) iron is harder for your body to absorb than the heme iron found in animal products. It’s entirely possible to get enough iron without meat, but it requires more deliberate planning.
How Iron Deficiency Is Confirmed
Symptoms alone can’t tell you for sure. The only reliable way to know your iron status is through blood work. Your doctor will typically order one or more of the following:
- Ferritin. This measures your stored iron and is the most sensitive early indicator. Normal ranges are roughly 15 to 205 ng/mL for women and 30 to 566 ng/mL for men, though being at the very low end of “normal” can still mean your stores are marginal. The World Health Organization defines iron deficiency as ferritin below 15 micrograms per liter in healthy individuals, but raises that threshold to 30 in children and 70 in adults who have any inflammation or infection, since inflammation artificially inflates ferritin levels.
- Serum iron. A snapshot of how much iron is circulating in your blood right now.
- Total iron-binding capacity (TIBC). Measures how much iron your blood could carry. When your stores are low, TIBC goes up because your body is trying to grab more iron from whatever is available.
- Transferrin saturation. The percentage of your iron-transport protein that is actually carrying iron. A low percentage confirms your body isn’t getting enough.
In the earliest stage of deficiency, only ferritin drops. Everything else looks normal. By the second stage, ferritin drops further while TIBC and transferrin start shifting, but your hemoglobin (the standard measure of anemia) still looks fine. It’s only in the third stage, full-blown iron deficiency anemia, that hemoglobin falls and red blood cells become smaller and paler than normal. This is why a basic blood count alone can miss early iron deficiency. If you suspect you’re low, specifically ask for ferritin.
Preparing for Your Blood Test
Your doctor may ask you to fast for 12 hours before an iron panel, and the test is typically done in the morning. Iron levels in the blood fluctuate throughout the day, so morning draws give the most consistent results. If you take iron supplements, ask your provider whether to skip them the day before testing, as recent supplementation can temporarily spike your serum iron and mask a deficiency.
Improving Iron Absorption
If you’re trying to boost your iron levels through food or supplements, what you eat alongside iron matters. Vitamin C significantly increases absorption of plant-based iron, so pairing iron-rich foods with citrus, bell peppers, or tomatoes makes a real difference. Beta-carotene, found in carrots, sweet potatoes, and leafy greens, also improves iron uptake and can partially counteract substances that block absorption.
The main absorption blockers are tannins (found in tea and coffee) and phytates (found in whole grains, beans, and nuts). Drinking tea or coffee with meals can meaningfully reduce how much iron you absorb. If you’re working on raising your levels, it helps to separate these beverages from your iron-rich meals by at least an hour. Calcium also competes with iron for absorption, so taking calcium supplements at a different time than iron supplements or iron-heavy meals gives your body a better chance to take in the iron it needs.
