Some people also notice sudden changes in breathing pattern during sleep, either unusually fast or slow.
Waking up with a headache, feeling unusually groggy, or having no memory of a disrupted night can also point to a low that corrected itself overnight. If these patterns repeat, it’s worth checking your blood sugar before bed and adjusting your evening routine.
When You Stop Feeling the Warnings
Some people, especially those who experience frequent lows, gradually lose the ability to feel early symptoms. This is called hypoglycemia unawareness, and it happens because repeated low episodes reset the body’s alarm threshold. If your blood sugar triggered shakiness at 60 mg/dL yesterday, today you might not feel anything until it hits 55. The threshold for symptoms keeps dropping, but the threshold for passing out does not. That gap is what makes this condition dangerous: you may feel fine right up until you lose consciousness.
Hypoglycemia unawareness is most common in people who take insulin or certain oral diabetes medications. The good news is that it can be reversed. Studies show that if you avoid low blood sugar episodes for a sustained period, your body can recalibrate and start producing warning symptoms again at safer levels. A continuous glucose monitor (CGM) is one of the most effective tools for people at risk, because it tracks your levels around the clock and alerts you when glucose drops too low, filling in for symptoms your body no longer provides. Checking your blood sugar before driving is especially important if you’ve experienced unawareness.
How to Confirm and Respond
If you have a glucose meter or CGM, checking your blood sugar is the fastest way to confirm what your body is telling you. A reading below 70 mg/dL confirms a low. But if you don’t have a meter nearby and you’re experiencing the classic combination of shakiness, sweating, and sudden hunger, treat first and test when you can.
The standard approach is the 15-15 rule: eat or drink 15 grams of fast-acting carbohydrates, then wait 15 minutes and check your blood sugar again. If it’s still below 70 mg/dL, repeat the process. Fifteen grams looks like about four glucose tablets, half a cup of juice or regular soda, or a tablespoon of honey. Choose something that’s pure sugar with no fat or protein, which slow absorption. Once your level stabilizes, follow up with a small snack that includes protein or complex carbs to keep it steady.
If someone with low blood sugar becomes confused, loses consciousness, or has a seizure, they need help from another person. Do not try to put food or liquid in the mouth of someone who is unconscious, as it can cause choking. Glucagon, available as an injection or nasal spray, is the emergency treatment. If glucagon isn’t available or you don’t know how to use it, call 911 immediately.
Patterns Worth Paying Attention To
A single low blood sugar episode might be explained by a skipped meal, unexpected exercise, or a medication timing issue. But if you’re noticing these symptoms repeatedly, at the same time of day, after the same activities, or during the night, that pattern carries useful information. Tracking when lows happen, what you ate beforehand, and how active you were helps identify the trigger so you can prevent the next one rather than just treating it.
People who don’t have diabetes can also experience low blood sugar, though it’s less common. Drinking alcohol on an empty stomach, prolonged fasting, and certain medical conditions can all cause glucose to drop below normal. If you’re having recurring symptoms of low blood sugar and you don’t take diabetes medication, that’s worth investigating, because the underlying cause matters.
