Eye tattooing involves injecting ink with a needle underneath the thin, clear membrane that covers the white of the eye, depositing color onto the sclera (the tough white layer beneath). The ink then slowly spreads from the injection sites to cover the surrounding area, changing the visible color of the eye’s white surface. This is fundamentally different from a skin tattoo: there’s no repeated needling of a design. Instead, liquid pigment is placed in a few spots and allowed to migrate outward, producing a wash of solid color rather than a detailed image.
How the Procedure Works
The technique, known as scleral tattooing, first appeared around 2007 as a form of extreme body modification. A practitioner uses a fine needle to inject small amounts of ink into the subconjunctival space, the thin gap between the conjunctiva (the eye’s outermost transparent layer) and the sclera underneath. Multiple injection points are placed around the eye so the ink can spread evenly. The goal is a uniform tint across the entire visible white of the eye, though the final result depends heavily on how the ink disperses.
The people performing these procedures are typically body modification artists, not eye surgeons. The work is done without a surgical microscope or the sterile operating room conditions that eye surgery normally requires. There’s no standardized training, no regulated technique, and no approved ink formulation specifically designed for injection into the eye. The inks used are generally tattoo pigments adapted from skin tattooing, not substances tested or approved for use inside the eye.
Scleral Tattoos vs. Keratopigmentation
It’s worth distinguishing the cosmetic scleral tattoo from a medical procedure called keratopigmentation. Keratopigmentation is performed by ophthalmologists on the cornea (the clear front part of the eye, over the iris) rather than on the white. It has legitimate medical uses: correcting the appearance of corneal scars, reducing glare and light sensitivity in people with iris damage, and improving comfort for patients with certain types of eye injuries. In therapeutic cases, patients sometimes see an improvement in visual acuity after the procedure.
A newer purely cosmetic version of keratopigmentation allows people to permanently change their iris color. This is done by trained eye surgeons under controlled conditions, and studies show visual acuity generally remains stable afterward. Still, even in these professional settings, reported side effects include pain (81% of cosmetic patients in one review), dry eye (76%), redness (67%), tingling (71%), and light sensitivity (56%). The complication profile is significant even when an experienced surgeon performs the work in an operating room.
Scleral tattooing, by contrast, has no medical application. It exists purely as body modification and carries a far less predictable risk profile because of the uncontrolled conditions in which it’s performed.
What Can Go Wrong
The list of documented complications from scleral tattooing is long and serious. Short-term risks include eye pain, blurry vision, sensitivity to light, and staining of surrounding tissues when ink migrates to areas it wasn’t meant to reach. Granulomas, which are small inflammatory lumps, can form around the injected pigment. Because the ink sits directly under the conjunctiva, it can trigger uveitis, a painful inflammation inside the eye that can threaten vision if untreated.
The more severe risks stem from the basic mechanics of the procedure. Pushing a needle into the eye without magnification and surgical training raises the chance of penetrating the globe itself. Case reports have documented traumatic cataracts, retinal detachment, and endophthalmitis (a devastating infection inside the eye that can lead to blindness). A review of ocular tattoo complications found that 71.5% of adverse events occurred within the first six months. Warning signs that something has gone wrong include excessive redness, swelling, discharge, changes in vision, and persistent eye pain.
The ink itself is another variable. Since no tattoo pigments are formulated or tested for injection into the eye, every procedure is essentially an uncontrolled experiment. The body may react to the pigment with chronic inflammation, and the long-term effects of having tattoo ink permanently sitting against the sclera remain poorly understood.
Can an Eye Tattoo Be Removed?
For practical purposes, scleral tattoos are permanent and irreversible. The ink spreads through the subconjunctival space in a way that can’t be precisely targeted by laser removal, and surgical removal would require peeling away the conjunctiva, risking further damage to the eye. Unlike a skin tattoo, where laser treatments can gradually break down pigment over multiple sessions, the delicate structures of the eye don’t tolerate that kind of intervention. If the result is uneven, the wrong color, or causing ongoing problems, there is no reliable fix.
Legal Restrictions
Several jurisdictions have moved to ban or restrict scleral tattooing. Parts of Australia, including New South Wales, have issued formal health warnings about the practice. Some U.S. states have passed laws specifically prohibiting eyeball tattooing, and the procedure occupies a legal gray area in many other places where existing tattoo regulations weren’t written with eye injections in mind. The lack of regulation in most areas means that where it is available, there’s no guarantee of practitioner competence, sterile conditions, or safe materials.
What the Experience Looks Like
People who’ve had the procedure describe it as quick but unsettling. The injections themselves take only minutes, but the eye typically becomes swollen and red immediately afterward. Vision may be blurry for days or weeks. The ink takes time to spread, and the final appearance isn’t visible right away, sometimes settling unevenly or producing a different shade than expected. Because the conjunctiva is rich in blood vessels and nerve endings, pain during and after the procedure is common, and the eye can remain irritated for weeks.
There’s no established aftercare protocol the way there is for skin tattoos. Most practitioners offer general advice about keeping the eye clean and avoiding rubbing, but there’s no clinical guidance to draw from. If complications develop, the person will need to see an ophthalmologist, who may have limited options for treating pigment-related problems inside the eye.
