How Do You Get a Crick in Your Neck and Relieve It?

A crick in your neck happens when one or more muscles along the side or back of your neck suddenly tighten or spasm, locking your head into a limited range of motion. The most common culprit is the levator scapulae, a muscle that runs from the top of your shoulder blade up to the upper vertebrae of your neck. When this muscle seizes up, you get that signature feeling: a deep, achy tightness along the upper back and neck that makes turning your head painful or nearly impossible.

The Muscle Behind Most Neck Cricks

The levator scapulae sits beneath the larger trapezius muscle and is responsible for lifting and rotating your shoulder blade. Because of its position and the amount of work it does throughout the day, it’s especially prone to tension and spasm. When it tightens up, it restricts your ability to bend your neck forward or tilt it to the opposite side. The dominant shoulder is involved about 82% of the time, which makes sense given that most people overwork one side through daily habits like holding a phone, carrying a bag, or using a mouse.

Pain from this muscle typically radiates to the neck and shoulder but rarely extends down the arm. It can also develop trigger points, which are small knots of contracted muscle fiber that refer pain to other areas. Trigger points in the levator scapulae commonly send pain along the inner edge of the shoulder blade and can even cause tension headaches that wrap around the side of the head.

What Triggers a Crick

Most people get a crick in their neck from one of a few predictable situations:

  • Sleeping in an awkward position. This is the classic scenario. You fall asleep with your neck twisted or propped at an odd angle, and the muscles on one side stay shortened for hours. You wake up with stiffness locked down one side of your neck, sometimes severe enough that your head tilts visibly to one side.
  • Sudden or jerky movements. Whipping your head around quickly, looking over your shoulder while reversing a car, or any abrupt rotation can overstretch or irritate the neck muscles and trigger a spasm.
  • Prolonged poor posture. Hours spent hunched over a laptop or looking down at a phone puts continuous strain on the muscles along the back and sides of the neck. The levator scapulae is particularly vulnerable because it has to work harder to stabilize your head when it’s pitched forward.
  • Cold exposure. Sitting under an air conditioning vent or sleeping near an open window can cause neck muscles to contract and stiffen. Cold air reduces blood flow to the muscles, making them more susceptible to spasm.
  • Stress and tension. When you’re anxious or stressed, you tend to hike your shoulders upward and clench muscles you’re not even aware of. Over time, this chronic low-level contraction fatigues the levator scapulae and upper trapezius, setting the stage for a spasm.

In many cases, a crick results from a combination of these factors rather than a single event. You might have spent the day hunched at your desk, then slept in a draft with your neck at an angle, and woke up unable to turn your head.

What a Crick Feels Like

The hallmark is one-sided stiffness. You can usually turn your head toward the painful side without much trouble, but turning or tilting away from it stretches the tight muscle and sends a sharp, grabbing pain through the side of your neck. Bending your chin toward your chest often hurts too.

The pain itself tends to be deep and achy rather than sharp or electric. It sits along the top of the shoulder blade, the side of the neck, or both. Some people also feel a dull headache on the same side, especially at the base of the skull. This headache is typically caused by trigger points in the tight muscle referring pain upward.

How to Relieve It

For the first 48 hours, stick with ice or cold packs if you suspect there’s any minor strain or irritation involved. Wrap ice in a towel and apply it for 15 to 20 minutes at a time. After that initial window, switching to heat is more effective. Heat reduces muscle spasm and joint stiffness, which is exactly what you need once the acute irritation has settled. A warm shower aimed at the tight side of your neck, a microwaveable heat wrap, or even a warm towel works well.

Gentle movement helps more than total immobilization. Keeping your neck completely still can actually prolong stiffness because the muscles tighten further from disuse. Slowly and carefully turn your head in each direction, stopping before you hit significant pain. The goal is to maintain some range of motion, not push through sharp discomfort.

Isometric exercises are a safe option even during the acute phase. These involve pressing against resistance without actually moving your neck. Place your palm flat against your forehead and push your head gently into your hand, resisting with your neck muscles. Hold for about 10 seconds, relax, and repeat five times. Then do the same thing pressing against the side of your head, then the back. This activates and engages the muscles without forcing them through a range of motion that could aggravate the spasm.

Over-the-counter anti-inflammatory pain relievers can take the edge off and reduce any swelling around irritated muscle fibers, making it easier to move and sleep.

How Long Recovery Takes

A straightforward crick in the neck, the kind caused by sleeping wrong or sitting too long in one position, typically resolves within a few days to a week. Most people notice significant improvement within the first two to three days, especially if they use heat and gentle movement. Stiffness may linger for a few more days after the sharp pain fades, but full range of motion usually returns within seven to ten days without any special treatment.

If a crick keeps coming back, that’s a sign of an underlying pattern worth addressing. Recurring episodes often point to a workstation setup that strains your neck, a pillow that doesn’t support your cervical spine well, or chronic tension you’re carrying in your shoulders.

Pillow and Sleep Position

Your pillow plays a bigger role than most people realize, though the research on ideal pillow design is less clear-cut than you might expect. What matters most is that your pillow keeps your neck aligned with the rest of your spine rather than propping it up at an angle or letting it drop. The shape and height of the pillow affect cervical alignment more than the material does. A pillow that’s too high pushes your head forward; one that’s too flat lets it fall back or to the side.

Roll-shaped pillows, sometimes marketed as cervical support pillows, can actually force the neck into too much backward extension and are poorly tolerated by many people. If you sleep on your side, the pillow needs to be thick enough to fill the gap between your shoulder and ear so your neck stays straight. If you sleep on your back, a thinner pillow that supports the natural curve of your neck without pushing your chin toward your chest is a better fit. Stomach sleeping is the hardest position on the neck because it forces you to rotate your head to one side for hours.

When a Crick Might Be Something Else

A simple crick stays in the neck and shoulder area and gets better over days. Certain symptoms suggest something beyond a muscle spasm. Pain that radiates down your arm, especially with numbness, tingling, or weakness in your hand or fingers, can indicate a pinched nerve in the cervical spine. The pattern of numbness matters: tingling in the thumb and outer forearm points to a different nerve root than tingling in the ring and little finger, and each pattern carries different implications.

Other warning signs include fever alongside neck pain, unexplained weight loss, pain that consistently wakes you from sleep (not just hurts when you roll over, but actively wakes you), or weakness in specific muscle groups like difficulty gripping objects or raising your arm. If neck stiffness persists beyond six weeks despite home treatment, that also warrants a closer look. A useful self-check: if raising your arm above your head on the painful side relieves the arm symptoms, that’s a strong indicator the problem is in your neck rather than your shoulder.
