How Do You Get a Sore Throat? Common Causes

Most sore throats come from viral infections, which account for 50% to 80% of all cases. The rest are caused by bacteria, environmental irritants, or underlying conditions like acid reflux. Understanding which cause is behind your sore throat matters because it determines whether you need treatment or just time.

Viral Infections Are the Most Common Cause

The common cold is the single biggest source of sore throats. Rhinoviruses, the viruses behind most colds, inflame the lining of your throat as they multiply in your upper airway. But colds aren’t the only culprit. The flu, COVID-19, seasonal coronaviruses, respiratory syncytial virus (RSV), and adenoviruses all cause sore throats as part of their symptom package. Even mono, caused by the Epstein-Barr virus, often starts with a severe sore throat that can last weeks.

These viruses spread the same way: through tiny droplets released when an infected person coughs, sneezes, talks, or breathes near you. You can also pick them up by touching a contaminated surface and then touching your mouth, nose, or eyes. Most viral sore throats develop within one to three days of exposure and resolve on their own within five to seven days. Antibiotics do nothing against them.

A viral sore throat typically comes with other cold or flu symptoms: runny nose, sneezing, cough, mild body aches, or a hoarse voice. If your sore throat fits that pattern, a virus is almost certainly the cause.

Bacterial Infections, Especially Strep Throat

Group A Streptococcus is the bacterium responsible for strep throat, the most common bacterial cause of a sore throat. It spreads through respiratory droplets, so close contact with an infected person is the primary route. Sharing drinks, kissing, or being in a crowded room with someone who has strep can all transmit it. The bacteria can also spread through contact with saliva or nasal secretions, and in rare cases through contaminated food. Surface transmission is possible but less common.

After exposure, it usually takes two to five days before symptoms appear. Strep throat tends to feel different from a viral sore throat. The pain often comes on suddenly, is more intense, and typically occurs without the cough or runny nose you’d expect from a cold. You might also notice swollen lymph nodes in your neck, white patches on your tonsils, or a fever above 101°F. Strep requires antibiotics because untreated infections can lead to complications affecting the heart and kidneys.

Children between ages 5 and 15 get strep throat most often, though adults are not immune. A rapid strep test or throat culture at a clinic can confirm or rule it out quickly.

Dry Air and Environmental Irritants

You don’t need an infection to get a sore throat. Breathing dry air, especially during winter when indoor heating strips moisture from the air, dries out the mucus lining your throat. That mucus normally acts as a protective, gooey coating that traps germs and keeps tissues moist. When it dries out, you’re left with a scratchy, inflamed throat that feels worse in the morning after hours of breathing dry air while sleeping.

Keeping indoor humidity between 30% and 50% helps prevent this. A humidifier in your bedroom is the most straightforward fix. Below 30%, your sinuses also dry out, which makes you more vulnerable to catching the very infections that cause sore throats in the first place, because dried-out mucus is less effective at trapping germs.

Other environmental triggers include cigarette smoke, air pollution, chemical fumes, and dust. Breathing through your mouth, whether from nasal congestion or a habit during sleep, also dries the throat and can cause soreness by morning.

Acid Reflux and Throat Irritation

Stomach acid that travels up into your throat can cause a persistent, low-grade sore throat that doesn’t behave like an infection. This happens when stomach contents, including acid and digestive enzymes, back up past the esophagus and reach the larynx and pharynx. The tissues in your throat are more sensitive to acid than the esophagus is, and refluxed acid tends to pool there, resulting in prolonged exposure and more damage.

This type of sore throat often comes with a feeling of something stuck in your throat, frequent throat clearing, a hoarse voice, or a bitter taste. It tends to be worse after meals, when lying down, or in the morning. Many people with reflux-related sore throats don’t experience the classic heartburn you’d associate with acid reflux, which makes it easy to overlook as a cause. If your sore throat keeps coming back without any signs of infection, reflux is worth considering.

Other Common Triggers

Allergies cause sore throats indirectly. When your nose is congested or producing excess mucus from an allergic reaction, that mucus drips down the back of your throat (postnasal drip), irritating the tissue and causing soreness. Seasonal allergies, pet dander, and dust mites are frequent triggers.

Straining your voice by yelling, singing, or talking for extended periods can inflame the muscles and tissues in your throat. Teachers, coaches, and singers are especially prone to this. The soreness usually feels more like muscle fatigue than the sharp pain of an infection.

Breathing through your mouth during sleep, often because of nasal congestion or sleep apnea, exposes your throat to hours of unhumidified air. This is one of the most common reasons people wake up with a sore throat that fades by midmorning.

How to Tell What’s Causing Yours

A few patterns help narrow it down. If your sore throat came with a runny nose, cough, and sneezing, it’s almost certainly viral. If the pain is severe, came on fast, and you have a fever but no cough, strep is more likely and worth testing for. If the soreness is mild and only shows up in the morning or in dry environments, the air you’re breathing is probably the issue. And if it lingers for weeks without other symptoms, reflux or allergies deserve a closer look.

Most sore throats, regardless of cause, improve with basic comfort measures: staying hydrated, using lozenges, and breathing humidified air. The key distinction is whether you need antibiotics (bacterial) or not (everything else), which is why identifying the cause matters more than treating the symptom alone.
