Corns form when repeated pressure or friction on the same spot of your foot triggers your skin to build up thick, hardened layers as a defense mechanism. This thickening, called hyperkeratosis, is your skin’s normal protective response to mechanical stress. It only becomes a problem when the buildup grows large enough to cause pain or inflammation on its own.
What Happens Inside Your Skin
When a specific area of your foot is subjected to chronic pressure or friction, the skin cells in the outermost layer ramp up their activity. They produce extra keratin, a tough structural protein, and stack it into dense layers. Over time, this creates a visible, hardened bump.
What distinguishes a corn from a simple callus is its shape. A corn develops a central cone-shaped core of compacted keratin that points inward, pressing into deeper layers of skin. That inward-pointing core is why corns hurt. It acts like a small wedge being driven into your foot with every step, irritating the tissue underneath. A callus, by contrast, is a broader, flatter patch of thickened skin without that concentrated core.
The Main Causes: Shoes and Foot Shape
The two biggest factors behind corn formation are footwear and the structural anatomy of your foot. They often work together.
Shoes cause corns through several specific design features. A tight or narrow toe box squeezes the toes together, creating friction between them and pressure on the tops and sides. Pointed-toed shoes force the toes into unnatural positions, concentrating pressure on bony areas. High heels push the toes forward into the front of the shoe, increasing pressure on the ball of the foot and the tips of the toes. Even shoes that are too loose can cause corns, because your foot slides around inside and rubs repeatedly against the same spots.
Foot structure matters just as much. Hammertoe, a condition where a muscle imbalance causes the first joint of a toe to curl upward, creates a prominent knuckle that rubs against the inside of your shoe with every step. Bunions push the big toe sideways, increasing friction along its inner edge and crowding the smaller toes. Any bony prominence, whether from a deformity or just your natural bone structure, creates a pressure point where a corn is likely to develop.
Three Types of Corns and Where They Form
Not all corns look or feel the same. The type depends on where the pressure is coming from.
- Hard corns are small, dense bumps that usually form on the tops of toes, right over the joints where bone pushes outward against your shoe. These are the most common type.
- Soft corns appear between the toes, where two bones press against each other. They stay soft and whitish-gray because the moisture between your toes keeps the skin from fully hardening.
- Seed corns are tiny and form on the bottom of the feet. They tend to appear in clusters and are often associated with dry skin.
High heels are a particularly common cause of hard corns on the tops of the toes and seed corns on the ball of the foot, because they shift your body weight forward onto those areas.
Other Risk Factors
Going sockless or wearing socks that don’t fit well removes a friction buffer between your skin and your shoe. Internal seams in poorly constructed shoes can also create repetitive rubbing in one spot. People who spend long hours on their feet, especially on hard surfaces, generate more cumulative pressure on the soles and toes.
Age plays a role too. The natural fat padding on the bottom of your feet thins over time, leaving bones closer to the surface and more exposed to pressure. This is one reason corns become more common in older adults even when shoe habits haven’t changed.
Corn or Wart? How to Tell the Difference
Corns and plantar warts can look similar at first glance, since both are small, flesh-colored, and firm. But there are reliable visual differences. A corn is a raised, hard bump surrounded by dry, flaky skin, with smooth skin on its surface. A wart has a grainy, rough texture and contains tiny black dots scattered through it. Those black dots are small, clotted blood vessels. Warts are caused by a virus, not by pressure, so they can appear anywhere on the foot regardless of friction points.
What You Can Do About Them
Since corns are caused by pressure, the most effective approach is removing the source. Switching to shoes with a wide or rounded toe box lets your toes spread naturally and reduces friction between them. Lower heels take pressure off the front of the foot. Cushioned pads or moleskin placed over the corn can shield it from further irritation while it heals. Once the pressure is gone, many corns gradually resolve on their own as the thickened skin naturally sheds.
Over-the-counter corn removal products typically contain 40% salicylic acid, which chemically dissolves the hardened keratin. These come as medicated pads or plasters that you apply directly to the corn. They work, but they carry real risks for certain people. If you have diabetes, poor circulation in your feet, or any signs of infection or redness around the corn, salicylic acid products are not safe to use. Reduced blood flow slows healing, and reduced nerve sensation means you may not feel the acid damaging healthy skin around the corn.
For corns that persist or keep coming back despite changes in footwear, a podiatrist can trim away the thickened skin with a scalpel during a routine office visit. This provides immediate pain relief, though the corn will return if the underlying pressure source isn’t addressed. Custom-molded shoe inserts can redistribute pressure away from problem areas, which is especially useful if a structural foot deformity is involved. In cases where a bony abnormality like a hammertoe or bone spur is the root cause, surgery to correct the bone alignment may be the only way to permanently prevent recurrence.
Why Corns Keep Coming Back
The most frustrating thing about corns is their tendency to recur. Trimming or dissolving the thickened skin treats the symptom, not the cause. If the same shoe keeps pressing on the same bony prominence, the skin will mount the same protective response and rebuild the corn. Lasting results require identifying the specific pressure point and either changing what contacts it (better shoes, padding, orthotics) or changing the bone structure underneath it. Treating a corn without addressing why it formed is essentially temporary maintenance.
