Hepatitis B and hepatitis C are both viral infections that damage the liver, but they spread in different ways. Hepatitis B passes through blood, semen, and other body fluids, while hepatitis C spreads almost exclusively through blood-to-blood contact. Understanding the specific routes for each virus helps you know your actual risk.
How Hepatitis B Spreads
Hepatitis B (HBV) transmits when blood, semen, or other body fluids from an infected person enter the body of someone who isn’t infected. Even microscopic amounts of blood can carry enough virus to cause infection, partly because HBV is remarkably hardy: it can survive on surfaces and remain infectious for at least seven days. That durability makes it far easier to contract through casual blood exposure than many people realize.
The most common transmission routes include:
- Sexual contact with an infected partner, through vaginal, anal, or oral sex
- Sharing needles or syringes for injecting drugs
- Needle sticks or sharps injuries in healthcare settings
- Mother to baby during childbirth
- Sharing personal items that may carry trace amounts of blood, such as razors or toothbrushes
HBV is not spread through kissing, sharing utensils, sneezing, coughing, hugging, breastfeeding, or food and water. The virus needs a direct route into the bloodstream or through mucous membranes, so ordinary social contact poses no risk.
How Hepatitis C Spreads
Hepatitis C (HCV) is far more dependent on direct blood-to-blood contact. The primary route is sharing contaminated needles, syringes, or other equipment used to prepare or inject drugs. This single behavior accounts for the majority of new infections in the United States.
Sexual transmission of hepatitis C is possible but uncommon. The CDC classifies it as a “less common” route. The risk increases with practices that involve blood exposure, such as rough sex or sex during menstruation, and is higher among men who have sex with men, particularly those living with HIV. For most monogamous heterosexual couples where one partner has hepatitis C, the transmission risk is very low.
Unlike hepatitis B, HCV is not efficiently spread through mucous membrane exposure. A CDC analysis estimated that only about 0.2% of accidental needle-stick injuries in healthcare settings result in hepatitis C transmission, and mucosal exposures (like a splash to the eyes) carry essentially zero risk.
Transmission During Pregnancy and Birth
Both viruses can pass from a pregnant person to their baby, but the stakes with hepatitis B are especially high. Without preventive treatment at birth, roughly 90% of infants born to mothers with active hepatitis B will develop chronic infection. About one in four of those children will eventually die from chronic liver disease. To prevent this, newborns receive a hepatitis B vaccine and an antibody injection within 12 hours of birth.
Mother-to-child transmission of hepatitis C also occurs but at lower rates, typically around 5 to 6% of births to HCV-positive mothers. There is currently no vaccine or preventive treatment given at birth for hepatitis C.
Tattoos, Piercings, and Other Cosmetic Risks
Getting a tattoo or piercing at a licensed, professional shop does not appear to carry significant hepatitis risk. A CDC-published review found no definitive evidence of increased HCV infection from tattoos and piercings received in professional parlors that follow standard sterilization practices.
The picture changes dramatically in unregulated settings. Tattoos applied in prisons, at home, or by unlicensed individuals carry two to three times the risk of hepatitis C infection. The common thread is reused or improperly sterilized needles and equipment. If you’re getting a tattoo or piercing, a professional environment with single-use needles and autoclaved equipment is what keeps you safe.
Household and Everyday Contact
You cannot catch either virus from hugging, sharing a meal, or being in the same room as someone who is infected. The concern in households centers on items that might carry traces of blood. Razors, nail clippers, and toothbrushes are the main culprits, since small nicks or bleeding gums can leave invisible amounts of blood on these items. Health authorities recommend that people living with someone who has hepatitis B or C simply avoid sharing these personal items. That single precaution effectively eliminates household transmission risk.
Who Should Get Tested
Many people with hepatitis B or C have no symptoms for years or even decades, which means they can unknowingly spread the virus. The CDC now recommends that every adult aged 18 and older receive hepatitis B screening at least once in their lifetime, using a panel of three blood tests. This is a significant expansion from earlier guidelines that only targeted people with known risk factors.
Additional testing is recommended for people who inject or have ever injected drugs, anyone incarcerated or formerly incarcerated, people with a history of multiple sexual partners or sexually transmitted infections, and anyone born to a mother with hepatitis B. Importantly, the CDC states that anyone who requests testing should receive it regardless of whether they disclose specific risk factors, recognizing that people are often reluctant to discuss stigmatized behaviors.
For hepatitis C, the CDC recommends screening for all adults at least once and for all pregnant people during each pregnancy. People who currently inject drugs should be tested regularly.
Key Differences in Prevention
The biggest practical difference between these two viruses is that hepatitis B has a highly effective vaccine and hepatitis C does not. The hepatitis B vaccine provides long-lasting protection, with studies showing immunity persists for at least 30 years when vaccination begins in infancy. Most people in the U.S. receive this vaccine as part of their childhood immunization schedule, but adults who were never vaccinated can still complete the series.
Because no vaccine exists for hepatitis C, prevention relies entirely on avoiding blood-to-blood contact. For people who inject drugs, this means never sharing needles, syringes, cookers, cotton, or water. Syringe service programs that provide sterile injection equipment are one of the most effective public health tools for reducing new infections. The good news is that hepatitis C, unlike hepatitis B, is now curable in most cases with a short course of antiviral treatment, typically lasting 8 to 12 weeks.
