Getting rid of edema depends entirely on what’s causing it. Mild fluid retention from sitting too long or eating salty food often resolves with simple measures like elevation, movement, and cutting back on sodium. But edema caused by heart, kidney, or liver problems requires treating that underlying condition, sometimes with prescription diuretics that help your body flush excess fluid. The approach that works for you will look different depending on whether your swelling is a temporary nuisance or a sign of something deeper.
Why Fluid Builds Up in the First Place
Edema happens when fluid leaks out of your small blood vessels and gets trapped in surrounding tissue. Normally your body reabsorbs this fluid, but several things can tip the balance. Gravity is the simplest culprit: sitting or standing in one position for hours lets fluid pool in your legs and feet. A high-sodium diet makes your body hold onto water. Hormonal shifts during pregnancy or before a menstrual period do the same thing.
More serious causes involve organs that regulate fluid balance. In congestive heart failure, the heart can’t pump blood efficiently, so it backs up in the legs, ankles, and feet. Kidney disease prevents your body from clearing excess fluid and salt from the blood. Liver damage from cirrhosis reduces a protein called albumin that normally keeps fluid inside blood vessels, leading to swelling in the legs and fluid buildup in the abdomen. Damaged filtering vessels in the kidneys (nephrotic syndrome) cause the same albumin drop with similar results.
Certain medications are also common triggers. Calcium channel blockers, a widely prescribed class of blood pressure drugs, cause ankle swelling in 1 to 15 percent of people taking them. Anti-inflammatory painkillers, some diabetes medications, and steroids can all contribute to fluid retention as well.
Elevation and Movement
The fastest way to reduce mild leg swelling at home is elevation. Position your legs above the level of your heart, using pillows or a recliner, for about 15 minutes at a time. Doing this three to four times a day helps gravity pull trapped fluid back toward your core, where your body can process and eliminate it. Even sleeping with a pillow under your calves can make a noticeable difference overnight.
Movement works alongside elevation by activating the calf muscles, which act like pumps that push blood and fluid upward through your veins. If you sit at a desk all day or have limited mobility, even small exercises help. Ankle pumps (pointing your toes up and down), marching in place, wiggling your toes, and doing seated leg extensions all engage these muscle pumps without requiring much effort or space. The key is frequency: short bouts of movement throughout the day are more effective than one long session.
Reducing Sodium Intake
Sodium makes your body retain water, so cutting back is one of the most effective dietary changes for managing edema. For people with heart failure, guidelines from the Heart Failure Society of America recommend limiting sodium to 2,000 to 3,000 milligrams per day, with less than 2,000 milligrams for moderate to severe cases. Even if your edema isn’t heart-related, staying under 2,000 milligrams is a reasonable target.
That number sounds generous until you start reading labels. A single fast-food meal can contain well over 2,000 milligrams. Canned soups, deli meats, bread, condiments, and frozen meals are the biggest hidden sources. Cooking at home with fresh ingredients and seasoning with herbs, citrus, or spices instead of salt gives you the most control. Your palate adjusts within a couple of weeks, and food that once tasted bland starts tasting normal.
Compression Garments
Compression stockings apply graduated pressure to your legs, squeezing fluid upward and preventing it from pooling. They come in different pressure levels measured in millimeters of mercury (mmHg), and choosing the right level matters.
- 8 to 15 mmHg: Light support for tired, achy legs and mild swelling from long days of sitting or standing.
- 15 to 20 mmHg: Everyday support for heavier legs, mild swelling, travel days, or pregnancy-related leg discomfort.
- 20 to 30 mmHg: Medical-grade compression typically prescribed for varicose veins, moderate swelling, or venous conditions.
- 30 to 40 mmHg: Higher compression for advanced venous disease or lymphedema, usually fitted with guidance from a clinician.
For the best results, put compression stockings on first thing in the morning before swelling has a chance to develop. They should feel snug but not painful. If they leave deep marks or cause numbness, the pressure level is too high or the fit is wrong.
Prescription Diuretics
When lifestyle measures aren’t enough, doctors often prescribe diuretics, commonly called water pills. These medications help your kidneys excrete more sodium and water, reducing the volume of fluid in your body. There are three main classes, and each works on a different part of the kidney. Loop diuretics are the most powerful and are commonly used for heart failure or significant swelling. Thiazide diuretics are milder and often used for blood pressure or lighter fluid retention. Potassium-sparing diuretics are sometimes added to prevent the potassium loss that other diuretics cause.
Diuretics work quickly. You’ll notice increased urination within hours, and visible swelling often decreases within a day or two. But they treat the symptom, not the cause. If your edema comes from heart failure, kidney disease, or liver problems, diuretics are typically one part of a broader treatment plan for that condition.
Medication-Related Swelling
If your edema started around the same time you began a new medication, the drug itself may be the problem. Calcium channel blockers are the most common offenders, particularly the dihydropyridine type (amlodipine and nifedipine are well-known examples). These drugs relax blood vessels, which can cause fluid to leak into surrounding tissue, especially in the ankles and feet.
Don’t stop or change any medication on your own. But it’s worth mentioning the timing to your doctor. In many cases, switching to a different drug in the same class or adjusting the dose resolves the swelling without compromising the treatment you need.
Pitting vs. Non-Pitting Edema
There’s a simple test you can do at home that gives useful information. Press your thumb firmly into the swollen area for about 10 seconds, then release. If an indentation stays behind for several seconds, that’s pitting edema. It’s the more common type and is usually associated with heart failure, kidney disease, venous problems, or medication side effects.
If the skin springs back immediately with no lasting dent, that’s non-pitting edema. This pattern is more typical of lymphedema (a blockage or damage in the lymphatic system) or lipedema (a condition involving abnormal fat distribution, usually in the legs). These conditions don’t respond well to diuretics and require different management, often involving specialized compression, manual lymphatic drainage, or other targeted therapies.
One nuance worth knowing: early-stage lymphedema can actually pit, because fluid hasn’t yet caused the tissue thickening that makes later-stage lymphedema firm and non-pitting. So pitting doesn’t automatically rule lymphedema out.
Warning Signs That Need Immediate Attention
Most edema is uncomfortable but not dangerous. Certain combinations, however, signal an emergency. Leg swelling paired with chest pain, difficulty breathing, shortness of breath when lying down, dizziness, or coughing up blood requires a 911 call. These can indicate a blood clot that has traveled to the lungs or acute heart failure.
Swelling that appears suddenly in one leg, especially if it’s painful, warm, or the skin looks pale or reddish, raises concern for a deep vein thrombosis (a blood clot in a deep vein). This needs urgent evaluation, typically with an ultrasound, which has about 95 percent sensitivity for detecting clots in the upper leg veins. Swelling in both legs that develops gradually is less likely to be a clot but still warrants a medical evaluation if it persists, since it may point to heart, kidney, or liver issues that benefit from early treatment.
