Swollen ankles usually improve with a combination of elevation, movement, and addressing whatever is causing fluid to pool in your lower legs. For most people, the swelling comes from sitting or standing too long, eating too much salt, or a medication side effect. Simple home strategies can make a noticeable difference within hours, but the approach that works best depends on whether the swelling is in one ankle or both, and whether it’s a new or recurring problem.
Elevate and Ice for Quick Relief
The fastest way to reduce ankle swelling is to lie down and prop your feet above heart level. This lets gravity work in your favor, pulling trapped fluid back toward your core. Stack pillows under your calves or rest your legs on the arm of a couch. Even 20 to 30 minutes in this position can visibly reduce puffiness, and doing it several times a day keeps swelling from building back up.
If the swelling came on after an injury or a long day on your feet, ice helps too. Wrap a cold pack in a thin towel and apply it for 10 to 20 minutes at a time, once every hour or two. The barrier between the ice and your skin is important to avoid cold burns. Combining elevation with icing gives you both the fluid-draining benefit of gravity and the vessel-constricting effect of cold.
Use Your Calf Muscles as a Pump
Your calf muscles act like a second heart. When they contract and relax, they squeeze the veins in your lower legs and push blood back up toward your chest. If you sit at a desk all day or spend long stretches on a couch, that pump barely activates, and fluid accumulates around your ankles.
The simplest fix is ankle pumps: point your toes down, then pull them back up toward your shin. Repeat this 30 times every one to two hours while you’re awake. It takes about a minute and you can do it at your desk, on the couch, or in bed. For a deeper effect, try calf stretches by looping a towel around the ball of your foot with your leg extended, gently pulling until you feel the stretch, and holding for 60 seconds. Three to five repetitions daily helps keep the muscle supple and the pump working efficiently.
Walking is the most natural version of this. Even a five-minute walk every hour during a sedentary day activates the calf pump repeatedly and prevents fluid from settling.
Cut Back on Sodium
Salt makes your body hold onto water, and that extra fluid tends to collect in the lowest points: your feet and ankles. Keeping sodium intake around 2,000 mg per day is a practical target for reducing fluid retention. For reference, a single fast-food meal can easily contain 1,500 mg or more.
The biggest sources of hidden sodium are processed foods, canned soups, deli meats, soy sauce, and restaurant meals. Cooking at home with fresh ingredients gives you the most control. You don’t need to eliminate salt entirely. Just reading labels and choosing lower-sodium versions of staples like bread, cheese, and canned goods can cut your intake significantly within a week, and you may notice less puffiness in that same timeframe.
Try Compression Socks
Compression socks apply graduated pressure to your lower legs, tightest at the ankle and gradually loosening toward the knee. This steady squeeze prevents fluid from pooling and supports the calf pump in moving blood upward.
For mild, occasional swelling, socks in the 15 to 20 mmHg range (listed on the packaging) are enough. If you have chronic swelling from vein problems or a condition your doctor has identified, 30 to 40 mmHg provides firmer support. Put them on first thing in the morning before swelling has a chance to build up. Pulling them on over already-swollen ankles is harder and less effective.
Check Whether a Medication Is the Cause
Certain blood pressure medications, particularly calcium channel blockers like amlodipine and nifedipine, are well-known for causing ankle swelling. The effect is dose-related: at higher doses, the incidence of ankle swelling can exceed 80% in long-term users. Other common culprits include anti-inflammatory painkillers, hormone therapies (including birth control pills and hormone replacement), and some diabetes medications.
If your swelling started or worsened after beginning a new prescription, that connection is worth raising with whoever prescribed it. Stopping or switching medications on your own can be risky, but your prescriber may be able to adjust the dose, swap to a different drug in the same class, or add a companion medication that counteracts the swelling.
Magnesium for Hormonal Fluid Retention
If your ankle swelling tends to flare before your period, magnesium supplements may help. A study published in the Journal of Women’s Health found that 200 mg of magnesium daily for two menstrual cycles significantly reduced premenstrual symptoms of fluid retention, including swelling of the extremities and bloating, compared to a placebo. The benefit became more pronounced in the second cycle, so it’s worth giving it at least two months before deciding if it’s working for you.
One Ankle vs. Both: What the Pattern Tells You
Whether the swelling is in one ankle or both is one of the most important clues to its cause, and it changes how urgently you should act.
Swelling in One Ankle
When only one ankle swells, the cause is almost always local: an injury, an infection, or a problem with blood flow in that specific leg. About 40% of cases of sudden one-sided leg swelling turn out to be a muscle strain, tear, or twisting injury. But the concern doctors prioritize is a blood clot in the deep veins, known as DVT. Warning signs of a clot include leg pain or cramping (often starting in the calf), skin that turns red or purple, and warmth in the affected leg. If you have those symptoms, especially after recent surgery, a long flight, or a period of immobility, get medical attention quickly.
Chronic swelling in just one leg most commonly points to vein problems in that leg, particularly if you notice skin darkening around the ankle or have a history of blood clots. Less commonly, it results from lymphatic damage after surgery or radiation, or from a cyst behind the knee that disrupts drainage.
Swelling in Both Ankles
Symmetrical swelling in both ankles usually reflects something systemic: too much salt, prolonged sitting or standing, a medication side effect, pregnancy, or a heart, kidney, or liver condition that causes the body to retain fluid overall. Heart failure is one of the more common medical causes of worsening bilateral swelling. If the puffiness has been gradually increasing over weeks and you’re also short of breath or unusually tired, those symptoms together warrant a medical evaluation.
Swollen Ankles During Pregnancy
Some degree of ankle swelling is normal in pregnancy, especially in the third trimester. Your blood volume increases significantly, your growing uterus puts pressure on the veins that return blood from your legs, and hormonal shifts cause your tissues to hold more fluid. Elevating your feet, wearing compression socks, and staying active all help.
What isn’t normal is sudden, severe swelling, particularly in the face and hands along with the ankles, accompanied by headaches, vision changes, or upper abdominal pain. That combination can signal preeclampsia, a serious pregnancy complication involving high blood pressure. Sudden swelling that looks different from your baseline puffiness deserves a same-day call to your provider.
Red Flags That Need Immediate Attention
Most ankle swelling is uncomfortable but not dangerous. A few scenarios, however, need urgent care:
- One leg suddenly swollen, painful, and warm: this pattern suggests a possible blood clot, especially with skin color changes in the affected leg.
- Swelling plus shortness of breath: if a clot breaks loose and travels to the lungs, it causes sudden difficulty breathing, chest pain that worsens with deep breaths, a racing pulse, or coughing up blood. This is a medical emergency.
- Rapidly worsening swelling with weight gain over days: gaining several pounds in a few days from fluid, along with increasing ankle swelling and breathlessness, can indicate a heart or kidney problem that’s decompensating.
