A sprained wrist typically causes pain, swelling, and tenderness around the joint, especially when you try to move or twist it. The telltale sign is that the pain gets worse when you grip something or put pressure through your palm, like pushing yourself up from a chair. But because a sprain can look and feel a lot like a fracture, knowing what to watch for helps you figure out whether you need to rest it at home or get it checked out.
What a Sprained Wrist Feels Like
A wrist sprain happens when one or more ligaments in the wrist get stretched or torn, usually from a fall onto an outstretched hand. The most common scenario: you trip, catch yourself with your palm, and your wrist bends too far back. That sudden force damages the tough bands of tissue that hold the wrist bones together.
The symptoms you’ll notice depend on how badly the ligament is injured, but most people experience some combination of the following:
- Pain that increases with movement. Twisting a doorknob, gripping a cup, or pushing yourself up from a flat surface all tend to make it worse. Milder sprains may only hurt during these motions, while more severe ones hurt even at rest.
- Swelling around the wrist joint. This usually appears within hours of the injury and can spread across the top or sides of the wrist.
- Tenderness to the touch. Pressing on the soft tissue around the wrist, particularly the top (dorsal) and thumb side, often produces sharp tenderness.
- Bruising. Discoloration may show up within a day or two as damaged blood vessels leak beneath the skin.
- Weakness or instability. The wrist may feel like it can’t support weight, and your grip strength can drop noticeably.
- A popping sensation at the time of injury. Some people hear or feel a pop when the ligament tears. This is more common with moderate to severe sprains.
One of the more commonly injured ligaments connects two small bones in the center of the wrist (the scaphoid and lunate). When this particular ligament is damaged, the pain tends to concentrate on the top of the wrist closer to the thumb side. You might also notice clicking or catching when you rotate your wrist, and pushing up from a flat surface (like doing a push-up) becomes especially painful.
Mild, Moderate, and Severe Sprains
Wrist sprains are graded on a three-level scale based on how much damage the ligament sustained. Understanding which grade you’re dealing with helps explain why some wrist sprains feel like minor nuisances while others make the joint nearly unusable.
Grade 1 (mild): The ligament is stretched but not actually torn. You’ll have mild pain, some swelling, and tenderness, but you can still move your wrist reasonably well. These typically heal with rest within a couple of weeks.
Grade 2 (moderate): The ligament is partially torn. Pain is more noticeable and may persist even when you’re not using the wrist. Swelling and bruising are more pronounced, and using the wrist for everyday tasks becomes difficult. These injuries often need several weeks of immobilization in a splint or brace.
Grade 3 (severe): The ligament is completely torn or pulled away from the bone. Pain is intense, the wrist is significantly swollen, and moving the joint is extremely difficult. A grade 3 sprain can make the wrist feel unstable, as though the bones are shifting under the skin. These injuries sometimes require surgical repair, particularly if the ligament damage leads to ongoing instability. Left untreated, a severe ligament tear can cause the wrist bones to shift out of alignment over time, eventually leading to chronic pain and arthritis.
How to Tell It Apart From a Fracture
This is the tricky part. A wrist sprain and a wrist fracture can produce nearly identical symptoms: pain, swelling, bruising, and difficulty moving the joint. There’s no reliable way to rule out a fracture at home just by pressing on your wrist or testing your range of motion.
One fracture that mimics a sprain especially well involves the scaphoid, a small bone near the base of your thumb. Scaphoid fractures are notorious for feeling like “just a sprain” because the pain is often moderate, the swelling is subtle, and you can still move your wrist. Swelling in the small hollow between the tendons on the thumb side of your wrist (sometimes called the anatomical snuffbox) increases the likelihood of a scaphoid fracture, but even that finding isn’t definitive on its own.
A few patterns suggest a fracture rather than a sprain: pain that is very localized to one bony spot rather than spread across the soft tissue, a visible deformity or crookedness in the wrist, and pain that doesn’t improve at all after a few days of rest and icing. But the honest answer is that clinical examination alone can’t reliably distinguish the two. If your pain is significant or isn’t improving, imaging is the only way to know for sure.
What Happens at the Doctor’s Office
Your doctor will start by examining the wrist, pressing on specific areas, moving the joint through its range of motion, and testing your grip strength. They’ll also check for signs of instability by gently shifting the wrist bones to see if the ligaments are holding them in place.
An X-ray is usually the first imaging step. It’s fast and widely available, and while it won’t show ligament damage directly, it can rule out a fracture and reveal whether the wrist bones have shifted out of their normal alignment, which signals a significant ligament tear. If the X-ray looks normal but your symptoms suggest more than a mild sprain, an MRI may be ordered. MRIs provide much better detail of soft tissues like ligaments, tendons, and cartilage, making them the best tool for confirming the grade of a sprain and spotting injuries that X-rays miss.
What Recovery Looks Like
For mild sprains, home treatment is usually enough. Resting the wrist, applying ice for 15 to 20 minutes several times a day, using a compression wrap, and keeping the hand elevated all help control swelling and pain in the first few days. A simple wrist brace or splint can keep the joint stable while the ligament heals. Most grade 1 sprains feel significantly better within one to two weeks, though full recovery may take a bit longer if you’re using the wrist heavily for work or sports.
Moderate sprains generally require a more structured approach. Your wrist may be immobilized in a splint for several weeks, and you might be referred for physical therapy afterward to rebuild strength and flexibility. Expect four to six weeks before you’re back to normal activities, sometimes longer.
Severe sprains with a complete ligament tear present the longest recovery. If surgery is needed to reattach the ligament, you’ll likely be in a cast for several weeks followed by a rehabilitation program. Full recovery can take three months or more. Even without surgery, a grade 3 sprain needs careful follow-up because an untreated torn ligament can lead to reduced grip strength and chronic wrist instability that worsens over time.
Signs You Shouldn’t Wait It Out
Not every tweaked wrist needs medical attention, but certain symptoms suggest something more serious than a mild sprain. Get your wrist evaluated if the pain is severe or doesn’t start improving within a few days, if the swelling keeps getting worse rather than better, if you notice numbness or tingling in your fingers, or if the wrist feels unstable when you try to use it. Any visible deformity warrants an immediate visit. And if you fell hard onto your hand and there’s pain near the base of your thumb, it’s worth getting an X-ray to check for a scaphoid fracture, since these small fractures heal poorly when they’re missed.
