How Do You Know If You Have Blood in Your Stool?

Blood in your stool can look bright red, dark maroon, or black and tarry, depending on where the bleeding starts in your digestive tract. Sometimes it’s invisible entirely, detectable only through a lab test. Knowing what to look for helps you figure out whether that unusual color in the toilet is actually blood or something harmless like last night’s beet salad.

What Blood in Stool Actually Looks Like

Bleeding from lower in the digestive tract, such as the colon or rectum, typically produces bright red blood. You might see it on the toilet paper, streaked across the surface of your stool, or dripping into the bowl. This is the most recognizable form and often comes from common issues like hemorrhoids or small tears near the anus.

Bleeding that starts higher up, in the stomach or upper intestine, looks very different. As blood travels through the digestive tract and gets broken down by enzymes, it turns dark. The result is black, tarry, sticky stool with a distinctly foul smell. This happens because hemoglobin, the protein that makes blood red, darkens as it’s digested. If your stool looks like this and you haven’t taken anything that could explain it, that’s a sign of upper gastrointestinal bleeding.

A third category is blood you simply can’t see. Called occult blood, it exists in amounts too small to change the color of your stool. You won’t notice any visual difference, but a screening test can pick it up. This is one reason routine screening matters: the bleeding can be completely silent.

Foods and Medications That Mimic Blood

Before assuming the worst, consider what you’ve eaten or taken recently. Beets and foods with red coloring can make stool appear reddish, closely mimicking fresh blood. On the dark end, black licorice, blueberries, iron supplements, activated charcoal, and bismuth-based products like Pepto-Bismol all turn stool black without any bleeding involved.

A simple way to check: if the color change lines up with something you consumed in the last day or two and goes away on its own, it’s likely harmless. If it persists or you can’t connect it to anything you ate, it’s worth investigating further.

Indirect Signs You Might Be Bleeding

When blood loss is slow and ongoing, you may never see anything unusual in the toilet. Instead, the clues show up elsewhere. Unexplained fatigue, shortness of breath during normal activities, and pale skin can all point to anemia caused by chronic low-level bleeding in the digestive tract. Some people are diagnosed with iron-deficiency anemia before anyone suspects gastrointestinal bleeding as the source.

Severe or sudden bleeding produces more dramatic symptoms: feeling lightheaded or faint, dizziness, a fast heart rate, cold hands and feet, sweating, or pale skin. In extreme cases, heavy blood loss can cause confusion or loss of consciousness. These are signs of shock and need immediate emergency care.

Common Causes, From Minor to Serious

The most frequent culprits behind visible rectal bleeding are hemorrhoids and anal fissures, small tears in the tissue around the anus. These tend to produce bright red blood, often noticed on toilet paper, and they’re rarely dangerous. Hemorrhoids are especially common during pregnancy, after straining, or with chronic constipation.

Moving up the severity scale, other possible causes include colon polyps (small growths that can become cancerous over time), diverticulosis (small pouches that form in the intestinal wall), and inflammatory bowel diseases like Crohn’s disease and ulcerative colitis. Infections, reduced blood flow to the colon, and rectal ulcers can also cause bleeding. Colon and rectal cancer are less common causes but are the reason screening exists. The point isn’t to panic but to recognize that the cause can’t be determined by appearance alone.

How Blood in Stool Is Detected

If you can see blood, that’s already detection. But for hidden bleeding, there are specific tests. The two main stool-based options work differently and have different requirements.

A fecal immunochemical test (FIT) uses antibodies to detect tiny amounts of hemoglobin in your stool. It’s designed to catch bleeding from the lower digestive tract. One practical advantage: no dietary restrictions before the test. Foods and medications don’t interfere with the results.

The older guaiac-based test (gFOBT) can detect bleeding from both the upper and lower digestive tract, but it’s more finicky. You may need to avoid ibuprofen, naproxen, and aspirin for seven days beforehand, since these can cause minor bleeding that triggers a false positive. Red meat needs to be off the menu for three days before testing because components of animal blood in the meat can skew results. High doses of vitamin C can actually mask real blood, producing a false negative.

A newer option, the multitarget stool DNA test (sold as Cologuard), looks for both blood and abnormal DNA shed by precancerous or cancerous cells. It’s more comprehensive but is used on a different screening schedule than annual stool tests.

All of these can miss polyps and some cancers, and all can produce false positives. They’re screening tools, not definitive diagnoses.

What Happens After a Positive Result

If any stool test comes back positive, or if you report visible blood to your doctor, the next step is typically a colonoscopy. During this procedure, a flexible tube with a camera is guided through the entire large intestine and rectum, allowing a direct look at the lining. If polyps or suspicious tissue are found, they can often be removed or biopsied during the same procedure. Your doctor will also review your medical history, ask about previous bleeding episodes, and do a physical exam to narrow down the likely source.

A positive stool test doesn’t mean cancer. It means blood was detected, and the cause needs to be identified. In many cases, the colonoscopy reveals something benign like polyps or hemorrhoids.

Screening Guidelines by Age

Most people should begin routine colorectal cancer screening soon after turning 45, according to current CDC recommendations. Screening catches problems before symptoms appear, which is especially important because early-stage colorectal cancer often causes no noticeable bleeding at all. If you have a family history of colorectal cancer or other risk factors, your doctor may recommend starting earlier.

If you’re under 45 and notice blood in your stool, don’t assume your age makes it irrelevant. Colorectal cancer rates in younger adults have been rising, and visible bleeding at any age deserves attention if it’s persistent, unexplained, or accompanied by changes in bowel habits, abdominal pain, or unintentional weight loss.
