How Do You Know If You Have High Cholesterol?

High cholesterol has no symptoms you can feel. There’s no headache, no fatigue, no warning sign that tells you something is off. The only reliable way to know if you have high cholesterol is a blood test called a lipid panel. That’s what makes it dangerous: cholesterol can quietly build up in your artery walls for years, narrowing blood flow to your heart, brain, and kidneys, without giving you any reason to check.

Why High Cholesterol Has No Symptoms

Cholesterol is a waxy substance your body uses to build cells and make hormones. When there’s more of it circulating than your body needs, the excess gets deposited inside artery walls. This buildup, called plaque, develops gradually over decades. It doesn’t cause pain or discomfort as it forms. Most people discover they have high cholesterol either through routine screening or after a cardiovascular event like a heart attack or stroke, which is why regular testing matters so much.

Rare Physical Signs in Severe Cases

There is one exception to the “no symptoms” rule, and it applies to people with extremely high cholesterol, usually from a genetic condition called familial hypercholesterolemia. These individuals can develop visible deposits of cholesterol under the skin called xanthomas. They appear as yellowish bumps or nodules, most commonly on the Achilles tendons, the knuckles, elbows, and knees. By their 30s, more than 60% of people with untreated familial hypercholesterolemia have tendon xanthomas.

Other physical clues include xanthelasmas (flat yellowish patches around the eyelids) and corneal arcus (a white or gray ring around the edge of the iris). Corneal arcus in younger adults is a red flag, though it can appear normally in older people. These signs are uncommon in the general population. If you don’t have them, it doesn’t mean your cholesterol is fine. If you do, it’s worth getting tested promptly.

How Cholesterol Testing Works

A lipid panel is a simple blood draw, typically done at your doctor’s office or a lab. It measures four things:

  • LDL cholesterol, the “bad” kind that drives plaque buildup in your arteries
  • HDL cholesterol, the “good” kind that helps remove LDL from your bloodstream
  • Total cholesterol, a combined measure of all cholesterol types
  • Triglycerides, a type of fat in your blood that also contributes to artery damage at high levels

You may not need to fast beforehand. Major guidelines from the American College of Cardiology, the American Heart Association, and others now consider a non-fasting test equally acceptable for screening. Total cholesterol and HDL readings barely change whether you’ve eaten or not. Your doctor may still ask you to fast for 9 to 12 hours if your triglycerides are known to be elevated, since triglyceride levels are more sensitive to recent meals.

What the Numbers Mean

Optimal LDL cholesterol is around 100 mg/dL. The higher it climbs above that, the more cholesterol is available to lodge in your artery walls. Your doctor will interpret your LDL number alongside your other risk factors (age, blood pressure, smoking status, diabetes, family history) rather than treating it in isolation.

For triglycerides, the ranges are more clearly defined. Below 150 mg/dL is considered healthy. Between 150 and 199 is borderline high. Levels from 200 to 499 are high, and anything at 500 or above is very high and carries serious risk of complications beyond heart disease, including inflammation of the pancreas.

HDL works in the opposite direction from LDL: higher is better. One useful way to think about your overall picture is the ratio of total cholesterol to HDL. If your total cholesterol is 200 and your HDL is 50, your ratio is 4 to 1. Lower ratios indicate lower risk. Many clinicians now also look at non-HDL cholesterol (your total cholesterol minus your HDL), which captures all the artery-damaging types in a single number and may predict heart disease risk better than LDL alone.

When and How Often to Get Tested

The American Heart Association recommends cholesterol screening every five years starting at age 19 for most adults. Children should be screened once between ages 9 and 11 to catch genetic conditions early. If you have risk factors like a family history of heart disease, diabetes, obesity, or high blood pressure, your doctor will likely want to check more frequently.

People already on cholesterol-lowering treatment typically get tested more often to see whether their levels are responding. The five-year interval is a baseline for low-risk adults, not a ceiling. If you’ve never had your cholesterol checked and you’re over 20, there’s no reason to wait for your next scheduled physical. Any routine blood draw can include a lipid panel.

What Drives Cholesterol Up

Your cholesterol level is shaped by a combination of genetics and lifestyle. Some people inherit a tendency to produce more LDL or clear it from the bloodstream more slowly. Diets high in saturated fat (red meat, full-fat dairy, fried foods) raise LDL. Trans fats, still found in some processed foods, are especially harmful because they raise LDL while lowering HDL at the same time.

Physical inactivity, excess body weight, and smoking all push your numbers in the wrong direction. Smoking doesn’t directly raise LDL, but it lowers HDL, which means less cholesterol gets cleared from your blood. Age plays a role too: cholesterol levels tend to rise naturally as you get older, particularly after menopause in women.

Some medical conditions also contribute. Diabetes, hypothyroidism, and kidney disease can all elevate cholesterol or triglycerides independently of diet and exercise.

What Happens if Your Numbers Are High

If your lipid panel comes back elevated, the next step depends on how high the numbers are and what other risk factors you have. For moderately elevated cholesterol in otherwise healthy people, the first approach is usually lifestyle changes: shifting toward a diet rich in fiber, healthy fats, fruits, and vegetables, getting regular aerobic exercise, losing weight if needed, and quitting smoking. These changes can meaningfully lower LDL over a period of weeks to months.

For people at higher cardiovascular risk, or those whose LDL stays stubbornly high despite lifestyle changes, medication becomes part of the plan. The goal isn’t just a better number on paper. It’s reducing the ongoing damage to your arteries that you can’t see or feel. Since high cholesterol gives you no warning, the only way to know it’s improving is to retest. That’s also why getting screened in the first place is the single most important step you can take.
