How Do You Know If You Have Inflammation: Symptoms & Tests

Inflammation shows up in two very different ways depending on whether it’s short-term or ongoing. Acute inflammation is hard to miss: you get redness, swelling, heat, pain, and reduced movement at the site of an injury or infection. Chronic inflammation is subtler and can simmer for months or years, producing vague symptoms like fatigue, joint stiffness, digestive problems, and mood changes that are easy to dismiss as stress or aging.

The Five Signs of Acute Inflammation

When your body fights an infection or heals a wound, it triggers a rapid inflammatory response with five hallmark signs: redness, heat, swelling, pain, and loss of function. Redness and warmth happen because blood flow increases to the affected area. Swelling results from fluid leaking into the surrounding tissue. Pain comes from chemical signals that activate nerve endings. And loss of function is the combined effect of all four, making it harder to use the injured area normally.

This type of inflammation is usually obvious and localized. A sprained ankle, a bee sting, or a sore throat all follow this pattern. It’s your immune system doing its job, and it typically resolves within days to a couple of weeks as the tissue heals.

Chronic Inflammation Symptoms

Chronic inflammation is a different story. It doesn’t always produce visible swelling or redness. Instead, the immune system stays mildly activated over long periods, and the symptoms are often widespread and nonspecific. Common signs include:

  • Persistent fatigue or insomnia that rest doesn’t fix
  • Joint pain or stiffness, especially in the morning or after sitting for a while
  • Digestive issues like diarrhea, constipation, or acid reflux
  • Skin rashes or mouth sores
  • Abdominal or chest pain
  • Unexplained weight changes in either direction
  • Frequent infections, suggesting an immune system that’s both overactive and less effective
  • Depression, anxiety, or other mood shifts
  • Low-grade fever

What makes chronic inflammation tricky is that none of these symptoms alone points directly to it. Fatigue and joint stiffness could be a dozen things. The pattern matters more than any single symptom. If several of these overlap and persist for weeks without a clear explanation, inflammation is worth investigating.

How Inflammation Affects Your Brain

One of the less obvious signs of inflammation is cognitive and emotional change. Inflammatory molecules produced elsewhere in the body can cross into the brain and disrupt normal signaling between nerve cells. This can show up as brain fog, low motivation, depressed mood, or difficulty concentrating.

The connection between inflammation and depression is well documented. In studies of patients receiving a specific immune-stimulating treatment for hepatitis C, roughly 30% developed major depression within three months. Brain imaging in those patients showed reduced activity in reward-processing areas, which correlated with low motivation, fatigue, and depressed mood. This doesn’t mean all depression is caused by inflammation, but if mood changes appear alongside other inflammatory symptoms like joint pain or digestive problems, the two may be connected.

Physical Clues You Can Check at Home

Some signs of inflammation are visible or measurable without a lab test. Joint stiffness that lasts longer than 30 minutes after waking, particularly if it affects both sides of your body symmetrically, is a classic marker of inflammatory joint conditions like rheumatoid arthritis. Swelling that interferes with everyday tasks (making a fist, buttoning clothes, bending your knees) is another red flag. Firm lumps under the skin near the hands or elbows, called rheumatoid nodules, can also develop.

Your waistline offers another clue. Visceral fat, the fat stored deep around your organs, is a major driver of chronic inflammation. You can estimate your risk with a tape measure: a waist circumference above 35 inches for women or 40 inches for men is associated with higher levels of internal inflammation. Another useful check is your waist-to-height ratio. If your waist measurement is more than half your height, research links that to increased risk of metabolic and cardiovascular inflammation.

Blood Tests That Measure Inflammation

If you suspect chronic inflammation, blood work is the most direct way to confirm it. Two tests are used most often.

C-Reactive Protein (CRP)

CRP is a protein your liver produces in response to inflammation. A normal level is below 0.9 mg/dL. Results between 1.0 and 10.0 mg/dL indicate moderate inflammation, and anything above 10 mg/dL is considered markedly elevated. Levels above 50 mg/dL suggest severe inflammation, often from an acute infection or a major flare of an inflammatory condition.

There’s also a high-sensitivity version of the test (hs-CRP) that detects very low levels of inflammation. This version is mainly used to assess cardiovascular risk. An hs-CRP below 2.0 mg/L is considered lower risk for heart disease, while 2.0 mg/L or above signals higher risk. The standard CRP and the high-sensitivity version measure the same protein, just at different scales. Your doctor chooses which one to order based on what they’re looking for.

Erythrocyte Sedimentation Rate (ESR)

The ESR test measures how quickly red blood cells settle to the bottom of a tube over one hour. When inflammation is present, certain proteins in the blood cause red blood cells to clump together and sink faster. Normal ranges vary by age and sex, so the result is interpreted in context. ESR is less specific than CRP, meaning it can be elevated for many reasons, but it’s useful as a screening tool and for tracking inflammation over time.

Fibrinogen

Fibrinogen is a clotting protein that also rises during inflammation. Normal adult levels fall between 200 and 400 mg/dL. Elevated fibrinogen shows up in inflammatory conditions, infections, tissue damage, and some cancers. It’s not ordered as often as CRP or ESR, but it adds another data point when the picture is unclear.

What Elevated Markers Can Mean

A single elevated CRP or ESR result doesn’t diagnose a specific condition. These tests confirm that inflammation exists somewhere in the body, but not where or why. The next step is usually ruling out infections and other straightforward causes before considering autoimmune or autoinflammatory conditions.

Persistently elevated markers without an obvious infection can point toward conditions driven by immune system dysfunction. Some of these, like rheumatoid arthritis or inflammatory bowel disease, have well-established diagnostic criteria. Others are harder to pin down. Some patients have recurring fevers and elevated inflammatory markers but don’t fit neatly into any recognized diagnosis. In those cases, labels like “unclassified systemic autoinflammatory disease” are sometimes applied while doctors continue monitoring and managing symptoms.

The important thing to understand is that inflammation is a signal, not a diagnosis. It tells you something is driving an immune response. Identifying the source, whether it’s excess visceral fat, an autoimmune process, a chronic infection, or something else, is what determines the right course of action. If your symptoms match the patterns described above and they’ve lingered for more than a few weeks, blood work is a reasonable next step to make the invisible visible.
