The most telling sign of plantar fasciitis is a sharp or aching pain in your heel or along the bottom of your foot that hits hardest with your first steps in the morning. That “first step” pain, which typically fades after a few minutes of walking, is the hallmark that separates plantar fasciitis from most other causes of heel pain. If that pattern sounds familiar, there’s a good chance you’re dealing with it.
The Classic Pain Pattern
Plantar fasciitis follows a distinctive cycle that revolves around rest and activity. The pain is worst when you stand up after sleeping or sitting for a while. Those first few steps can feel like you’re walking on a bruise or stepping on a sharp stone. Then, as you move around for a few minutes, the pain eases. This happens because the thick band of tissue running along the bottom of your foot (the plantar fascia) tightens up during rest, and those initial steps stretch it back out.
The pain usually centers at the inside edge of your heel, right where the fascia attaches to the heel bone, though it can radiate along the arch toward the ball of the foot. Many people also notice a flare-up after long periods of standing, after exercise (not usually during it), or at the end of a long day on their feet. If your heel pain is worst during activity rather than after it, that’s worth noting for your doctor, since it can point to a different problem.
What Raises Your Risk
Plantar fasciitis tends to show up when the fascia is repeatedly overloaded. Several factors make that more likely:
- Carrying extra weight. Even gaining a modest amount of weight shifts how force is distributed across your feet with every step. The added load increases stress on the fascia, especially during routine activities like walking or standing.
- Being on your feet for long stretches. Jobs that involve standing on hard surfaces for hours, like retail, nursing, or warehouse work, are strongly linked to plantar fasciitis.
- Ramping up activity too quickly. Runners, walkers, and anyone who suddenly increases their training volume or switches to less supportive shoes are common candidates.
- Tight calves. When the muscles in the back of your lower leg are stiff, they pull on the heel bone and put extra tension on the fascia with every step.
- Age. Plantar fasciitis is most common between ages 40 and 60, partly because the fascia loses elasticity over time and the natural fat pad under the heel thins out.
- Foot structure. Very flat feet or very high arches can both change how weight is distributed and place unusual strain on the fascia.
Most people with plantar fasciitis have more than one of these factors working together. A 45-year-old who recently gained weight and started a walking program, for instance, checks several boxes at once.
A Simple Self-Check
There’s a version of a clinical test you can try at home to get a better sense of whether the plantar fascia is the source of your pain. While seated, grab the toes on your affected foot and gently pull them back toward your shin. This stretches the fascia along the bottom of the foot. If that motion reproduces your familiar heel pain, that’s a positive sign that the fascia is involved. Clinicians call this the Windlass test, and it’s one of the main physical assessments used to diagnose plantar fasciitis in an office visit.
The test is more telling when done while standing. You can replicate this by standing with the ball of your foot on the edge of a stair step and having someone gently push your big toe upward. If it triggers your typical pain, that’s a strong signal. The standing version is considered highly specific for plantar fasciitis, meaning a positive result is very unlikely to be caused by something else.
Conditions That Feel Similar
Several other problems can cause heel pain, and they sometimes overlap with plantar fasciitis or get mistaken for it.
Heel fat pad atrophy causes a deep, bruise-like ache directly under the center of the heel rather than at the inner edge. It tends to hurt most with direct pressure, like walking on hard floors barefoot, and doesn’t follow the classic “worst in the morning, better after a few minutes” pattern as reliably. As people age, the fat pad under the heel naturally thins, so this condition becomes more common in older adults. It can also coexist with plantar fasciitis, which makes sorting out the two trickier.
Nerve entrapment in the heel area can cause burning, tingling, or numbness alongside the pain. One common culprit involves a small nerve near the inside of the heel. The pain from nerve issues doesn’t always follow the rest-then-walk cycle of plantar fasciitis and may worsen with specific positions or at night.
Stress fractures of the heel bone produce pain that gets worse with activity and doesn’t improve with walking. If your pain steadily increases the longer you’re on your feet, rather than easing after the first few minutes, a stress fracture is worth considering, especially if you recently increased your exercise intensity.
How It’s Diagnosed
Most of the time, a doctor or physical therapist can diagnose plantar fasciitis based on your description of the pain pattern and a physical exam. They’ll press along the bottom of your foot to locate the tender spot and may perform the toe extension test described above. Imaging like X-rays or ultrasound isn’t usually needed for a straightforward case, though it can help rule out a stress fracture or confirm thickening of the fascia if there’s any doubt.
Heel spurs sometimes show up on X-rays in people with heel pain, but they’re common in people without any pain at all. A heel spur on its own doesn’t mean you have plantar fasciitis, and removing it rarely solves the problem. The spur is a consequence of long-term tension on the heel bone, not the cause of the pain.
What Recovery Looks Like
Plantar fasciitis responds well to conservative treatment in most cases, but the timeline varies a lot. Mild, recent-onset cases (under six weeks) often improve with basic changes: supportive shoes, calf stretches, rolling the arch over a frozen water bottle, and reducing the activity that triggered it. Most people who take these steps start feeling meaningful improvement within 4 to 12 weeks.
Cases that have been lingering for more than three months typically need a more structured approach. Physical therapy focused on stretching and strengthening, custom or over-the-counter arch supports, and night splints that keep the fascia gently stretched while you sleep are all common next steps. Cortisone injections can provide short-term relief for stubborn pain but don’t address the underlying problem.
The biggest mistake people make is ignoring it and hoping it resolves on its own. Left untreated, plantar fasciitis can drag on for months or years. A long-term study that followed patients for up to 15 years found that in severe cases, half still had symptoms after 5 years, and nearly half were dealing with pain after 10 years. Early, consistent treatment makes a significant difference in avoiding that outcome.
