The most common signs of small intestinal bacterial overgrowth (SIBO) are persistent bloating, excessive gas, and abdominal cramps, especially after eating. About two-thirds of people with confirmed SIBO report these symptoms. But because they overlap heavily with irritable bowel syndrome and other gut conditions, recognizing SIBO requires looking at the full picture: your symptoms, your risk factors, and ultimately a breath test or other diagnostic workup.
Symptoms That Point Toward SIBO
SIBO happens when bacteria that normally live in the large intestine colonize the small intestine, where they ferment food too early in the digestive process. That fermentation produces gas, which drives most of the hallmark symptoms: abdominal distension, a persistent feeling of fullness, flatulence, and diffuse cramping. Bowel habits usually change too. Most people develop diarrhea, though some experience constipation or an unpredictable alternation between the two.
These symptoms tend to flare after meals, particularly meals high in carbohydrates or fiber, because those are the foods the misplaced bacteria feed on most aggressively. Many people also report chronic fatigue and difficulty concentrating, which likely stem from the inflammatory and nutritional consequences of the overgrowth rather than the gas itself.
In more advanced cases, the bacteria interfere with how your body absorbs nutrients. Fatty, foul-smelling stools (a sign your body isn’t digesting fat properly) can develop, along with unintentional weight loss. Vitamin B12 deficiency shows up in as many as half to three-quarters of confirmed SIBO patients, because the bacteria consume B12 and convert it into forms your body can’t use. Deficiencies in vitamins A, D, and E can follow as fat absorption worsens. Folate levels, interestingly, tend to stay normal or even run high, because the overgrown bacteria actually produce folate.
Who Is Most Likely to Develop SIBO
SIBO rarely appears out of nowhere. It almost always develops in people who have an underlying condition or risk factor that disrupts the small intestine’s natural defenses. Understanding whether you have one of these risk factors can help you and your doctor decide whether testing makes sense.
The small intestine normally keeps bacterial populations low through three main mechanisms: stomach acid kills many bacteria before they arrive, muscular contractions sweep residual bacteria downstream, and the valve between the small and large intestine prevents backflow. Anything that weakens one of these defenses raises your risk.
- Slow motility: Conditions like gastroparesis (delayed stomach emptying), intestinal pseudo-obstruction, and hypothyroidism all slow the muscular waves that clear bacteria from the small intestine.
- Low stomach acid: Long-term use of proton pump inhibitors (PPIs) or antacids reduces acid output, giving more bacteria safe passage into the small intestine.
- Structural changes: Abdominal surgery, gastric bypass, small bowel diverticula (small pouches in the intestinal wall), adhesions from prior operations, and radiation damage can all create pockets where bacteria accumulate.
- Medication effects: Frequent antibiotic use and narcotics can both disrupt the normal balance of gut bacteria and slow intestinal movement.
If you have one or more of these risk factors and you’re dealing with chronic bloating, diarrhea, or unexplained weight loss, SIBO becomes a strong possibility worth investigating.
How SIBO Is Diagnosed
There’s no way to confirm SIBO from symptoms alone. The two main diagnostic tools are a breath test and, less commonly, a small intestine fluid sample.
The Breath Test
This is the most widely used test because it’s noninvasive and can be done at a clinic or even at home with a kit. You drink a sugar solution, typically glucose or lactulose, then breathe into collection tubes at regular intervals over two to three hours. The test measures hydrogen and methane in your breath, both of which are produced when bacteria ferment the sugar.
In a healthy digestive system, baseline hydrogen levels sit below 16 parts per million (ppm). For a positive SIBO result, hydrogen needs to rise by at least 20 ppm above your baseline within the first 90 minutes. That timing matters: a rise within 90 minutes means fermentation is happening in the small intestine, not the colon where it would be normal.
Most clinics now measure methane alongside hydrogen. When methane levels are elevated (generally above 10 ppm), the condition is technically called intestinal methanogen overgrowth, or IMO, rather than classic SIBO. The distinction matters because high methane producers tend to have constipation rather than diarrhea, and the organisms responsible (archaea, not bacteria) may respond differently to treatment.
Preparation for the breath test usually involves a restricted diet the day before (plain rice, meat, and limited seasonings), fasting overnight, and avoiding certain medications. Poor preparation can skew results, so following the instructions closely is important.
Small Intestine Aspirate
The most direct way to diagnose SIBO is to collect fluid from the small intestine during an upper endoscopy and culture it in a lab. A bacterial count of 1,000 or more colony-forming units per milliliter is considered diagnostic according to the American College of Gastroenterology. This method is more accurate but also more invasive, more expensive, and not available everywhere. It’s typically reserved for cases where breath test results are unclear or when an endoscopy is being done for another reason.
Signs Your Doctor May Spot
A physical exam alone can’t diagnose SIBO, but it can reveal consequences of long-standing overgrowth. Visible abdominal distension is common. In advanced cases, malabsorption can cause nerve damage from B12 deficiency (tingling or numbness in the hands and feet), muscle spasms from low calcium, or skin changes from selenium deficiency. Significant, unexplained weight loss in someone with chronic digestive symptoms is another red flag.
Blood work can add supporting evidence. Low B12 with normal or high folate is a pattern that’s fairly characteristic of SIBO. Low levels of fat-soluble vitamins (A, D, and E), low albumin (a protein that drops when nutrition is poor), and signs of anemia from B12 deficiency can all push a clinician toward ordering a breath test.
SIBO vs. IBS: Telling Them Apart
Many people searching for SIBO information have already been told they have irritable bowel syndrome. The symptoms overlap almost entirely: bloating, gas, abdominal pain, and irregular bowel habits. Studies estimate that a significant portion of people diagnosed with IBS actually have SIBO as the underlying driver.
A few patterns can help you distinguish between the two. SIBO symptoms tend to be tied closely to eating, worsening predictably 30 to 90 minutes after a meal. Unintentional weight loss and nutrient deficiencies point more strongly toward SIBO than typical IBS. And if you have a known risk factor like prior abdominal surgery, long-term PPI use, or a motility disorder, SIBO should be on the radar. The only definitive way to separate them is through breath testing.
What Happens After a Positive Test
Treatment for SIBO typically involves a course of antibiotics targeting the small intestine, often lasting two weeks. Many people feel noticeably better within the first few days of treatment as gas production drops. Dietary changes, particularly reducing fermentable carbohydrates, often help manage symptoms during and after treatment.
Recurrence is common, especially if the underlying cause (slow motility, structural abnormality, ongoing PPI use) hasn’t been addressed. Some people go through multiple rounds of treatment. Identifying and managing the root cause is just as important as clearing the overgrowth itself.
