The most common signs of miscarriage are vaginal bleeding and cramping that intensify beyond normal spotting. But miscarriage doesn’t always announce itself clearly. Some happen with heavy bleeding and obvious tissue passage, while others produce no symptoms at all and are only discovered at a routine ultrasound. Understanding what to look for, and what doctors use to confirm a loss, can help you make sense of what’s happening in your body.
The Main Signs to Watch For
Bleeding is the most recognizable symptom, but it varies widely. It can start as light spotting and stay that way, or it can escalate to flow that’s heavier than a period. The blood may be brown, pink, or bright red. Some women pass fluid or tissue from the vagina, which can look like blood clots. In very early losses (before six weeks), you may not see anything recognizable beyond clots and a small fluid-filled sac. By eight weeks, the tissue often looks dark red and shiny, sometimes described as resembling liver, and you may be able to identify a small sac containing an embryo about the size of a bean.
Cramping typically accompanies the bleeding. It can feel like intense period cramps or a deep ache in the lower abdomen and back. The combination of worsening cramps with increasing bleeding is the pattern that most clearly suggests a miscarriage is underway. A sudden disappearance of pregnancy symptoms you’d been experiencing, like breast tenderness and nausea, is another signal, though symptoms naturally fluctuate in early pregnancy, so this alone isn’t definitive.
Chemical Pregnancy: A Very Early Loss
A chemical pregnancy is a miscarriage that happens within the first five weeks, before anything is visible on ultrasound. You might get a positive pregnancy test, then get your period about a week later than expected. Or you test positive but a follow-up test a few weeks later comes back negative. The bleeding tends to be heavier than a typical period with more intense cramps. Many chemical pregnancies happen without the person ever realizing they were pregnant, especially if they weren’t actively testing.
Missed Miscarriage: No Obvious Symptoms
Not all miscarriages involve bleeding or pain. In a missed miscarriage, the embryo or fetus has stopped developing, but your body hasn’t begun to expel the tissue. There’s no bleeding, no cramping, and sometimes pregnancy symptoms persist because hormonal changes lag behind. This type is typically discovered during a routine ultrasound when no heartbeat is detected. It can be a particular shock because nothing felt wrong.
A threatened miscarriage is different. You have bleeding, but the cervix remains closed and the pregnancy may still be viable. About half of threatened miscarriages resolve on their own, and the pregnancy continues normally. Until your doctor evaluates you, bleeding in early pregnancy doesn’t automatically mean the pregnancy is lost.
How Doctors Confirm a Miscarriage
Two main tools are used: ultrasound and blood hormone levels.
On ultrasound, doctors look for specific measurements. If an embryo measures at least 5 to 7 millimeters in length but has no heartbeat, that confirms a loss. If there’s a gestational sac but no embryo visible inside it, and the sac measures 21 millimeters or larger, that also confirms a nonviable pregnancy. When measurements fall below these thresholds, your doctor will often ask you to come back in a week or two for a repeat scan rather than making a definitive call, because dating can be off by a few days and it’s critical to avoid a false diagnosis.
Blood tests track levels of the pregnancy hormone hCG. In a healthy early pregnancy, hCG roughly doubles every 48 to 72 hours. A rise of 53% or more over two days confirms a viable pregnancy in about 99% of cases. If levels are barely climbing (for example, going from 120 to 130 over two days) or actively dropping from a previous measurement, that pattern points toward a nonviable pregnancy. Your doctor will typically draw blood on two separate days to compare the trend rather than relying on a single number.
Second Trimester Losses Feel Different
Miscarriages after the first trimester share some symptoms with earlier losses, including bleeding and cramping, but the physical experience is more intense because the pregnancy is further along. After about 20 weeks, most women can feel fetal movement, so a loss of that movement becomes an important warning sign. The cramping may feel more like contractions. Second trimester losses are less common (most miscarriages happen in the first 12 weeks), but they involve more bleeding and a longer physical recovery.
Signs That Need Emergency Attention
Some symptoms during a suspected miscarriage require immediate medical care. Go to an emergency department if you’re soaking through two pads per hour or passing clots the size of a golf ball. Severe abdominal pain, especially if it’s sudden or concentrated on one side, needs urgent evaluation. Shoulder pain is a less obvious red flag: it can signal internal bleeding from an ectopic pregnancy (where the embryo implants outside the uterus, usually in a fallopian tube). Dizziness, fainting, fever, chills, or foul-smelling vaginal discharge also warrant an ER visit.
One-sided pelvic pain with light bleeding is the hallmark pattern of an ectopic pregnancy, which requires different treatment than a standard miscarriage and can become life-threatening if a fallopian tube ruptures. If your pain is sharp and localized to one side, don’t wait to be seen.
What Recovery Looks Like
After the pregnancy tissue has passed (or been removed with a procedure), light spotting or bleeding can continue for up to a month. Your first period typically returns about two weeks after the spotting ends, which works out to roughly two to three months after the miscarriage. If you passed tissue at home, your doctor may want to confirm through ultrasound or blood tests that everything has been expelled completely. An incomplete miscarriage, where some tissue remains, can cause prolonged bleeding or infection and may need medical intervention.
If you do pass tissue at home, placing it in a clean container and bringing it to your doctor can help confirm the diagnosis and, in some cases, allow testing to identify a cause. This isn’t required, but it can provide answers if you want them.
