Understanding the normal pattern helps you recognize when something is off. A steeper decline from morning to night is associated with better physical and mental health.   • Late-night salivary cortisol: You collect a saliva sample between 10 p.m. and midnight, when cortisol should be at its lowest. This test is convenient because you do it at home. It has roughly 92% sensitivity and 89% specificity, meaning it catches most true cases while producing relatively few false alarms. Two separate collections on different nights are recommended.
  • 24-hour urine cortisol: You collect all your urine over a full day in a single container. The normal range is 10 to 100 micrograms per 24 hours. Values above that suggest excess cortisol production. At least two separate collections are recommended because cortisol output can vary day to day.
  • Overnight dexamethasone suppression test: You take a small pill at bedtime that should suppress your cortisol production. A blood draw the next morning checks whether your body responded. If cortisol stays high despite the pill, it suggests your body is overproducing it.
  • Morning blood cortisol: A standard blood draw taken between 6 and 8 a.m. typically shows a normal range of 10 to 20 mcg/dL. This test is the most familiar but also the most limited on its own, since a single snapshot can miss patterns that emerge over 24 hours.

No single test is definitive. An abnormal result on one test usually leads to a second type of test for confirmation. This is partly because cortisol fluctuates naturally and partly because certain factors can skew results.

What Can Throw Off Your Results

Several common situations produce falsely elevated cortisol readings. Oral contraceptives raise the protein that carries cortisol in your blood, which can inflate blood test results even though the active, free cortisol in your body is normal. This is one reason salivary and urine tests, which measure free cortisol, are often preferred for screening.

Certain anti-seizure medications can also interfere. Carbamazepine, for instance, produces a metabolite that mimics cortisol on some lab assays, causing urine tests to read falsely high. It can also speed up your body’s processing of the dexamethasone pill, making the suppression test appear to fail when it shouldn’t. Physical stress from illness, surgery, or intense exercise raises cortisol legitimately but temporarily, which can confuse the picture if you’re tested during those periods. Depression, alcoholism, and severe obesity can also elevate cortisol enough to mimic the hormonal pattern of true overproduction.

If your first round of results comes back elevated, your doctor will typically account for these factors before moving to more advanced testing. Being upfront about medications, recent illness, and lifestyle stressors helps avoid unnecessary follow-up.
