A healthy liver typically gives you no symptoms at all. Because the liver can function normally even when partially damaged, the absence of warning signs doesn’t guarantee everything is fine, but there are concrete ways to check. A combination of blood tests, physical indicators, and imaging can give you a clear picture of how well your liver is working.
What a Healthy Liver Looks and Feels Like
The liver sits in the upper right side of your abdomen, mostly tucked behind your rib cage. When it’s healthy, you shouldn’t be able to feel it, and it shouldn’t cause any discomfort. Your skin and the whites of your eyes should be their normal color, not yellowish. You shouldn’t bruise unusually easily, and your legs, ankles, and belly shouldn’t be swollen without explanation.
When the liver starts struggling, it often shows up in subtle ways before anything feels seriously wrong. Persistent itchy skin, dark urine, pale stools, or unexplained fatigue can all be early signals. Yellowing of the skin and eyes (jaundice) is one of the more recognizable signs of liver trouble, though on darker skin tones it can be harder to spot visually. If none of these are present, that’s a good baseline sign, but it’s not the whole story.
Blood Tests That Measure Liver Health
The most reliable way to check your liver is a simple blood draw called a liver function panel. Your doctor can order one during a routine physical, and results usually come back within a day or two. The panel measures several markers, each reflecting a different aspect of how your liver is performing.
Liver Enzymes
Two key enzymes, ALT and AST, leak into your bloodstream when liver cells are damaged or inflamed. In a healthy adult, ALT runs between 7 and 55 units per liter, and AST between 8 and 48. When both are within range, it generally means your liver cells aren’t under significant stress. Elevated levels don’t automatically mean serious disease; temporary spikes can happen from intense exercise, certain medications, or even a recent illness. But persistently high numbers warrant a closer look.
A third enzyme, ALP, ranges from 40 to 129 units per liter in healthy adults. ALP can rise due to liver problems or bone conditions, so doctors sometimes pair it with a GGT test to tell the difference. If both ALP and GGT are elevated, the issue is likely liver-related. If ALP is high but GGT is normal, a bone disorder is more probable. Normal levels of GGT on their own are a strong indicator that your liver and bile ducts are in good shape.
Bilirubin
Bilirubin is a yellow pigment your liver processes and removes from your blood. A healthy level falls between 0.1 and 1.2 milligrams per deciliter. When bilirubin builds up because the liver can’t clear it efficiently, it causes the yellowing of skin and eyes that characterizes jaundice. Normal bilirubin means your liver is handling this waste-clearing job well.
Albumin
Your liver manufactures albumin, a protein that keeps fluid balanced in your bloodstream and carries nutrients throughout your body. A healthy albumin level is 3.5 to 5.0 grams per deciliter. Low albumin can signal that the liver has lost some of its ability to produce proteins, which tends to happen in more advanced liver disease. It’s one of the markers that tells doctors how well the liver is functioning overall, not just whether it’s inflamed.
Keep in mind that standard ranges can vary slightly between laboratories, and normal values differ somewhat for women and children compared to adult men.
How Blood Clotting Reflects Liver Function
The liver produces most of the proteins your body needs to form blood clots. A test called prothrombin time (PT) measures how quickly your blood clots, and the result is often expressed as an INR score. In someone not taking blood thinners, a normal PT is 11 to 13.5 seconds, with an INR between 0.8 and 1.1.
If your blood takes longer than normal to clot, it may mean the liver isn’t producing enough clotting factors. This is why people with significant liver damage bruise easily or bleed more than expected from minor cuts. Normal clotting times are a quiet but meaningful sign that your liver’s protein-making machinery is working properly.
Imaging Tests for Deeper Answers
Blood work catches inflammation and functional problems, but it doesn’t show physical changes in the liver tissue itself. That’s where imaging comes in. An ultrasound is the most common first step. It can reveal whether your liver looks normal in size and texture, spot fatty deposits, and check for structural issues.
For a more detailed assessment, a specialized ultrasound called a FibroScan measures two things at once. The first is liver stiffness, reported in kilopascals (kPa). Normal results fall between 2 and 7 kPa. Higher stiffness suggests scarring (fibrosis), which develops when the liver has been repeatedly injured over time. The second measurement is the CAP score, which estimates how much fat is stored in the liver. A CAP score below 238 dB/m means your liver’s fat content is within the normal range.
A FibroScan is painless, takes about 10 minutes, and doesn’t require any preparation. It’s particularly useful if you have risk factors for fatty liver disease, such as carrying extra weight around the midsection, having type 2 diabetes, or drinking alcohol regularly.
Risk Factors Worth Knowing About
Even without symptoms, certain factors increase the chance that your liver may not be as healthy as it seems. Heavy or long-term alcohol use is one of the most common causes of liver damage worldwide. But non-alcoholic fatty liver disease has become equally prevalent, driven largely by obesity, insulin resistance, and diets high in processed sugar.
Other risk factors include a history of hepatitis B or C infection, regular use of medications that are processed through the liver (including over-the-counter pain relievers like acetaminophen at high doses), a family history of liver disease, and exposure to certain industrial chemicals. If any of these apply to you, periodic liver function testing is worthwhile even if you feel perfectly fine.
Putting It All Together
No single test gives you the full picture. A truly healthy liver shows normal enzyme levels, adequate albumin production, proper bilirubin clearance, normal clotting times, and a soft, non-fatty appearance on imaging. Most people only need blood work to get reassurance. If your liver panel comes back normal and you have no risk factors, your liver is almost certainly doing its job well.
If anything comes back borderline or elevated, it doesn’t necessarily mean you have liver disease. Repeat testing a few weeks later, along with a conversation with your doctor about your specific risk factors, usually clarifies whether there’s a real problem or just a temporary blip. The liver is remarkably good at repairing itself when given the chance, especially if issues are caught early.
