A hamstring pull happens when the muscle fibers at the back of your thigh are forced to lengthen while they’re actively contracting. This creates a tug-of-war inside the muscle that can overload and tear individual fibers. It most commonly occurs during sprinting, but it can also happen during a deep stretch, a high kick, or any movement that pushes the hamstring beyond what it can handle at that moment.
What Happens Inside the Muscle
Your hamstrings are a group of three muscles running from your sit bones (at the base of your pelvis) down to just below the knee. During normal movement, these muscles contract to bend your knee and help extend your hip. The trouble starts when the muscle has to lengthen and contract at the same time, something called an eccentric contraction.
During sprinting, the most vulnerable moment is the late swing phase, the split second before your foot strikes the ground. Your leg is swinging forward, stretching the hamstring to near its maximum length, while the muscle is simultaneously firing hard to decelerate the lower leg and prepare for ground contact. The hamstring reaches peak stretch just before foot strike, and that’s when fibers are most likely to fail. Research published in the British Journal of Sports Medicine confirmed that the degree of damage depends more on how much the muscle is actively stretched than on the raw force involved.
Two Distinct Ways It Happens
Not all hamstring pulls are the same injury. There are two recognized types, and they differ in mechanism, location, and recovery time.
Sprinting type: The most common variety. It happens during high-speed running and typically tears fibers in the long head of the biceps femoris (the outer hamstring muscle) near the junction where muscle meets tendon in the upper thigh. These injuries tend to cause more immediate pain and functional loss, but they generally heal faster.
Stretching type: This occurs during movements that push the hamstring into an extreme lengthened position, like a high kick, a sliding tackle, or doing the splits. It tends to involve a different muscle (the semimembranosus, on the inner side) and damages tissue closer to the sit bone. A useful rule of thumb from sports medicine: the closer the injury is to the sit bone, the longer the rehabilitation. These stretching-type injuries are deceptive because initial pain and functional impairment are often relatively mild, but recovery takes significantly longer than the sprinting type.
What a Pulled Hamstring Feels Like
The hallmark sensation is a sudden “pull” or snap at the back of the thigh during physical activity. You might feel it as a sharp grab of pain that stops you mid-stride. In milder cases, it can feel more like a cramp or tightness that worsens over the next few hours.
Hamstring strains are graded on a three-point scale:
- Grade 1: A mild strain with very little torn muscle. You’ll feel tightness and discomfort but can usually still walk without much trouble.
- Grade 2: A partial tear. Noticeable pain, some loss of strength, and often bruising or swelling within a day or two.
- Grade 3: A complete tear. Severe pain, significant loss of strength, and visible bruising. Walking is difficult.
Grade 2 and Grade 3 injuries cause a measurable loss of strength in the affected leg. If you can barely bend your knee against resistance or the back of your thigh is visibly swollen and bruised, you’re likely dealing with more than a mild strain.
Is It Your Hamstring or Something Else?
Pain at the back of the thigh isn’t always a muscle pull. Sciatic nerve irritation can mimic hamstring tightness, and the two are sometimes confused. A few differences help sort them out.
Hamstring pain stays localized to the back of the thigh or upper buttock. It usually starts suddenly during activity. The area is tender to touch, stretching the muscle reproduces the pain, and you might see bruising or swelling. There’s no tingling or numbness.
Sciatica, by contrast, tends to radiate from the lower back or buttock down the leg, often below the knee. The pain is sharp, burning, or electric. It’s triggered or worsened by sitting, bending forward, or coughing, not by running or jumping. Tingling, numbness, or weakness in the foot are strong signals that a nerve is involved rather than a muscle.
Who Is Most at Risk
Two factors stand above the rest: age and injury history. A systematic review in the British Journal of Sports Medicine found that age can start influencing hamstring injury risk in athletes as young as 24. The effect increases from there, likely because tendons and muscle fibers gradually lose some of their elasticity and ability to absorb eccentric loads.
Previous hamstring injury is the single strongest predictor of a future one. In men’s professional football, around 18% of all hamstring injuries are recurrences, and more than two-thirds of those happen within two months of returning to play. This makes the rehabilitation period critical. Coming back too early, before the muscle has regained full strength and flexibility, substantially raises the odds of reinjury.
How Long Recovery Takes
Recovery timelines vary widely depending on the grade and location of the tear. Mild Grade 1 strains often resolve within one to two weeks with rest and gradual return to activity. Moderate injuries involving deeper tears at the muscle-tendon junction average roughly 34 to 41 days before return to full training in competitive athletes. Severe tears involving the internal tendon structure can take a mean of 84 days, with a range stretching from 40 to 128 days depending on the extent of damage.
Stretching-type injuries deserve particular patience. Even when the initial symptoms seem manageable, rehabilitation tends to drag out longer than expected. Returning to activity based on how you feel rather than objective strength testing is one of the main drivers of reinjury.
Reducing Your Risk
The single most studied preventive exercise is the Nordic hamstring curl, where you kneel on the ground and slowly lower your body forward while a partner holds your ankles. This movement trains the hamstring to handle exactly the kind of eccentric loading that causes pulls. A meta-analysis of over 8,400 athletes found that programs including Nordic curls cut hamstring injury rates by 51%.
The exercise works because it strengthens the muscle specifically in its lengthened position, building the capacity to absorb the forces generated during sprinting, kicking, and sudden deceleration. Starting with two to three sessions per week during preseason, then maintaining one to two sessions per week during the competitive season, is the protocol most commonly used in the research. If you’ve never done them before, expect significant soreness for the first week or two as the muscle adapts to the eccentric demand.
