A baby comes out through the birth canal (vagina) after hours of labor that gradually open the cervix wide enough for the baby’s head to pass through. In some cases, a baby is delivered surgically through an incision in the abdomen. Either way, the process involves a remarkable sequence of physical changes in both the mother’s body and the baby’s positioning that make delivery possible.
How the Body Prepares for Birth
Long before labor starts, pregnancy hormones are already reshaping the body for delivery. A hormone called relaxin loosens the muscles, joints, and ligaments around the pelvis, back, and abdomen. This loosening allows the pelvis to widen slightly and gives the baby more room to move downward. A final surge of relaxin just before delivery helps relax the pelvis further and soften the cervix.
The cervix itself, which has stayed tightly closed throughout pregnancy, needs to undergo two changes before a baby can pass through. It must thin out (a process called effacement) and open up (dilation). Full readiness means 100% effacement and 10 centimeters of dilation, roughly the diameter of a large bagel. Getting to that point is what the first and longest stage of labor is all about.
Stage One: Contractions Open the Cervix
Labor begins when the uterus starts contracting in a regular, sustained pattern. These aren’t the random practice contractions some people feel in late pregnancy. Real labor contractions come at predictable intervals, grow stronger over time, and don’t stop when you change positions.
The driving force behind contractions is a feedback loop in the brain. When the baby’s head presses against the cervix, nerve signals travel to the brain and trigger the release of oxytocin into the bloodstream. Oxytocin stimulates the uterus to contract, which pushes the baby’s head harder against the cervix, which triggers even more oxytocin. This cycle steadily increases both the strength and frequency of contractions. Oxytocin also boosts the production of prostaglandins, hormone-like compounds that move labor along even faster.
For a first-time mother, this entire first stage typically lasts 12 to 18 hours. For someone who has given birth before, it’s usually 8 to 10 hours. Early labor contractions are mild and widely spaced. As active labor progresses, contractions become intense, lasting about 60 seconds and arriving every few minutes. The final stretch of this stage, sometimes called transition, is the most physically demanding part. Contractions are very strong and close together, and the cervix opens those last few centimeters.
Stage Two: Pushing and Delivery
Once the cervix is fully dilated, the baby is ready to move through the birth canal. This is when the pushing begins. The mother works with each contraction, bearing down to help move the baby through the pelvis and out of the body.
How the Baby Navigates the Pelvis
The birth canal isn’t a straight tube. It curves, and the baby’s head is relatively large compared to the space available. To fit through, the baby performs a series of movements, rotating and tilting as it descends. The head typically enters the pelvis facing to one side, then rotates to face the mother’s back. As the head reaches the vaginal opening, it tilts upward (extends), allowing the smallest part of the skull to lead the way. After the head emerges, the baby rotates one more time so the shoulders can slip through. These movements happen naturally in response to the shape of the pelvis pressing against the baby’s body.
Crowning and the Ring of Fire
The moment when the widest part of the baby’s head becomes visible at the vaginal opening is called crowning. Many women feel an intense burning or stinging sensation at this point, often called the “ring of fire.” The name refers to the circle the baby’s head makes as it stretches the vaginal opening to its maximum, and the burning feeling that comes with that rapid stretching of skin and tissue.
The birth team will often ask the mother to stop pushing briefly during crowning. This pause gives the tissue between the vagina and anus (the perineum) time to stretch more gradually, which helps reduce the risk of tearing. Once the head is out, the rest of the body usually follows quickly, often with just one or two more pushes.
Stage Three: The Placenta
Birth isn’t quite finished when the baby arrives. The placenta, the organ that nourished the baby throughout pregnancy, still needs to come out. After the baby is delivered, the uterus continues to contract. These contractions separate the placenta from the uterine wall, and the mother pushes gently one more time to deliver it. This stage is much shorter and less intense than the previous two, typically taking just a few minutes to half an hour.
What Happens Right After Birth
Within the first minute of life, the medical team does a quick assessment of the newborn. They check five things: breathing effort, heart rate, muscle tone, reflexes, and skin color. Each one gets a score, and the total gives the team a snapshot of how the baby is adjusting to life outside the womb. The same check is repeated at five minutes after birth to see if the scores are improving. In most cases, the baby is placed on the mother’s chest during this process.
How a Cesarean Delivery Works
About one in three births in the United States happens by cesarean section. This surgical delivery is used when vaginal birth poses risks to the mother or baby, such as when the baby is positioned feet-first, the placenta is blocking the cervix, or labor isn’t progressing safely.
During a cesarean, the surgeon makes a horizontal incision low on the abdomen, just above the pubic bone. The procedure works through several layers of tissue: skin, fat, the tough connective tissue covering the abdominal muscles, and then the lining of the abdominal cavity. The bladder is gently moved aside, and an incision is made in the uterus itself. The baby is lifted out through this opening, the umbilical cord is clamped, and the placenta is removed. Then everything is closed back up in layers.
The entire surgery typically takes 30 to 45 minutes, though the baby is usually out within the first 10 to 15. Recovery from a cesarean takes longer than a vaginal birth because it is major abdominal surgery. Most people stay in the hospital for two to four days afterward and need several weeks before returning to normal activity.
Why Labor Length Varies So Much
No two births follow the same timeline. Several factors influence how long labor takes and how the baby comes out. First-time mothers almost always have longer labors because the cervix and birth canal haven’t stretched before. The baby’s size and position matter too. A baby facing the mother’s back (the ideal position) typically moves through the pelvis more efficiently than one facing forward. The shape and dimensions of the mother’s pelvis, the strength of her contractions, and whether she’s using pain medication that might affect her ability to push all play a role. Some labors take six hours, others take more than 24, and both can be completely normal.
